Our Aims and objectives
Purposes and Aims
Our charity’s purposes are as follows:
The advancement of health, including the prevention or relief of sickness, amongst underprivileged communities in any part of the world by:

(a) Providing free surgical and medical care to patients in need of such care;

(b) Training local medical professionals; and

(c) Assisting with the development of a sustainable medical infrastructure.
The relief of poverty of children living in underprivileged communities in any part of the world, including in particular children living in orphanages, in such ways as the charity trustees shall think fit.
The advancement of education of such children in such ways as the charity trustees shall think fit.
The focus of our work
Our main objectives continued to be to deliver safe surgical care, free at the point of use. We aim to do this by gathering a team of clinicians and surgical equipment and partnering with a hospital in a lower or middle income country (LMIC) to organise a week-long trip to that hospital. During that trip we are allocated local staff and operating theatres with which we deliver a high volume of surgical procedures to the local population who have been notified of our arrival in advance and travel far and wide to reach us.
Who benefits from our services?

Two thirds of the world’s population do not have access to safe, timely and affordable surgery. There are 1.1 million Specialist Surgeons worldwide, yet less than 20% of those are in LMICs. Africa and South East Asia are particularly affected. Given that population in LMICs represents almost 50% of the world’s population, LMICs are massively underserved when it comes to the provision of surgery.
The Impact of Covid-19
The global covid-19 pandemic has brought unprecedented disruption to economies and societies globally. This disruption has curtailed a range of activities, from local fund-raising to delivering surgical healthcare abroad.
Two of our trustees are surgeons working in the UK whose primary focus has been to maintain a minimum standard of care in their respective hospitals. During the initial waves of the pandemic all but trauma and emergency surgeries were cancelled. Staffing levels were drastically reduced from sickness, isolation, and re-deployment to covid wards. In subsequent waves of the pandemic some limited elective surgery was permitted but volumes were vastly reduced, and staff sickness and isolation precluded any extra-curricular travel to run a surgical mission.
Added to this was the material risk of transporting and spreading variants of covid to the donor recipient countries. While covid protocols were created to minimise this risk, the overall level of risk involved, coupled with the severely low rates of immunisation against covid-19 in the donor recipient countries, meant that we were forced to cancel a surgical mission in 2020 and in 2021.
This review does however help us ensure our aim, objectives and activities remained focused on our stated purposes. We plan to deploy a small team of clinicians for a mission in 2022.

Financial Review
Against the backdrop of the covid-19 pandemic of 2020-2021 it has not been possible to hold any in-person fundraisers or run any surgical care missions.
We have submitted an application to a foundation (Global Giving) but were unsuccessful as we had not performed a mission since our establishment. They did however encourage us to re-apply.
We have been able to get the basic infrastructure of our charity established as follows:

Principal Funding Sources
We envisage that our principal funding source will continue to be grants from our affiliated charity, Operational International, incorporated in the USA.
We also aim to organise in-person fundraisers in 2022 and raise funds from ticket sales and raffle draws.
Finally, we aim to submit grant applications after we have run a mission and may demonstrate a track record of activity.

Structure, Governance and Management
Governing Document
The organisation is a charitable incorporated organisation (CIO), registered as a charity on 7 May 2020. The company has a constitution which established the objects and powers of the CIO and is governed under its articles of association.
Recruitment and Appointment of Trustees
The directors of the CIO are also charity trustees for the purposes of charity law. Under the requirements of the constitution a trustee will serve until i. the trustee dies, ii. if the CIO ceases to exist; or iii. The trustee sends notice of resignation to the charity trustees provided that there shall be at least one trustee after the notice of resignation takes effect.
All Trustees give their time voluntarily and receive no benefits from the charity. Any expenses reclaimed from the charity are set out in the accounts.
Due to the nature of the charity’s work, the trustees seek to ensure that the needs of this group are appropriately reflected through the diversity of the trustee body, as well as the more traditional business and medical skills.

Risk Management
The Trustees have conducted a review of the major risks to which the charity is exposed. These risks and associated mitigants are outlined in the ‘Working Overseas’ and ‘Safeguarding’ policy documents that are both available on the trust website (oiuk.uk). These policies are reviewed every two years and updated where necessary.
Where appropriate, systems and procedures have been established to mitigate identified risks to the CIO. In other instances, the charity will rely on the risk management procedures of the volunteer’s employer. For example, the Director of the charity has been designated as the safeguarding lead to ensure rapid response to any safeguarding concerns. All clinical volunteers are also required to have undertaken hospital-delivered safeguarding training within two years of joining a surgical mission. This ensures a uniform understanding of overall safeguarding responsibilities. We believe that this requirement, in conjunction with familiarly with our specific safeguarding policy, offers a sufficient control against safeguarding risks.
Organisational Structure
The CIO has three trustees who meet quarterly and are responsible for strategic direction and day-to-day management.
Related Parties
In so far as it is complimentary to the charity’s objects, the charity is guided by both the national and the international areas of surgical best practice.
The CIO also has an affiliate relationship with Operation International Ltd (OI Ltd), who have provided financial and organizational support to establish OIUK. The chair of Operation International UK is a member of the Board of Directors of Operation International Ltd.
Responsibilities of the Trustees
The trustees are required to prepare statements of receipts and payments for each financial year. As there has been no activity in the CIO in the prior financial year, the financial report is net zero.
The Trustees are responsible for maintaining proper accounting records which disclose with reasonable accuracy at any time the financial position of the CIO and to enable them to ensure that the financial statements comply with the Companies Act 1985. The Trustees are also responsible for safeguarding the assets of the CIO and hence for taking reasonable steps for the prevention and detection of fraud and other irregularities.
