The Paediatric Diabete5 SeMc4 is still golng from strength to strength. We have continued lo support them lo become

During this financial yoar we have been PLthing for tfansition services to be stsrted for the children when they berL)m8
adults. The Paedi8lric Diabetes Sgrv((x is excellènt but the young people seem to fall off a drff when they reach
adulthood and donl receive the close monitoring that they are used to. We aim to prevent this situation by having a
transition seFvice that helps them genly along the path to adulthood. Ouring 2021r22 will continue our quest for

Annual review Icontinu•d)
The Trust is committed to invest in professional and patient educatlon. We have continued lo support robust Irainlng
for GP'S and nurses tr81 will help to drfve up the professior7aS knovAedge that is required to deal wth DiBbetes. The
Trust has this year embarked on a series of new inits'alives in West Kent and Medway th professional training coursès
being organised now. We have also allocated and are preparing advanced support fur our patfjen15. To aid patient
education and to empower them wth enough knowledge to make important decisions about their treatment and control,
we a￿ lunding a nufnber of projects that help support patient understanding. We have continued the support of
children's patient groups for those under 18. Better knowledge and greBter patient empowerment should help to save
the NHS a lot of money in reduced costs for long tami problems. bul mors importantly will help in saving patients from
long term suffèring. Thls yaar we fvnded somo very interesting work In Medway, which has some very poorly controlled
Diabetes patients. In one part ol Methay we employed a dinical phamiacist who ￿d$ tasked vAlh offering one to one
appointmèrrts in an effort lo get botter patient control of their Diabetes. In the other part of Medway, we employed
0i8beles Nurse Specialtst to carry out similar work. l am pleased to report that as I write this re￿&w, that IhÈ results
Seen so far are exceptional and as a resutt of the success is being rolled out fijrther into Kent.
We contSnue to fund spedalFsed foolweor and walking aids for wdt81ry and contonue to SUPPOrt the Vas￿1￿r

The Tnjst continues to meet the runnlng costs of 4 glycaled haemoglobin machines. These machines enable chlldren
to have 3 regular blood test which assesses how well Iheif diabetes is being looked after.
One of the maior PToblems faang the NHS is the conlinued increase in the number of patietts getting Typè 2 Dl8betes.
Not only Is this conditlon mostly unnecessary. it is also very expensNe bÈe2use of increased costs of treating the
re$ult$ of negknt, 8uch 85 blindness, arnputation and cliatysls. 11 Is time for this trend lo be stopped In its track and we
¢onlinutr looking 8t ways to gel people back to exerrAse with a wew to irnPTowng their long temi outlook. With the
changes in the delivery of healthcare throLtghout the NHS and wth the inadence of D18bete8 continuing to increase
throughout the UK, the Trust is still re8dy, wslling and vary able to meet the problems a88ociated wfth this difficu
condition head on. We ￿1[ continue to segk oui ngw solutlons In support of those patients who suffer an ever Increasing
