**What the charity does:** The Lade Centre is a community centre, wholly run by volunteers, which aims to support people who have, or care for people with life-limiting illnesses or long-term conditions. It also aims to reduce social isolation through group activity. The Centre has three main objectives: to create a drop-in centre where people can socialise and relax, combating isolation and providing social support. to provide professional support to people with long-term conditions and their carers, led by A&amp;B HSCP MacMillan Nurse and supported by a team of therapists. and to reduce social isolation through the provision of an increasing range of activities to improve quality of life.

**Objectives:** The organisation&#x27;s purpose is to relieve the needs of people who are disadvantaged due to debilitating, life-limiting and/or long term medical conditions, in particular by means of activities that will:  4.1 support their health and wellbeing. and  4.2 reduce their social isolation.
