At the heart of this model is a Specialist Pallialwe Nursing Team thal is available 2417 and provides contlnuity of eare for all pallialive and end-of-life patients and their families. Servi¢as provided include. SP8aalist symptom control for patients wf(h complex needs.. hospice at home rbursing r3ra to enable more patients to rgmain in their own homes and prevent unnecessary hospital admissions. psychological and emotional support for 811 patients and familles., COLTrrdinalion and oversight of wder community support lo mael the indiwdual needs of each patient and IheiT family.
