**Objects:** THE OBJECTS OF THE CIO ARE THE PRESERVATION, PROTECTION AND PROMOTION OF GOOD HEALTH AMONGST THE PATIENTS REGISTERED WITH THE ROSELAND SURGERIES, CORNWALL (“THE SURGERY”) BY: I) PROVIDING INFORMATION, ADVICE AND SUPPORT AND ENCOURAGING PATIENTS TO TAKE RESPONSIBILITY FOR THEIR AND THEIR FAMILY’S HEALTH II) ARRANGING ASSISTANCE WITH TRAVEL TO AND FROM THE SURGERY FOR THOSE WITHOUT THEIR OWN TRANSPORT OR WHO ARE UNABLE TO ACCESS PUBLIC TRANSPORT III) PROVIDING EQUIPMENT OVER AND ABOVE THAT PROVIDED FROM STATUTORY MEANSIV) ORGANISING SPECIAL HEALTH EVENTS AND SOCIAL EVENTS TO REDUCE ISOLATION V) PROVIDING THE SURGERY WITH AN INSIGHT FROM THE PATIENT PERSPECTIVE ON THE QUALITY OF ITS SERVICES INCLUDING COMMUNICATION WITH AND SURVEYS OF THE PATIENTS WHERE APPROPRIATE.
