During 1/6/23 to 31/5/24 we conceived, developed, ran,
supported and/or wrote up eighteen projects. All of these
projects further our charitable objects because
1)
they all require project management (which is
used to train junior doctors and haematologists
in how to be researchers and principal
investigators) and
2)
they ultimately result in the publication of
research findings that benefit patients.
Projects take several years to complete from the time of
inception. Activities completed 1/6/23 to 31/4/23 are
highlighted inred text.

Topic:to establish attitudes towards and
compliance with two (historic) sample rule in
blood transfusion.

Size of study:209 responses across 22
hospitals.
Main outcomes:96% aware of 2 sample rule
in their hospital. 57% understand what the rule
is. 55% understand the rationale for the rule.
60% admitted to going against the rule on
more than one occasion. Non-compliant
behaviour not associated with 2SR training or
seniority.

HaemSTAR RAPIDO (A ‘Flash-Mob’ UK national
audit of the use of Reversal Agents in Patients
antIcoagulated with Direct Oral anticoagulants)
Type of study:Retrospective National Audit.
Topic:Compliance with BSH guidelines for the
use of reversal agents in patients with major
haemorrhage who are taking direct oral
anticoagulants (DOACs).
HaemSTAR role:Conceived and developed
by Dr Richard Buka (Stoke) and HaemSTAR.
Run by HaemSTAR. Data management
provided by BiCOPS. Funded by AstraZeneca
and the Dudley Leukaemia Unit Appear Fund.
Size of study:2265 patients.
Main outcomes:875 patients had GI
haemorrhage, 1012 had ICH. Median age 81
years. Median time from bleed onset to
reversal: 6.5 hours. Median time from last dose
of anticoagulation to reversal: 16.0 hours. 1001
of 1253 non-ICH bleeds fulfilled criteria for
major hemorrhage. Propensity score-matched
analysis of 494 patients with GI haemorrhage:
outcomes similar for andexanet alfa and four
factor prothrombin complex concentrate.
Incidence of stroke was significantly higher
with andexanet alfa.

MOM-ITP: Audit of management of neonatal ITP

pregnant women with ITP and neonatal ITP.

TXA for HMB in acute VTE
Tranexamic acid is an effective antifibrinolytic drug that
can reduce heavy menstrual bleeding. Despite excellent
safety data showing negligible increased risk of
thrombosis across millions of patient years, its use in
women with heavy menstrual bleeding who have a
recent venous thromboembolic event remains
controversial. The goal of this project is to work towards
a pilot Randomised Control Trial.
Type of study:UK Clinician Survey
Topic:Clinical attitudes and practice towards
use of tranexamic acid in female patients with
menorrhagia who have previously experienced
venous thromboembolism.
HaemSTAR role:Conceived and developed
by Dr Eman Hassan (Birmingham and
HaemSTAR). Distributed by HaemSTAR. Data
analysis by Dr Hassan. Funding: nil.
Size of study:102 respondents.
Main outcomes:Apixaban preferred
anticoagulant for patients with HMB. Wide
variation in practice re timing of TxA use; 35%
prescribed it any time following initiation of
anticoagulation, 12% delayed its use for 3
months; 8% would never give it. 47% of
respondents advise to discontinue COCP
following VTE (whilst treated with
anticoagulants). 63% of respondents willing to
participate in a study of TxA ≤ 4 weeks
following VTE.

MATRON (Maternal Audit on ThRombolysis
Outcome and DecisioN)
Type of study:UK Clinician Survey &
Retrospective Audit
Topic:The use of thrombolysis for pregnant
women with venous thromboembolism.
Size of study:Survey with 70 responses.
Target audit responses of 200 patients
(completion Q2 2025).
HaemSTAR Role:Conceived and developed
by Dr Giulia Simini (Royal London &
HaemSTAR). Run by HaemSTAR. Data
management by HaemSTAR.
Main outcomes:Survey: CDT available to
74%, Surgical thrombectomy to 62% and
ECMO to 52% of respondents. Only 50% of
sites had specific guidelines on CDT and 17%
had no guidelines on managing pregnancy
associated PE. 38% had received specific
training on pregnancy-associated PE. 22% had
had training in systemic thrombolysis. 46% did
not have access to PERT with geographic
inconsistency. Audit in planning stages.

VAMP: Venesection vs Active Monitoring in
Secondary Polycythaemia
Type of study:International clinician survey,
retrospective audit and then prospective
clinical trial.
Topic:Venesection in patients with idiopathic
and secondary erythrocytosis
Size of study:Survey with 123 responses (90
senior clinicians). Audit with 206 patients.
Prospective study in development.
HaemSTAR Role:Conceived and developed
by Dr Richard Buka, Dr Bernard Maybury and
Dr Pip Nicolson (Birmingham & HaemSTAR),
Run by HaemSTAR, Data management by
HaemSTAR.
Main outcomes:

Survey: minority venesect routinely,
2/3 will venesect in specific
circumstances; 1/3 never venesect.
More likely to venesect if previous
thrombosis or if headache or visual
disburbance. Majority use Hct
threshold of 0.55 but some use 0.6
and most have Hct target of <0.55
but some use <0.52. 85% willing to
consider entering patients into a trial.

Audit: 2360 patients screened, 206
eligible. 27% no EPO done. Minority
have RCM scan done. Most given
appropriate lifestyle advice. 60%
never venesected. 29% of those
venesected have no target Hct
documented. Mean Hct reduces prior
to diagnosis and further reduces
regardless of venesection. 5 year
thrombosis rate 12% regardless of
venesection vs no venesection or
whether polycythaemia resolves.
Cancer and prior unprovoked VTE
are the only factors to predict
subsequent thrombosis.

Apixaban thrombo-prophylaxis in multiple myeloma
It is recommended that multiple myeloma patients
treated with IMIDs such as thalidomide and lenalidomide
be offered prophylactic anticoagulation due to the high
risk of VTE. We understand that practice in the UK has
shifted considerably and despite a lack of evidence,
clinicians now favour the use of oral factor anti-Xa
inhibitors, often apixaban, for this purpose. The aim of
this project is to perform a retrospective, observational
study to document thrombosis and bleeding outcomes in
patients who receive this treatment compared to those
who receive low molecular weight heparin.

Anticoagulation at the end of life
A survey is to gather data from secondary care clinicians
in the UK on their management of anticoagulation in
patients at the end of life.
Perioperative management of Haemophilia B
‘FIX-Surg’
The primary objectives of the project are to audit the
haemostatic management of patients with haemophilia B
who are undergoing surgery against international
guidelines (Srivastava et al., 2020).
An Epidemiological Survey of Morbidity and
Mortalityin Patients Over the Age of 35 With
Thalassaemia (Transfusion Dependent and Non-
Transfusion Dependent) in England: SUMMIT
Thalassaemia has historically been a severely life
limiting disease with significant comorbid complications
from an early age. As treatments, monitoring and
awareness of this condition improve, so does quality of
life and life expectancy. This study aims to assess
whether patients over the age of 35 with thalassaemia
are monitored in keeping with national guidelines, and to
build a picture of the co-morbidities affecting patients
with both transfusion and non-transfusion dependent
thalassaemia over the age of 35 in the United Kingdom.
FUNDING AWARDS
From the ISTH Legacy Fund we were able to fund the
conference attendances (see website for awardees).
Application for award through competitive application
with award by grant panel.
Part of the ISTH Legacy Fund was to promote
collaboration with LMIC (Nigeria) and help them develop
their own trainee research network. In collaboration with
NSHBT (Nigerian Society for Haematology and Blood
Transfusion) they have founded HaemRYNE. Funds go
towards infrastructure development and travel costs to
promote educational visits within Nigeria and to UK.
CONFERENCE
In association with Hartley Taylor and the Post
ISTH/ASH UK Update meeting we hosted a HaemSTAR
Update day on 5/2/24 at Austin Court in Birmingham.
This comprised updates and results on current
HaemSTAR projects, plenary talks from experts
including reflections on clinical/academic careers,
management of HMB, CRISPR-Cas9, rural haematology
as well as discussions about the role of the NIHR HTA.
Very positive feedback. increased industry engagement
from previous year. Increased attendance. Free
registration HaemSTAR helped with travel
reimbursement to enable attendance as well the time of
the invited speakers.
DISEASE THEMED WEBINARS
Teamed up with ‘The Limbic’ to provide monthly online
webinars from KOLs providing thought provoking and
educational talks on disparate areas of non-malignant
haematology. KOLs are reimbursed for their time. The
Limbic reimburse HaemSTAR if attendees fill in
feedback surveys.
PODCAST
Received funding from Sobi to purchase equipment and
fund external agency to provide sound engineering
expertise. Monthly journal club podcast on latest papers
in non-malignant haematology. Includes author
interviews and discussion with experts.
BSH CONFERENCE SESSION
HaemSTAR now has a session at BSH (in association
with UK-TReN) in the form of Dragon’s Den. HaemSTAR
funded the prizes in this session.

committed spending for 2024-25 comprises website
hosting fees of approximately £350, podcast hosting
fees of £200, accountancy fees of ~£150 and
administration costs (paid for the time of our
administrator) of ~£3000. This is all covered by the
administrative grant of £5000 from Takeda and does not
represent a financial risk. We also aim to apply for
additional sponsorship of the podcast for when the Sobi
sponsorship ends in 2025.
In 2023-24 we expanded our ability to fund registrars to
attend conferences. £19,200 of this comes from the
ISTH Legacy fund and we are able to use this until
2026. It is unlikely that costs will exceed this figure.
The educational webinars are a source of an
approximately £1200 annual cost. This is voluntary
however and can be stopped if there are insufficient
funds.
There is an approximate £30,000 cost still associated
with RAPIDO that is waiting to be paid to the University
of Birmingham but this is covered by the Astra Zeneca
grant from last year.
The HaemSTAR Update Day is now a recurring event
but this is organised through Hartley Taylor and they
secure industry support for the meeting. The only
HaemSTAR costs are voluntary and will not go ahead if
there are insufficient funds.
All the projects currently active are fully funded.
However we would like to run unfunded projects by
using money underspent from other projects and also
pump prime our pipeline by employing a project/data
manager through the University of Birmingham at a cost
of approximately £70,000 per year. This is not yet
feasible and will not go ahead unless there are sufficient
funds. The funding for this will be reviewed on an annual
basis and no commitments beyond 1 year will be made
to ensure there is no financial risk to the charity.
