Promote health and social welfare by providing community & education training programmes and the provision of social support and outreach services. Supported Accommodation provides support at home to individuals who, because of health and social barriers need extra help to live in the community. Housing Support supported almost 251 vulnerable individuals to maintain their tenancies and reduce the risk of homelessness and almost 155 individuals were able to move to independent living. Accommodation Service provided more than 70 individuals with a tenancy and removed them from the threat of homelessness. Outreach Services include crisis intervention, drug overdose, cuckooing and we also support individuals in initial custody and recently released individuals from Perth Prison with a history of substance use and a risk of homelessness. The Outreach Services have supported 1500 individuals across a wide spectrum of needs. Education Service designs & delivers programmes.
