It is an 'HDAC inhibrtor., il blocks enzyme5 called histone deacetylases, which are invofved in turning genes 'on' and 'off within cells lo help reduce inflammation and muscle loss. Work is underway lo hopefulty gel this drug approved by NICE so that every child and young adutt living with Duchenne can access this drug through the NHS. In the meantime, this drug has been offered free of charge In the UK through the earty access programme. However. il has been the decision of each hospital trust lo decide whether they will provide il lo their Duchenne community. Some have, and Some slill haven't. Duchenne UK and the Duchenne communrty has been working incredibly hard mpaignin9 for a¢ce$$ o¢ros$ the UK. We have been supporting NICE as a patient represenlalive, offering insights into why gaining access to this drug is so important. Whilst Alex'$ Wish did not directly financial contribute towards this projecl. as a family we have been tsking part in the clinical trial over several years. Gen• Th•rapy We were told that Gene Therapy wouldnl happen in our son's lrfelime. During the time that Alex's Wish ha$ bèen in operation, we supported Solid Biosciences, a start-up to pur$ue this new thnolOgY, and boys are now being dosed with gene therapy in clinical trials taking place now in the UK. We know there are Challenges with Gene Therapy. which is why we are continuing lo support projects that are looking lo address these problems - and bring better therapies lo our children. Bèst practlce In Duchenne care We are working alongside DMD ca UK. to help support better qu81ty of care for everyone liwng wrth Duchenne in the UK. To date. wfj have fvnded several projects. including fi'nancial support lor a DMD Hub Project Manager designed lo increase clinical trial capacty in the UK and, a nutrrtn programme supporting those affected by Duchenne, and a project k)oking lo address the lack of accessib housing arnongsl our community. Heart cells are replaced by scar and fatty tissue. which leads to a type of heart muscle disease called dilated cardiomyopathy. The age when this happens In Duchenne can vary from person lo pèrson, from earty childhood lo adutthood. Children with Duchenne don't usually have cardiac symptoms. Howevef, weakening heart muscles are part of the natural progre$$ion of the disease. This Iransrtion period is a critical lime, not just for medical management, bul also for the psychosocial support these young individuals need lo thrive as they move into adurthood. Sadly. this Iransrtion period often lacks the necessary slru¢ture and support. famils are left nawgating a fragmented heatthcare system with varying levels of support across the Untry. The consequences ol not having a cleaf and WelsUppOrted transrtion pathway are severe. Wrthout proper Iransrtion planning, young people often lace abwpl changes in their care. leading to confusion, anxiety and in some cases delerioralion in their heatth. Additionalty. the variation in services across the country means that while some re¢eive excellent care, others are left wrthout the necessary resoLsrces and support, particularty when their condrtion becomes more Complex. A UK wide survey of families of boys showed that for 46% of palients, nulrilional issues are nol monitored or addressed in the clinic. The 2018 inlemational standards of care recommend that a registered dietician should assess nutrrtional status and ¢ree a specth'c nutritional plan, both of which are currently not available lo UK patients through the NHS. Taken together. these highlight a major gap in UK care ol boys wrth Dijchenne. which we are uniquely placed lo help address through DMD Care UK and funding this project.
