How we docldo what to fund. Every proj•cl is revwrtd by an independent sC￿n11fiC Advisory Board, made up
of leading global experts in Duchenne. This en$ure$ that funding only goes lo the best $¢18nce worldwide. No
duplication of existing research. Every prtyect has a clear path of delivery. Treatments move faster from the lab
lo the patient. Wo work closely wrth Duchenne UK. lo maximise impad and accderale progress.
How we got h•ro. Alex's Wlsh was founded by Emma and Andy Hallam after Iheir son Alex was dia9nos8d with
Duchgnne shortly before his fourth birthday.
Al the lime, support was limited. We were told lo lake Alex hor￿ and live the best life po$$iblo. Long-lerm
steroids were the onty Irealmenl option offered. despite significant Gide effects. There was no eure, and little
hop8 of modi￿1 breakthroughs.
Since Alex's diagnosis in 2010, Emma and a small, deduted team - supported by an incredible community of
trustees, amb888adors, supporters, and p8rtners- have W0￿ed tirele88ly lo create real, measurable change.
Alex has played his part loo. spending much of his youn9 lrfe taking part in clinical trials that havè hèlped
advance treatments and deliver genuine progress.

The science is 81 a very exciting lime." a lime of optimism as new Irealmenls have emerged, one of which is
proving to be delaying disease piogression. We are aware thal time is not on our side. and that for families
affected by Duchenne - whether they have a child diagnosed several year5 ago, or families receiving a recent
diagnosis - they need solutions to support thetr chikl. This is what drtves us every single day. lo lundraise and
raise awareness lo continue supportsng nthv innovative projacts to address the problems within the Duchenne
communty. This financial year, we have been 8ble lo support a further four projects.
Vamorolone la stsiold altematlv• drug)
2023 was a bieakthrough year
the first ever drug approved for use on ¢hildren with Duchenne
¢alled
Vamorolone, thanks lo charities like Aex's Wsh who helped fund the earty stages of this project. When Alex was
diagnosed back in 2010. we were told the slandaid medication for chIld￿n wrth Duchenne. steroids, would keep
them independenlty mobile for longer. bul wrth hamiful s￿e effeds. Vamorolone a new Steroid drug, th81 h85 less
h8mful side*ffecls lo traditional steroids, has been approved by the Medicine5 and Heatthcare produ¢t$
Regulatory Agenw IMHRAI in the UK. We are incredibty proud that we were able to invest in the earty stages lo
make this a realty proof that Alex'5 Wish is a charity that is making a positive impact for this generation ol
childfen and young adutts IT￿n9 wrth Duchenne.

Glvino$tat (provin9 to delay disease pro9ressionl
2024 was another breakthrough year- Gwinoslal. a drug lo help delay disease progress has been conditionally
approved in the UK by MHRA. but it is not yel readity available through the NHS. It is an 'HDAC inhibrtor., il
blocks enzyme5 called histone deacetylases, which are invofved in turning genes 'on' and 'off within cells lo help
reduce inflammation and muscle loss. Work is underway lo hopefulty gel this drug approved by NICE so that
every child and young adutt living with Duchenne can access this drug through the NHS. In the meantime, this
drug has been offered free of charge In the UK through the earty access programme. However. il has been the
decision of each hospital trust lo decide whether they will provide il lo their Duchenne community. Some have,
and Some slill haven't. Duchenne UK and the Duchenne communrty has been working incredibly hard
mpaignin9 for a¢ce$$ o¢ros$ the UK. We have been supporting NICE as a patient represenlalive, offering
insights into why gaining access to this drug is so important. Whilst Alex'$ Wish did not directly financial
contribute towards this projecl. as a family we have been tsking part in the clinical trial over several years.
Gen• Th•rapy
We were told that Gene Therapy wouldnl happen in our son's lrfelime. During the time that Alex's Wish ha$ bèen
in operation, we supported Solid Biosciences, a start-up to pur$ue this new t￿hnolOgY, and boys are now being
dosed with gene therapy in clinical trials taking place now in the UK. We know there are Challenges with Gene
Therapy. which is why we are continuing lo support projects that are looking lo address these problems - and
bring better therapies lo our children.
Bèst practlce In Duchenne care
We are working alongside DMD ca￿ UK. to help support better qu81ty of care for everyone liwng wrth Duchenne
in the UK. To date. wfj have fvnded several projects. including fi'nancial support lor a DMD Hub Project Manager
designed lo increase clinical trial capacty in the UK and, a nutrrt￿n programme supporting those affected by
Duchenne, and a project k)oking lo address the lack of accessib￿ housing arnongsl our community. This
financial year, we have supporte¢J a transitioning project. supporting young adulls living wrth Duchenne during the
transitioning stage when they move from Paed￿1￿C care Into adum services- somgthing which we have iecenlly
encountered ourselves with Alex.
We are incredibly posrtive about Ihe fulure and Ale￿5 Wish gTves us ho￿. Ouchenne Muscular Dystrophy i¥ part
of our lives, not out of choKo, but we wll make it 8$ best as we cgn. and we will never slop our journey lo
conquering Duchenne.

Wish and are also a parent-led charty. As a Charty Partner to Duchenne UK, we receive proie¢ts that require
co-funding. All such projects have been rewewe(l extenswety by their SC￿nI￿￿ Advisory Board ISABI which is
made up of some of the worfd's leading experts in Duchenne Musculai Dystrophy. They bling with them a wide
range ol essential skills and knowledge basis including scientTfK, clinical and drug discovery and development.
When scientists approach DLschenne UK with their ideas fof ne•V research Pfojects rt is the SAB who apply their
exporience and expertise lo lesl these ideas. helping lo refine them. rf necessary. in order that Duchenne UK can

Last year we gave £63,IXIO, and thi$ financ￿1 year, we have giwn ar￿ther £40.000 tty4ard$ thi$ important
project. Our heart rnuscles need dystrophin to fijnclion efficienlty. In Duchenne. a lack ol dystrophin means that
heart muscles weaken over lime. Heart cells are replaced by scar and fatty tissue. which leads to a type of heart
muscle disease called dilated cardiomyopathy. The age when this happens In Duchenne can vary from person lo
pèrson, from earty childhood lo adutthood. Children with Duchenne don't usually have cardiac symptoms.
Howevef, weakening heart muscles are part of the natural progre$$ion of the disease. Therefore. gady
monitoring, pieventalive treatment, and ongoing management are Important for keeping the heart healthy into
adutthood. 1115 curTent￿ not possible to PTevenl heart weakness in Duchenne completely. However. steroids and
heart medication can slow down the decline in heart function.
Thi$ grant call will help us understand how the heart is affècted and Idenlfy potential innovative Irealmenl
strategies and approaches. In Tesponse lo a huge unmet need in cardiac management and effective Iherapeulic
options largeled to at the heart. leading to poor outcomes in people living with Duchenne. This 1$ an area of
focus for us arsd thrilled lo share that cardiac projects are now pushing forward, as follows.. One in

Among the recent improvements In adult heart failure management. the s(xlium glu¢os8 type 2 inhibitor ISGLT21
empagliflozin was found lo reduce cardiov85cular death or heart failure by 25°A. However. little is known on ils
use in children or adolescents with Duchenne. cur￿￿1 heart failure therapy in children is unsatisfactory and heart
failure trials in children have historically often failed be￿uSe the nght dose was not used or inappropriate
formulations of the drug and clinical endpoints were adopted in the trials. We have funded this project. enabling
the team lo explore whether this drug can be repurposed for the Duchenne heart in a clinical study which
addres$e$ all the limitations above. If the optimal dose is ￿nfirMed and early efficacy signals are detected In this
pilot study, this will allow for a larger study lo demonslrale the benefit of this therapy of Duchen￿￿$S0cIaIed
cardiomyopathy.

the heart, and more clinical research is r￿ded lo better understand how these individuals can be beller
supported and Irealed. We are supporting a team as they lead the development of a suNey which aims lo gain
deeper insights into the clinical Signs and $ympt¢)ms thal are observed by medical professionals which show
what being a carrier looks like, how carriers health is tested ar￿ observed. l$pecifically heart and muscle health),
how genetic risks are managed and addressed. and the impact of caring for an affeeled person on physi¢81
symptoms and mental health. The results of the survey wll infofm a much-needed natural history study, develop

therapy for Duchenne Musculai Dyslfophy. The proiecl enlilled 'OptiM￿all9n of a gene editing therapy. will focus
on refining MyoGene's cutting-edge CRISPAJCas 9-based therapy. MyoDys4>55 aims lo permanenlty delelg
mutslion hol$pol in the DMD gene, restoring the reading frame and enabling produclion of 8 shorter but
functional dystrophin protèin, like that lound In individuals with Becker Muscular Dystrophy la milder forn of
Muscular Dyslrophy). The therapy offers several advantages over existing genetic medicines, includin9
pemianenl DNA edrting. preservation of natural gone regulation. and restor*ion of a dystrophin protein three
lime5 the size of micrctrdystrophin.
If successful, the therapy could be suitable for approximalety 50% of boys wrth Duchenne. The funding will
enable MyoGene to optimise Mltsc￿ targeting. assess dufabilty and explore potential Irealmenl regimen5 that
could improve edrting effic￿ncy. The company has already engaged the FDA in the US to accelerate their abilty
lo tonducl a elinical trial in DMD boys. and all being well. they should be in the clinic In 2027.

For young people living wrth Duchenne, the transiti'on fr(yn paediatri¢ to adult care 1$ fraught wrth challenges.
This Iransrtion period is a critical lime, not just for medical management, bul also for the psychosocial support
these young individuals need lo thrive as they move into adurthood. Sadly. this Iransrtion period often lacks the
necessary slru¢ture and support. famil*s are left nawgating a fragmented heatthcare system with varying levels
of support across the ￿Untry.
The consequences ol not having a cleaf and Wel￿sUppOrted transrtion pathway are severe. Wrthout proper
Iransrtion planning, young people often lace abwpl changes in their care. leading to confusion, anxiety and in

from previous prototypes. In 2025. user<enlred producl lesling of several prototypes was condutled in the real
world, and we are n¢)w al a stage where most ol the key engineering chaI￿nge$ have been resolved. The next
stage is lo complete development of the final lealures of the mechanical architecture as well as the design.

Developing nutrltional guld8lln8s. resou￿•$ and a $tru¢tured nutrltlonal programme £50.865 glven
towards thls prolect - Obesty Is a Ser￿￿$ heatth complication in Duchenne. with negative implications on
cardiac and respiratory function. mental health. and qualty of lrfe. Numerous factors contribute lo weight gain,
including limited physical acliv¢ty and the use of k*ng-temi steroids. Whilst the need for better diet and nutrrtional
management in boys with Duchenne is recognised. this aspect of care is not addressed in the NHS, due to lack
of resoU￿e$ and $pecrfi¢ gui¢Jance. Tho solution is lo develop the evidence requiied lo affect change in
Duchenne weight management care and a range of resources. tailored lo the need$ of l)oy$ with Duchenne and
their families, that they can access Part￿VIadY after Initiat￿ft of Steroid Ir&*menl.
There is currently no structured nulrrtional and wevJhl rnanagement guidance available lo boys with Duchenne in
the UK. A UK wide survey of families of boys showed that for 46% of palients, nulrilional issues are nol
monitored or addressed in the clinic. The survey also htghlighled that Duchenne spe¢ifi¢ nutrrtional and lifestyle
advice is Conside￿d essential by these familos. The 2018 inlemational standards of care recommend that a
registered dietician should assess nutrrtional status and ¢re*e a specth'c nutritional plan, both of which are
currently not available lo UK patients through the NHS. Taken together. these highlight a major gap in UK care ol
boys wrth Dijchenne. which we are uniquely placed lo help address through DMD Care UK and funding this
project. This prqecl 15 ongoing, and the NulrinD study is up and running and wwiting, with the aim of identifying
the caloric needs of both ambulant non4mbulant boys on steThd therapy.

opened working groups across all agreed standards of ¢are. Each working group is working towards having
published guidelines. endofsed by the ielevant professional bodies cardiac, respiratory, bone and endocrine.
Patient and family guides have been produced in paral￿1 wrth a publication of each clinieal guideline and
consullalion with the Famity Focus Group of DMD Care UK. This enables patients anil families lo better
advocate for their Ca￿ and to bring latesl infomwtson lo the attention of their clinical teams as well as lo make
informed choices about their Care.
DMD Hub 1$ a flagship project that has successfully expandèd eapaclty for clln5cal trlals for Duchennè
treatments Sn the UK - In the first lour years of the piojecl. Duchenne UK. and its partner charities (including
Alex's Wishl invested £2.7m in the DMD Hub, with an additional £1.6m ¢ommrtted over the next 4 years. The
DMD Hub is a collaboration befvleen Duchenne UK, the John Walton Muscular ￿$t[OphY Research Centre
IJWMDRCI in Newcastle, and Great Ormond Street Hosprtal London. It was set up to expand clinical trial
capacity and expertise and stop ¢lini¢al trials being turned away from the UK. The project team devolopod the

Fibrosis can lead lo failure of the heart and respiratory muscles. which Is the most common cause of death in
adults wrth Duchenne. While gene therapies could be Iransfom)alive for people living with Duchenne, the
progressive nalure ol the disease Iimils their impact. Combining gene therapies with drugs th81 could reduce
muscle delefioration. such as anli-fibrolics could help to maximise the effect of these Irealmènls. Anti-fibrotics
could also be used lo treat patients who are not eligible for gene therapy. and for those in the later stages of the
disease. We funded a PhD post for 3 years. which concluded at the end 012025. during this lime the toam has
selected a short-list of potentially Interesting compounds and were studying th6 effects of selected diugs, more

the diseased muscle tissue wrth stem cells that can create healthy myofibèrs which are able lo regenerate. Thè
aim ol this project is lo produce and test these stem Cells to gel the pwlinical data required lo progress lo
