Governance and management Goals of Care Charity is governed by a consiitulion registered with the Office of the Scottish Charities Regulator on 24" October 2019. Goals of Care charity is governed by a board of Charlty Trustees, the initial three being those who signed the trustee declaration forms on incorporation of the charity. Hitherto, Charity Trustees have retired from office in rotation but have been eligible for re-appoiniment_ Charity Trustees ore appointed by the board. The board and charity are led by the chair, who is also it5 principal executive off icer. There are suff icient operational and governance procedures in place with the other tru51ee5 to ensure adequate supervision and division of duties. Charitable Purposes. Activities and Outcomes Charitable Purposes The Charity was established .to promote greater understanding about health-related decision-making to the general public, as well as to clinicians and other5 in medically-related professions. .to encourage and inform understandin8 by members of the public about medical decision- making. in particular for those or those with a family member who are facing critical or life- limiting illness Activities and Outcomes Goals of Care Charity has 3 royalty free licence in the UK to publish Coping with Crisis, a booklet written by Professor Robin Taylor specif ically to assist patients and families in navigating their way through the tensions and difficult decisions that arise when facing major or critical illness. The aims of the charity continued to be fulfilled during 2023-2024, although at much reduced scale.

Circulation of Coping with Crisis during the 2023-24 financial year. The challenges of marketing and distributing Coping with Crisis continued. Educational literatu re that 15 Tlot primarily produced within the NHS itself usually does not get approved if individuals apply for funding. I nterested hea Ithcare profe55iona1s have been very happy to receive copies gruri5 which have been provided to relevant persons in order to promote the goals of the charity. Flowever, a few sales were made during 2023-4. 2. Teaching and education among health professionals in the NHS. Most of activities in the year to October 2024 were based in NHS Greater Glasgow and Clyde. In boili th e Royal Iiiverclyde Hospital, Greenock and the Royal Alexand ra Hospital, Paislpy continued to implement Treatment Escalation Planning as dd¥l>ei Iu i¥HS GGC staff. Having coTrversations abolit troatm(bnt chi)icps particularly at end-of-life is an integral part of the TEP programme. The philosophy undergirding TEP work 15 in keeping with the aims of the Goals of Care Charity although there is no formal link and there are no financial implications lo the Trust. The TEP concept now has nationwide importance and uptake. In October 2023, a National Advi50ry Group INAGI was instituted under the Scottish Government Department of Health under the auspices as parl of the Future Care Planning initiative. The NAG is currently chaired by a renal physician in N HS Lanarkshire l and there are 12 consultants drawn from the majority of NHS regions in ScoLlai)d Iii the Group. Teachin8 and education opportunities on the Deteriorating Patient and the TEP themes have continued to be numerous. Where possible Iface to faie rather than virtuall or necessary, existing copies of CWC have been distributed to staff. This includes all of the final year undergraduate medical Students in the University of Edinburgh and the University of Aberdeen.

Operations and Finances During the year ended 31" Ottober 2024. the charity had a net deficit of £226 being the excess of costs of domain names and website hosting over sales of Coping with Crisis. It has cash reserves of £2612. During the year the charity has continued to use Tricord Limited as Its warehousing and fulfilment partner for Copin9 Wlth Crisis. We are very grateful for their continued assistance. Future of the Goals of Care Trust The maioiity of the Trust's activities over the13st 5 years have been undertaken by ofieii 111 the context of his professional role as a consultant physician within the NHS 1% shortly going lo retire from medical practice on November 3011. 21J24. 1 he I fustees are agreed that thereafter, the activities of the GOC Trust will cease. rhe Trustees have agreed thal all remainin£ funds in the GOC account will be donated to PATCH, a registered Scottish Charity15C04423 11. The PATCH Trustees provided start-up funding to the GOC Trust In 2019. Tru5tees' remuneration and expenses The Trustees received no remuneration in the year. None were reimbursed for any personal expenses incurred. Statement of Trustees, Responsibllities The Tru5tee5 prepare financial Statements which give suff icient detail to enable an appreciation of the transactions of the Charity durin8 the financial year. The TTilStees have been responsible for keeping proper accounting record5 which, on request, reflect the financial position of the Charity at that time. This is dono to ensure that the financial statements comply with the Charities and Trustee Investment Iscotlandl Act 2005 and the Charities Accou nis15cotlandl Regulations 21X)6. The Trustees are also responsible for safeguarding any asset5 of the Charity and mv5t take reasonable Steps for the prevention ndlor detection of fraud and other irregularities.
