TO PROVIDE A SPECIALIST PALLIATIVE CARE SERVICE FOR PATIENTS WITH SPECIFIC LIFE-LIMITING ILLNESSES LIVING IN THE AREA COVERED BY WREXHAM HOSPITALS AND TO SUPPORT THEIR FAMILIES IN A CARING ENVIRONMENT. THE ACTIVITIES WILL BE UNDERTAKEN BY A SPECIALIST TRAINED MULTI-DISCIPLINARY TEAM WHO WILL PROVIDE SYMPTOM CONTROL, ASSESSMENT, TERMINAL CARE, ONGOING EMOTIONAL AND BEREAVEMENT SUPPORT + EDUCATION

Objects: (a) To Relieve Sickness And Suffering And Promote The Spiritual Well Being Of Persons Of All Ages Requiring Palliative Care And (b) To Relieve Sickness By Extending And Promoting Screening And Treatments For Persons Of All Ages Living In The Area Covered By The Wrexham Hospitals
