ANNUAL REPORT YEAR ENDED 31 MARCH 2024 Despite a lengthy 8tay in the neuro Trhab ward, JL was thclared as havin8 'platsaued', with Sever¢ deficits particularly around memory and cognition. JL was found to lack m¢ntal capcity for health and welfare and would not be able to live independently. As such, Adult Social Care d¢cid¢d that the only 8Uitabl¢ residence for JL to be discharged to was a nursing home. This was du¢ to his exten8iv¢ nursing needs (meththe treaknenl use of PRN Medicatio assistance with dressing and showering. ets), and also the fact that he was under a Deprivation of LilKrty order due to hi8 bigh absconding risk JL had limited family involvement in his care, and the only per8on who was actively involved in his rewvery was his daughter.
