Evolve Housing
Association
Evolve Housing Association is a Nottingham City based, non-profit organisation Icharity
Incorporated Organisation) which has been set up since July 2018 to provide affordable
homes for people with housing needs. We cover both Nottingham City,
Nottinghamshire County and the East Midlands.
Evolve Housing Association embraces the most vulnerable people in our communities,
including women and children who are victims of or have been aff ected by domestic
violence and abuse, those who are currently vulnerably housed, homeless, rough
sleeping or sofa surfing and those with substance misuse issues.
We have established excellent working partnerships already with The Resettlement
Programme and Nottingham City Council. We continue to work with housing providers
to safeguard clients and receive referrals.
Evolve Housing Association continued to work on the"Alfie House Projecf to provide
an alternative pathway out-of-hospital for people who do not have a home.

CEO'S Statement
Welcome to our Annual Report.
This report covers the period between April 2023 and April 2024. It
highlights our achievements, our overall effectiveness and reflects
on how we have grown as a housing association over the 5 years
since being established.
We actively welcome constructive feedback from residents and
stakeholders, as we believe it is essential for our development and
growth as a company. Continuously seeking ways to improve allows
us to maintain the highest standards of safety and service for the
vulnerable individuals we support.
Our primary focus is on aiding the most vulnerable members of
society, particularly within the homeless sector. With the support of
Nottingham City Council, we strive to reduce homelessness across
Nottingham to the best of our ability. We are proud to play a
slgnlflcant role In tachllng homelessness in the city, a feat made
possible through the generous funding that enabled the opening of
Alfie House. We are deeply grateful to the donors whose support
made this achievement possible.

They have skills which enable Evolve and its staff to reach higher
expectations and aims to be one of the best housing associations in

OUR OBJECTIVES
-To contribute to the supply of high quality, accessible, secure, affordable
and sustainable homes where financially viable.
-To provide a comprehensive and responsive customer service to tenants.
-To provide support to our clients around important practical matters
such as the use and maintenance of property amenities, the management
ot rent, tenancy agreements and any other proceedings.
-To fully understand our internal service provisions, how they work
together and what they can deliver for partners.
-Transparency with our partnership's working is very key. We intend to
develop and maintain a strong understanding of what is important to them
to achieve better outcomes of service delivery.
-To have support staff that is fully engaged and motivated to achieve high
levels of care to individuals, with empathy and taking into consideration
the individual's specific needs.
-To develop staff through a comprehensive tailor-made training package
focussing on developing leaders and future managers. Helping them to
become more resilient and performance focused. Also maximising staff
engagement with incentivised schemes. We believe that this, in time, will
maintain staff turnover levels to a minimum and increase staff
sustainability.

2024
What did we do?
ALfie House was created to offer accommodation
to individuals who were medically fit to leave
hospitaL but still needed additional support. This
important initiative aimed to provide a secure and
nurturing environment, acting as a hub where
residents could receive continuous aftercare and
recovery assistance from a dedicated community
nursing team. Each resident is assigned a support
worker, who takes responsibility for developing
and regularly reviewing a taiLored support plan on
a weekly basis.
The service also ensured access to drug, alcohol,
and mental health services, providing residents
with opportunities to engage in essential
treatment and support.

After a period of up to three months, individuals
are transitioned to more stable housing or
speciaLised accommodation. Between 2023 and
2024, we successfully supported 28 individuals in
moving into independent tenancies or supported
housing.
The success of Alfie House can be attributed to
many factors, but the commitment and
professionalism of the staff in delivering their
services is the key element that stands out.
Residents frequently praise the caring,
compassionate, and empathetic approach of the
staff, especially when handling challenging
behaviours.
There is a clear and urgent need for this type of
service, as it fills the gap for individuals who,
although medically fit to leave hospital, still
require time for housing arrangements, care
assessments, and specialist support. The
availability of such services plays a vital roLe in
reducing delays in hospital discharges due to
housing-reLated issues.

Our Funders
Nottingham City Council
Evolve Housin9 Association has partnered with Nottingham City Council on the
Vulnerable Person's Resettlement Scheme (VPRS) to offer accommodation and
essential tenancy support to refugee and asylum-seeking families. These tenancies
are managed by Evolve Housing Association and funded by Nottingham City Council.
We handle all tenancy documentation, compliance certificates, maintenance, and
conduct 12-week inventory reviews to help tenants maintain high property
standards. We extend our gratitude to Nottingham City Council for their continued
cooperation and support in assisting us to house vulnerable individuals in our
communities.
Our goal is to enable our clients to make positive changes to their lives and for them
to fulfil their individual aspirations, which the Alfie House Project was set out to do
for those who are leaving hospital. Our aim was to create support and care plans for
the start and exit of tenancies for beneficiaries in order to achieve an effective move
to more sustainable mediuml long term assisted accommodation, which would deter
the need for re-admissions to hospitals. Our aims this year were executed, which of
course would not have been possible without the funding and support of Nottingham
City Council.

Our Funders
Awards for All - Helping
Hands
The project targeted homeless people in Nottingham City - individuals at
risk of homelessness, living in B&B's, on sofas, living on the street and
individuals who are being discharged from hospital with no
accommodation to return to. We provided one to one support to
beneficiaries with the aim of supporting individuals to help keep them
secure accommodation and help peopLe get the support they need for
wider issues they face that impact on their housing situation.
The 29th May1961 Charitable Trust
Funding was provided to grow, stabilise and support more vulnerable
people through delivering a range of positive interventions once housed to
support Long term tenancy and movement into training, employment
andlor voLunteering.
Groundwork
We received funding to support the most vulnerabLe people in our
communities especially refugee and asylum seekers (50% of
beneficiaries). We provided one to one support to beneficiaries with
the aim of supporting individuals to heLp keep them secure at home,
find a home/place to stay and heLp people get the support they need
for wider issues they face that impact on their housing situation.
Evolve played a key role in preventing and ending homelessness for
vulnerable peopLe.

||I report to the trustees on my examination of the accounts of the above<br>charity (“the Trust”) for the year ended**31/03/2024.**<br>As the charity's trustees, you are responsible for the preparation of the<br>accounts in accordance with the requirements of the Charities Act 2011<br>(“the Act”).<br>I report in respect of my examination of  the Trust’s accounts carried out<br>under section 145 of the 2011 Act and in carrying out my examination, I<br>have followed all the applicable Directions given by the Charity Commission<br>under section 145(5)(b) of the Act.|||
