and wl)ere the charity aligns itself within a national and local agenda that has older people at the
foretronl. Given the charity is enhancing currgnl sorvices and seeking to develop new services il is
imperative that this review takes place annually. unless there are extenuating drcumstsnces which
prevenl it occurring.

Charilles Act 2006. Age UK Plymouth interocts wlth up to 2,300 older people each month through its
services and 15 therefore playlng an essential role wilhln the City and in local communllles.
The Charity helps older peopl8 to malntaln their Sndependence, onhan¢o their quallty of lffe. make
Infom)ed cholces and rèalS8è Ihalr rfghts. Evéry cllant is spadal and 8 cllgnl cantrgd approach Is taken
throughout Ihe charlly.
The Charlly has a mixlure of clients, many are able to pay feas for the services provided, whilst others
receive support from Plymouth City Coundl or thè NHS. Age UK Plymouth also provid8$ many
SO￿1¢e$ which a￿ free al the point of delivery - Ihg Information and Advice service being an example
ol one whlch assists older people lo remaln in eonlrol and able to mBke informed cholce$ on Issues
that may affect their Independence, finBncial h8811h and wellbeing. In addition, the Charity provides
frae Demantla Awargness training and has a Wellbelng Hub that encourages those In the local
communlly lo drop Sn for IndSvldu81 advlce support. The Charfty introduced free 88rvlces in
response lo Ihe Coronavlrus Covkl 19 pandemic and many of thes8 such as Befrlending and Phone
Frlgnds have continued and are now part of (he core offèr.

would've taken her own life had she remained in the care home as She felt helpless. The di8nt's niece
called the STCC following her dI￿harge to thank all staff involvéd. we had taken a burden from her

and flgxlble $gryico lo people aged 50 and ov8r, Ilving in and around the Clty of Plymouth. This
8ervlc8 is tBilored to p￿P1a,5 Indlvldual needs and includes shopping, cleaning, assisted trips,
gardening, COmpan￿nship and much more. The Community Support Assistants play a vllal role
assisting those who need additional support enabling many to continue lo Ilve happily in their own
hcffles whilst improving their wnfidence and gengral wgllbeing.
The Help al Home and Communlty Support service In numerlcal lernis supported 1.067 older people
to continue living in their own home, maximislng thgir independence, whilst hglping with the activities
of daily living. The te8m provided an astonishing, 21,758 vlslls a total of 27,823 Support hours to older

day care provision. The William and Patricia Venlon Day Centre operates 4 days a week and provides
day care and day opportunilies for old8r people. who may be physically frdil. have become gocially
isolated or simply want to benefit from peer group support. The atmosphere within the Day Centre Is
always positive and a number of wonderful volunle8r8 of all ages and backgrounds provide additional
SUPPOrt. Pr()viding this 16v&l of day car8 &nablgs Garèrs the lima and the opportunity lo have some
free time or respite and allows them lo focus their end8avours els8wher8.
The Oay Contre offars a range of 8venis. actlvlties, singalong8, compei5tions, quizzes and mor8 all
geared towards stimLJlaling the mind and body and improving the wellboing and m8ntal health of
those attending. There is o three-course quality and freshly cooked maal Provided every day and
clients eat this in a n8￿Y refurbished dlnlng room. The abilily to use our podiatry and hairdresslng
¥ervlce has begn wo11 receiv8d by clients.

Sinc8 the Short-Term Carè Cèntra ISTCCI opènèd it has acted as a ratalysl enabling a grèat deal of
effective servlce dev8lopmenl lo lake place throughout the year and has madg an enormous
dmference lo the flow of patients from hospitsl to homè throughout the City and It has be6n lauded for
its ¢ffecliveness and impact.

bedroorns Pfovlding a s8fe place lor patients lo stay after they have been In hospllal. Thls could b8 for
anything up lo four weeks arKJ the aitn is that when people leave the STCC they are feeling rnuch
better and ready lo go home and lo live a5 independenlly as possible. Age UK Plymouth provides all
the catering requirements Wlthin the STCC and this includes, breakfast, dinner and an evening
provision. The Charlty also works seamle551y wlth LiveweS1 Southwest provkjlng a housekeeplng and
maintenance service that ensures that the dally n8eds of patients are mat In the most conslruclive
and lolnad up mannor.
In 2023124, 143 patl8nts stayed in the STCC. Th8 av8r8g8 stay was 21 days and 131 of th8se
patients left with no or minimal care. This is an extremely positive outcome. 12 pats'ents which is less
than 10% needed readmission lo hospital either direct from th8 STCC or al a18ter stage from home.

The advent of the STCC has acted a5 a catalyst for change and for providing the opportunity lo
d8liv8r new services. Age UK Plymouth provid8S SUPPDrt lo pali8nls being discharg8d from the STCC,
from Mount Gould and Derriford Hospital. The support the charily provid8rs is person centred and is
ta51or8d lo me81 the needs of the individu81 patient. Age UK Plymouth supports patients who live wthin
the City of Plymouth belng dlscharg8d on 8ilher pathway O or pathway 1. Thg $8rvlce Indud8S
shopping. cleaning, simple food preparation picking up pre5criplions. confidence building, benefits
check, the opening ol mall, ¢ompanionyhlp and much 11 hay becorne yuch an Important
component of tho discharge provi810fl across the Clty and has qulckly becom8 one of the most eruclal
5ervlce3 provided by Age UK Plymouth.
In the year ended March 31512024 Age UK Plymoulh provided packages of care to 567 patients
