During 1/6/24 to 31/5/25 we conceived, developed, ran,
supported and/or wrote up nine projects. All of these
projects further our charitable objects because

they all require project management (which is
used to train junior doctors and haematologists
in how to be researchers and principal
investigators) and
2)
they ultimately result in the publication of
research findings that benefit patients.
Projects take several years to complete from the time of
inception. Activity completed from 1/6/24 to 31/5/25 is

AudHIT – Heparin- Induced Thrombocytopaenia
audit

national audit

Topic:National Audit evaluating diagnosis and
management of HIT against BSH guidelines.
Included use of 4Ts score, lab testing and
treatment pathways.

HaemSTAR role:Led by Dr Richard Buka.
Data collection coordinated via the trainee

national audit.
Topic:Management of delivery and neonatal
monitoring in mothers with ITP. Comparison to
international consensus guidelines.

ATOMM (Anticoagulation for ThrOmbosis in Multiple
Myeloma)
Type of study:National, retrospective audit.
Topic:Audit of thromboprophylaxis in newly
diagnosed myeloma in the UK. To help inform
the evidence-based selection of
thromboprophylactic agents for patients
receiving first line myeloma treatment.
HaemSTAR role:Lead: Tom Bull and the
ATOMM committee. Lead site: Cambridge
University Hospitals. Project developed in
conjunction with myeloma patients. Supported
by HaemSTAR with project management by
BiCOPS. Funded by Myeloma UK.

March to July 2025. Data cleaning completed
December 2025. Hoping to present results
during 2026.
Outputs:Data analysis underway
A short questionnaire has followed on from this initial
data collection. The survey is not limited to those
involved in ATOMM, but was aimed at anyone who is
regularly responsible for prescribing thromboprophylaxis
for myeloma patients.
TXA in Heavy Menstrual Bleeding with acute VTE
Type of study:Survey of clinician practice,
observational data collection, future RCT
development.
Topic:To evaluate the use of tranexamic acid
in women with heavy menstrual bleeding and
recent VTE.
HaemSTAR role:Lead by Dr Eman Hassan.

Main outcomes:Apixaban preferred
anticoagulant for patients with HMB. Wide
variation in practice re timing of TxA use; 35%
prescribed it any time following initiation of
anticoagulation, 12% delayed its use for 3
months; 8% would never give it. 47% of
respondents advise to discontinue COCP
following VTE (whilst treated with
anticoagulants). 63% of respondents willing to
participate in a study of TxA ≤ 4 weeks
following VTE.

HaemSTAR RAPIDO (A ‘Flash-Mob’ UK national
audit of the use of Reversal Agents in Patients
antIcoagulated with Direct Oral anticoagulants)
Type of study:Retrospective National Audit.
Topic:Compliance with BSH guidelines for the
use of reversal agents inpatients with major
haemorrhage who are taking direct oral
anticoagulants (DOACs).
HaemSTAR role:Conceived and developed
by Dr Richard Buka (Stoke) and HaemSTAR.
Run by HaemSTAR. Data management
provided by BiCOPS. Funded by AstraZeneca
and the Dudley Leukaemia Unit Appear Fund.
Size of study:2265 patients.
Main outcomes:875 patients had GI
haemorrhage, 1012 had ICH. Median age 81
years. Median time from bleed onset to
reversal: 6.5 hours. Median time from last dose
of anticoagulation to reversal: 16.0 hours. 1001
of 1253 non-ICH bleeds fulfilled criteria for
major hemorrhage. Propensity score-matched
analysis of 494 patients with GI haemorrhage:
outcomes similar for andexanet alfa and four
factor prothrombin complex concentrate.
Incidence of stroke was significantly higher
with andexanet alfa.
Outputs:Poster presentation at ASH Annual
Congress 2024 and BSHT Annual Conference
2025.Paper in process within the above

MATRON (Maternal Audit on ThRombolysis
Outcome and DecisioN) - Survey
Type of study:UK Clinician Survey
Topic:The use of thrombolysis for pregnant
women with venous thromboembolism.
Size of study:Survey with 70 responses.
Target audit responses of 200 patients
(completion Q2 2025).
HaemSTAR Role:Conceived and developed
by Dr Giulia Simini (Royal London &
HaemSTAR). Run by HaemSTAR. Data
management by HaemSTAR.
Main outcomes:Survey: CDT available to
74%, Surgical thrombectomy to 62% and
ECMO to 52% of respondents. Only 50% of
sites had specific guidelines on CDT and 17%
had no guidelines on managing pregnancy
associated PE. 38% had received specific
training on pregnancy-associated PE. 22% had
had training in systemic thrombolysis. 46% did
not have access to PERT with geographic
inconsistency. Audit in planning stages.

MATRON – Audit

Topic:Aiming to assess management of high
risk and intermediate-high risk pulmonary

Size of study:75 cases.

VAMP: Venesection vs Active Monitoring in
Secondary Polycythaemia
Type of study:International clinician survey,
retrospective audit and then prospective
clinical trial.
Topic:Venesection in patients with idiopathic
and secondary erythrocytosis
Size of study:Survey with 123 responses (90
senior clinicians). Audit with 206 patients.
Prospective study in development.
HaemSTAR Role:Conceived and developed
by Dr Richard Buka, Dr Bernard Maybury and
Dr Pip Nicolson (Birmingham & HaemSTAR),

Main outcomes:

Survey: minority venesect routinely,
2/3 will venesect in specific
circumstances; 1/3 never venesect.
More likely to venesect if previous
thrombosis or if headache or visual
disburbance. Majority use Hct
threshold of 0.55 but some use 0.6
and most have Hct target of <0.55
but some use <0.52. 85% willing to
consider entering patients into a trial.

whether polycythaemia resolves.
Cancer and prior unprovoked VTE
are the only factors to predict
subsequent thrombosis.

Anticoagulation at the end of life
Type of study:National survey
Topic:To gauge current practice and decision-
making surrounding anticoagulation at the end
of life. Hoping to pave the way for further
research in the area.
Size of study:186 responses.
HaemSTAR involvement:led by Dr Thomas
Shevlin and Dr Olga Tsiamita. Distributed by
HaemSTAR network.
Main outcomes:56% of respondents routinely
seek advice from colleagues regarding these
decisions. 53% of respondents reported having
worried about the medico-legal implications of
discontinuing OAC, 52% of whom reported the
worries influence their decision making. 83%
would feel more confident in broaching this
subject if national guidance existed.
Outputs: Publication: clinical medicine: Are we
comfortable managing oral anticoagulation at
the end of life? A national survey of secondary
care clinicians in the UK
In set up:
Perioperative management of Haemophilia B
‘FIX-Surg’
The primary objectives of the project are to audit the
haemostatic management of patients with haemophilia B
who are undergoing surgery against World Federation of
Haemophilia guidelines. Project running from April to
June 2025.
PaTRIC– Practice of Transfusion In Cirrhosis
PaTRIC is the UK arm of an international audit
evaluating real-world transfusion practices in
hospitalised patients with cirrhosis, focusing on platelet
and coagulation factor replacement. Anticipated data
collection March to June 2026.
TRUTH- sTeroids in warm aUToimmune Haemolytic
anaemia
TRUTH is a UK-wide audit of the management of warm
autoimmune haemolytic anaemia, focusing on
corticosteroid use. Aiming to open in March 2026.
AQuA- Acquired Haemophilia A – National Audit
Acquired haemophilia A is a rare, potentially life-
threatening bleeding disorder caused by autoantibodies
against FVIII. There are national BSH guidelines for the
‘Diagnosis and Management of Acquired Coagulation
Factor Inhibitor’ from 2013, and international consensus
recommendations from 2020, but management options
have changed considerably over this time and local
practices across the UK differ.
The HaemSTAR hub-and-spoke model is an ideal
platform for looking at rare diseases. AQuA is a
multicentre, national audit, comprising a 5-year
retrospective review of patients diagnosed with acquired
haemophilia A. We aim to include a minimum of 6
comprehensive care centres in the audit.
FUNDING AWARDS
From the ISTH London 2022 Legacy Fund we were able
to fund travel grants for trainees attending conferences
(e.g. EAHAD 2025) and the International fellowship
programme (Nigeria collaboration). This enabled
conference attendance, presentation of HaemSTAR
work, professional development and ongoing
international collaboration work.
Awards were through competitive application with award
by grant panel.

CONFERENCE
In association with Hartley Taylor and the Post
ISTH/ASH UK Update meeting we hosted a HaemSTAR
Update day on 10/2/25 at The Liner Hotel, Liverpool.
This comprised updates and results on current
HaemSTAR projects and plenary talks from experts on
research in global health settings, ‘blood is the heart of
medicine’ and National haemovigilance updates (SHOT).
Very positive feedback. Attendance was free and was
increased from the year before. Free registration for
HaemSTAR conference helped with travel
reimbursement to enable attendance as well supporting
the invited speakers.
HaemSTAR breakfast Briefings
In collaboration with ‘The Limbic’ we provide monthly,
free, online webinars led by experts with interactive Q&A
segments. These are targeted at haematology trainees
and consultants. They are predominantly focused on
non-malignant haematology education and research
translation. Topics in the last year have included VTE in
pregnancy, genetic testing in haemostasis, iron
deficiency, PNH and new therapies.
The Limbic reimburse £5 to HaemSTAR for every
attendee that fills in feedback surveys.
PODCAST- Don’t Just Read the Abstract
Hosted by Dr Richard Buka and Dr Pip Nicolson, the
focus of discussion is of recent research papers in non-
malignant haematology, critical appraisal of evidence
and expert guest interviews. The purpose of it is to help
trainees go beyond abstracts – to understand methods,
bias and clinical impact.
BLOG
HaemSTAR maintains a regular blog with contents
which include conference reflections, trainee
experiences, project updates, and thought pieces on
collaboration and research.
BSH CONFERENCE SESSION
HaemSTAR delivered a dedicated session within the
BSH 2025 programme. This included an overview of
HaemSTAR, project presentations, how to get involved
and research education. This raised awareness of
ongoing HaemSTAR studies, research opportunities and
encouraged new project proposals. HaemSTAR also
had its own stall with promotion of current and upcoming
studies, HaemSTAR events, signposting to website and
project sign-up and networking with potential
collaborators.

funds and £25,742 in restricted funds for ISTH Legacy
fund spending
committed spending for 2024-25 comprises website
hosting fees of approximately £350, podcast hosting
fees of £200, accountancy fees of ~£150 and
administration costs (paid for the time of our
administrator) of ~£3000. This is all covered by the
administrative grant of £5000 from Takeda and does not
represent a financial risk. We also aim to apply for
additional sponsorship of the podcast for when the Sobi
sponsorship ends in 2025.
In 2023-24 we expanded our ability to fund registrars to
attend conferences. £19,200 of this comes from the
ISTH Legacy fund and we are able to use this until
2026. It is unlikely that costs will exceed this figure.
The educational webinars are a source of an
approximately £1200 annual cost. This is voluntary
however and can be stopped if there are insufficient
funds.
The HaemSTAR Update Day is now a recurring event
but this is organised through Hartley Taylor and they
secure industry support for the meeting. The only
HaemSTAR costs are voluntary and will not go ahead if
there are insufficient funds.
All the projects currently active are fully funded.
However we would like to run unfunded projects by
using money underspent from other projects and also
pump prime our pipeline by employing a project/data
manager through the University of Birmingham at a cost
of approximately £70,000 per year. This is not yet
feasible and will not go ahead unless there are sufficient
funds. The funding for this will be reviewed on an annual
basis and no commitments beyond 1 year will be made
to ensure there is no financial risk to the charity.

I report to the trustees on my examination of the accounts of the above charity (“the Trust”) for the year ended 31 May 2025

Responsibilities and As the charity's trustees, you are responsible for the preparation of the basis of report accounts in accordance with the requirements of the Charities Act 2011 (“the Act”).

I report in respect of my examination of the Trust’s accounts carried out under section 145 of the 2011 Act and in carrying out my examination, I have followed all the applicable Directions given by the Charity Commission under section 145(5)(b) of the Act.
