**Objects:** TO RELIEVE THE NEEDS OF INDIVIDUALS, PRIMARILY BUT NOT EXCLUSIVELY OF ASIAN AND AFRICAN ETHNICITY, AND THEIR FAMILIES OR CARERS, WHO SUFFER FROM CHRONIC LONG TERM HEREDITARY HEALTH CONDITIONS BY:RAISING AWARENESS ABOUT CONDITIONS SUCH AS CONGESTIVE HEART AND RENAL FAILUREPROVIDING INFORMATION AND ADVICE OR SIGNPOSTING TO OTHER PROVIDERSPROVIDING SMALL GRANTS FOR THOSE EXPERIENCING FINANCIAL HARDSHIP.

**Activities:** Mina Foundation promotes and raises awareness of long term chronic conditions that impacts communities and carers, while providing direct support, advice, information, advocacy, healthy behaviour education and guidance on carers rights. We organise community events and wider activities with local partners to improve the physical and metal well-being for carers and their families facing inequality.
