Case study one: A 95-year-ald gentleman who is b8dbound Ènd Iivos on his own is totally rdlant on CATH lor all
his care needs and advocacy for bills and mainlenance of the house.
This gentleman lives in a soci81 housing b8sÈd in E8St London and had numerous issues outsianding from the
Electric shower, imrnersion hot water tank not working and mi¢8 spottod Inside Ihe propety wth an excessively
overgrown garden. All issues had been ouistanding for over 6 rn(y)Ihs.
In addition to the malntenance issues. the pump for the air mattress was inlermitlently working. This mattr885 wa
hlgh prk)rity as It helped this gentleman move around or change p￿tIOnS to prevenl pressure ulcers.
Alr Mattress
The Outy OT sént a réferral to ihe DN advising Ihal a specialist bed order that was not a nom)al catalogue item is to
be ordered vrith 8 joinl order visit, allowing a $pe(ific delivery timè slot to b8 trrt)oked.
Our Care Coordinator Donna called NRS and the district nursing team dally for an update but lound no order had
baèn pl3c8d. 11 was apparént the district nurses dtdn'l know the protocd on h¢)W to false an order for the mattress,
nor did they know how lo rals8 a jolnt vislt order.
Our carè workers had lo be extr8 vigi18nl in checking for pressure sores and repositioning this g￿tleMen 81 regular
Intervals to prevent any pressure sor85 or any ge￿Ing wor88.
A8 a matter of weeks passed. the concem aboul pressure sores was incfe8sing. Donna had no cholce but to raise a
complaint to PALS 88 li was becoming a safègu8rding Issue.
Thère ￿re a lot of miscommunications be￿en NRS and the dI$￿ct nurse team vthich delayed the ordering and
th8 dalfvory of ihe maltress.
Malntenance Issues
The houslng asso¢iation ralused to send in pest control unless the garden was ¢omplot¢ly cleared and it is the
homeowner's responsibility. Unfortunately, thg garden had nol been maintaine(I for years and has been o dumping
ground for years of unwanled fumilure and other haus8hdd debris.
Donn8 ar￿u1r8d wilh local charil¢es. bul they were unable to help dua to the volume of WO￿ Snvolved. After sorne
eliberation and wilh the fa¢t the pest conirol Issue will only gel worse unless w8 h81p this gentl￿an out, Ihe
housing association de¢lde<l lo help as a one-off. They arranged for a garden de8r8nce contr8Ctor to dear the
garden and transfom il back lo Its ortglnal state and the pest o)ntrol was under control.
Tho ￿MbinatIon of having incorrect contact detsils and numèrous wrong parts being ordered for the lobs
t￿nt￿but$d to the detay In addressing the outstanding maintenance jobs.
Case study two: A service user living in extr8 ¢arÈ shèltèred accommodation faced signifieant ¢h8118ngos duo lo a
18nguage barrl8r. as he communicated solely in Cantonese. To provide him wlh effectlve support. we collaboraled
dosely with 8nolher care agèncy.
FollovAng his discharg# from hospital. his medl¢allons were redLKed, and he required daily insulin inlections.
However, he soon began misman8ging hls medlcines. refusing to take his tsblels and oc¢asion811y s818Cting the
wrong compartments from his ￿l$ler packs. Although he had full mental capaaty, the language barrier p￿Vente<l
him from explaining his raason$ for rèfv8al to thè other agèncy.
Our support Worker engaged wlh hlm In Canlon$se and dlscovered that he believed the t8bl8ts wère damaglng hls
kidneys. With this valuable insight, we establisheil re9ular communlcatSon with the other agency and updatèd both
the social worker and GP. This proactive step led to a multi-agency meeting involving the social worker. dlstrict
nur8e, and repre8ent8tive¥ both 8genei8s. As 8 rasult, his medication s¢Jpport was adjusted lo'promptlng". and
the distriet nursé pMv1d￿ a lockabla boK for hls blister packs. Care workers would o￿n the box on ardv81 and
prompt hlm to tskg hls medlclnes safely and CA)rrectly.
