The trustees present their report with the financial statements of the charity for the year ended 31 December 2020. The trustees have adopted the provisions of Accounting and Reporting by Charities: Statement of Recommended Practice applicable to charities preparing their accounts in accordance with the Financial Reporting Standard applicable in the UK and Republic of Ireland (FRS 102) (effective 1 January 2019).
OBJECTIVES AND ACTIVITIES
Objectives and aims
The purposes of the charity as set out in its governing document The objects of the CIO are, for the public benefit, particularly, but not exclusively, in Africa and other low/middle income countries (LMICs):
(1) the relief of sickness and the preservation of health, particularly but not exclusively by promoting the sharing of healthcare knowledge and skills and assisting in the provision of medical treatment;
(2) the advancement of education in healthcare, in particular by the delivery of quality improvement and leadership training in a healthcare context and other health- related education and training to those involved in health and care delivery at all levels and to the wider public, and by raising awareness of global health issues;
(3) to develop the capacity & skills of members of socially and economically disadvantaged communities in LMICs in which the CIO is working, in such a way that they are better able to identify and help meet their needs and to participate more in society.

OBJECTIVES AND ACTIVITIES
Significant activities
The main activities in relation to those purposes for the public benefit, in particular, the activities, projects or services identified in the accounts. The charity has 8 workstreams (with a 9th in development) directed at achieving our objectives. Not all are active every year. Each is described below along with the activities conducted during the period of this report. All of the work was conducted in Kenya. Further detail can be seen at www.futurehealthafrica.org
All of our work aligns with the Sustainable Development Goals.
WORKSTREAM 1: TRAUMA TEAM
Description: This is a clinical & training project in which a joint UK and Kenyan team of skilled professionals of different cadres undertake orthopaedic trauma activities in Kenyan health facilities.
Trauma Team activities are categorised into Bronze, Silver and Gold. All services are delivered at the highest standard, with improving quality as a central aim.
Bronze: immediate & direct patient care through
- high quality consultations, advice and where appropriate, both operative and non-operative treatment to alleviate pain, suffering and disability
Silver: learning opportunities/ new skills

side-by-side working with exchange of knowledge & skills in relevant disciplines (surgery, anaesthesia, nursing, operating department practice, radiography, physiotherapy)

developing a safer surgical environment

exchanging knowledge on safe general and regional anaesthesia and pain management
Gold: sustaining improvement/ development

identifying areas to develop a stepwise pathway

supporting local need and wishes to change

Activities undertaken:
The COVID-19 pandemic unfortunately resulted in the postponement of two one-week planned projects for November 2020 to Nyahururu and Marsabit. However, regular Zoom meetings occurred throughout the year with orthopaedic colleagues in Kenya, planning for 2021 activity and exploring potential teaching links through Dr Vincent Mutiso, Head of Orthopaedics at the University of Nairobi. Preliminary work also took place to establish a physiotherapy teaching programme around rehabilitation. There is interest in establishing a link with the single orthopaedic surgeon in Sierra Leone who received some training from Trauma Team in November 2019. Ad hoc case advice continued to be given. Dr Samuel Ndanya. Orthopaedic surgeon (Nanyuki) and Dr Mary Nduta continue to advise from Kenya.

WORKSTREAM 2: GRASPIT
Description: GRASPIT (Global Recognition & Assessment of the Sick Patient & Initial Treatment) is a one-day course which equips clinicians with a structured systematic approach to the management of the deteriorating acutely ill patient. GRASPIT has been shown to increase a clinician's confidence in the management of these challenging patients. The four key principles of GRASPIT are:

Team working through effective communication
When introduced to a new health facility, an introductory 3-day course is run by Kenyan trainers which includes a 'training of trainers' course. Local trainers are then able to deliver the course to more healthworkers. The UK lead (Dr Matt Halkes) holds oversight of standards of delivery, strategy and grant applications within his remit.
Activities undertaken:
A lack of funding has constrained implementation of the well-developed strategy. However, GRASPIT courses continue to be run at Kenyatta National Hospital because they are highly valued.

WORKSTREAM 3: QUALITY IMPROVEMENT & LEADERSHIP
Description: This workstream recognises that 'sick systems' prevent better patient outcomes. Our q4a (quality for all) faculty has designed the SPRINGBOARD course to empower frontline healthcare workers to improve quality of care by learning and applying 'The Science of Improvement' and the 'Art of Leadership'. The course normally runs for 10 days and imparts knowledge of improvement methodology, team working, leadership, patient-centred care and an understanding of how to create a positive culture for change. The skills are learned through doing real live quality improvement (QI) projects.
Activities undertaken:
This workstream won a grant of £48,781 from the DFID (now FCDO) Small Charities Challenge Fund (SCCF) grant stream. The objective was to test the SPRINGBOARD programme (QI & Leadership) in 2 new settings

in another county (Marsabit) where the Governor had agreed to fund a course and we were to evaluate and support the QI projects
-
in a community setting to assess the impact of QI approaches at this level of the health system
Start was delayed because DFID halted all grants for a while and then COVID intervened . When the grants were reactivated, we were asked to pivot the work of the grant to accommodate COVID-19 . The pandemic was clearly going to prevented any visits to Kenya by the UK team in person so we partnered with a Kenyan NGO, LVCT Health, who we had identified were doing some excellent QI work with communities. A partnership agreement (MoU) was signed and the grant work commenced in December 2020.
The partnership gained agreement with Mannion Daniels (the grant manager for the SCCF) to direct the grant activities to community work only. The work is to engage with Community Health Volunteers (CHVs), Community Health Assistants (CHAs) and Sub-County Government leads to test QI approaches focussing on pregnancy outcomes in disadvantaged communities. This focus was chosen as it is a local/country/WHO health priority and because it was clear that these were likely to deteriorate during the pandemic. It was decided to deliver the training in

Migori County where LVCT had already been working on a similar project
-
Laikipia county (with the hope that the UK team might be able to visit Kenya in 2021)
LVCT had identified (through their previous work) that the QI work would be considerably strengthened by enhanced leadership skills but did not have a curriculum to deliver this. We were able to adapt our 5 day course into 3 days to enable them to test this. We provided a timetabled curriculum, workbook and teaching slides to accompany the course.
In December 2020, Phase 1 (of 2) QI training was delivered in Migori to CHVs and CHAs in 3 new community health units in Migori county, followed by the 3-day leadership course (which included sub-county staff). Both were very well received and work began on the projects to improve implementation and reporting related to the Kenyan official guidance on maternity care.
Regular reporting meetings were set up and will continue throughout the grant. Relevant proportions of the grant were devolved to LVCT Health on receipt of detailed reporting.
WORKSTREAM 4: COMMUNITY
Description: This project aims to improve health outcomes in rural areas of Kenya. It includes working with conservation NGOs (such as Northern Rangelands Trust and Lewa Wildlife Conservancy) and the Ministry of Health and providing technical assistance in rural health services and programmes. This involves looking at where to develop a new clinic, what services should be offered in specific areas given the geographical location, tribal culture and disease epidemiology.
Activities undertaken:
No visits were possible but ad hoc advice continued during 2020.

WORKSTREAM 5: TEAM TALK
Description: Team Talk seeks to directly improve the lives of girls, enabling them to be aspirational and play a fuller role in the family and the wider community. It encourages boys to develop a greater level of respect for the girls. It started at the request of community elders in the Westgate and Ntalabany regions in 2010 who said they would like to get their girls into school and that they wanted to prevent young marriages.
Team Talk uses TAG rugby in primary schools alongside classroom sessions to address gender inequality in rural Kenyan schools. Boys and girls to play together, thereby developing teamwork and a mutual understanding of each other. In 2011 the first project ran in these regions and has since expanded into schools in Laikipia, Mweiga and the Mara. TeamTalk works alongside the Mukogodo Girls Empowerment Program run by Josephine Ndirias.
Activities undertaken:
One UK-led TeamTalk project took place in February 2020 before the declaration of the COVID pandemic. The programme (sport plus education/mentoring sessions) ran in Sangaa school in Borana in Laikipia county, culminating in a successful tag rugby tournament which is a way of celebrating the achievements of youth. A follow-up visit to Leperua community then took place and another tournament was held.
Planning meeting were held to define the ongoing youth leadership programme, community support, Kenyan ethics for follow up and research.
Planned visits from the UK in the latter stages of 2020 were not possible due to the ongoing pandemic but this actually allowed the Kenyan team the chance to really take a lead. When Covid allowed in November , the Kenyan Team led by Ronnie Okoth started to visit the schools once/twice each month. This was hugely welcomed by the schools and we gathered some excellent feedback from the students.
Before Christmas 2020 we held a tournament at one of the school (Lokusero) to which we invited and transported children from all of the other schools with which we work.
Ronnie conducted a week of visits to the schools with plans for ongoing monthly visits.
A research project to evaluate the TeamTalk programme was defined and the ethics approval process in Kenya was initiated.
WORKSTREAM 6: MATERNITY
Description: The overarching objective of the 3 midwife members of the Maternity team is to reduce maternal mortality & morbidity through education of healthworkers, traditional birth attendants and mothers in safer childbirth practices. They have delivered training in

The focus of the teaching is management of the most common emergencies in maternity/obstetric care ie haemorrhage, sepsis, pre-eclampsia/eclampsia, complications of labour (eg obstructed labour).

No projects were possible due to the pandemic.

WORKSTREAM 7: EMERGENCY MEDICINE
Description: The purpose of the Emergency Medicine (EM) stream is to improve patient outcomes in Emergency Departments (EDs).
Activities undertaken:
The training programme in Emergency Medicine (EM) in SW England has started to award Global Health Fellowships and placements were wanted. One of these has been arranged through the charity at Nanyuki Hospital. The first was planned to take place in January 2020. The lead for the SW Training Programme and for the Fellowship programme is Dr Andy Lockyer, Consultant in EM in Bristol. The training programme will determine the work and provide the funding and FHA will provide guidance and governance for the Fellows, having had an 11-year relationship with Nanyuki Hospital which has included work in the Emergency Department. An MoU is in place describing the partnership arrangements.
Activities undertaken:
Emergency Medicine registrars from the Bristol training programme started regular placements in the Emergency department of Nanyuki Teaching and Referral hospital. They undertook teaching and improvement projects. The initial placement doctors returned early due to the pandemic but placements resumed later in the year.

WORKSTREAM 8: BURSARIES
Description:
When our charity resources allow, we support courses and study visits for LMIC individuals. These may take place in either Kenya or the UK.
Activities undertaken:
None.
WORKSTREAM 9 (emergent): STORYTELLING/THEATRE ARTS
Description: A new project is currently under development to promote improved health and wellbeing in teenagers, building confidence and improving attendance and performance at school. It will use storytelling and theatre skills as a medium.
Activities undertaken:
A scoping visit was provisionally planned for November but UK facilitators were unable to travel due to COVID-19. A grant application has been made to the British Council - outcome anticipated early in 2021.
WORKING WITH OTHERS

We confirm that trustees have regard to the guidance issued by the Charity Commission on public benefit and have considered https://www.gov.uk/government/publications/public-benefit-the-public-benefit-requirement-pb1 They conclude that FHA met public benefit requirements in 2020.
SOCIAL INVESTMENTS
The charity has not utilised social investment finance.
GRANTMAKING
FHA has not to date awarded any grants. Bursaries are decided upon using the charity's agreed policy 'Bursaries'.

ACHIEVEMENT AND PERFORMANCE
Charitable activities
The charity further consolidated its systems and processes during 2020 although activities were curtailed due to the global pandemic.
One trustee resigned in August for personal reasons (Simon Gittoes-Davies) and replacement was deferred as activity was low.
Very sadly, trustee Bob Grose died suddenly at the end of the year. He will be greatly missed.
The charity's achievements in brief were:
Trauma Team: Reduction of pain and disability for patients referred to them through 2 projects. New partnerships were fostered.
GRASPIT: Healthworkers trained in vital skills and further roll-out explored with potential partners.
q4a: DFID grant won and QI and leadership training was delivered in Migori in collaboration with new partner LVCT Health.
Community: technical assistance given to Kenyan NGOs and conservancies
TeamTalk: life-chances of rural children, especially girls enhanced through one UK led project with subsequent followthrough by the Kenyan team. Support from the Atlas Foundation continues.
Maternity: rural healthworkers, traditional birth attendants and mothers are educated in dealing with life-threatening problems during childbirth.
Emergency Medicine: Global Health Fellowship placement established.
Public benefit
All projects comply with the requirement for public benefit.
The TRAUMA clinical projects have sought to alleviate pain, disability and suffering through direct patient care. Treatment has been open to any member of the public in Kenya and is particularly directed at those who cannot otherwise access care because of geography or poverty.
GRASPIT and q4a teach healthworkers the skills they need to enhance care of the public who they serve. Community projects enhance medical services available to communities and individuals.
TeamTalk is directed at improving the life-chances of young rural children and is available to all.
Maternity and Emergency Medicine projects improve education in these specialities for the benefit of any patient receiving services from those trained.
Quality improvement and leadership skills training is pursued with the prime aim of enabling health workers to improve healthcare for their communities.
The charity trustees believe that the projects deliver the requirement for public benefit and volunteers also gained personal and professional development that they will apply for further public benefit in their individual environments.
Fundraising activities
All necessary funds were raised from volunteer contributions, donations and grants.
FINANCIAL REVIEW
Financial position
At the end of the financial year on 31.12.20, the charity had assets of £82,972, £64,219 of which was restricted. Funds are held to cover future activity, including planned trips to Kenya.
The charity reserves £10,000 as contingency in any one year to cover any unanticipated events or incidents.

Reserves policy
Trustees allocate a reserves figure when agreeing the activity plan for the forthcoming year. It is based on the nature of the projects planned for that year and is to cover any unexpected events.
As at 31st December 2020 the charity held free reserves of £18,753. Although this was in excess of the stated policy, it will enable the charity to support projects that are unlikely to raise sufficient income during the following year.

STRUCTURE, GOVERNANCE AND MANAGEMENT Related parties
FHA has a tri-partite Memorandum of Understanding (MoU)with the Laikipia County Government and Torbay & South Devon NHS Trust.
MoUs are in place for Trauma projects with Marsabit County Government.
GRASPIT has a partnership registered with THET with the National Resuscitation Council for Kenya.
A partnership arrangement with the Kenyan NGO LVCT Health was signed on 20th November 2020 with the purpose of co-delivering the DFID SCCF grant awarded to FHA's q4a faculty
Through KJ's advisory role to the Board of the new Society for Quality Healthcare for Kenya, a close informal relationship is starting to evolve.
University of Nairobi - Dr Vincent Mutiso, Head of Orthopaedics is now working closely with the Trauma Team. The Kenya Orthopaedic Association is also liaising with the Trauma Team.
Several FHA key members are also associated with the University of Plymouth (Faculty of Health Sciences). This relationship involves participation in the Global Health Collaborative, the Masters' degrees in Global Health and Healthcare Management Innovation & Leadership. Key FHA members were involved in organising and presenting at the University's first international conference on Global Health partnerships.
FHA retains close links with the Tropical Health & Education Trust (THET).

Independent examiner's report to the trustees of Future Health Africa
I report to the charity trustees on my examination of the accounts of Future Health Africa (the Trust) for the year ended 31 December 2020.
Responsibilities and basis of report
As the charity trustees of the Trust you are responsible for the preparation of the accounts in accordance with the requirements of the Charities Act 2011 ('the Act').
I report in respect of my examination of the Trust's accounts carried out under section 145 of the Act and in carrying out my examination I have followed all applicable Directions given by the Charity Commission under section 145(5)(b) of the Act.
Independent examiner's statement
I have completed my examination. I confirm that no material matters have come to my attention in connection with the examination giving me cause to believe that in any material respect:

accounting records were not kept in respect of the Trust as required by section 130 of the Act; or

the accounts do not accord with those records; or
-
the accounts do not comply with the applicable requirements concerning the form and content of accounts set out in the Charities (Accounts and Reports) Regulations 2008 other than any requirement that the accounts give a true and fair view which is not a matter considered as part of an independent examination.

1. ACCOUNTING POLICIES
Basis of preparing the financial statements
The financial statements of the charity, which is a public benefit entity under FRS 102, have been prepared in accordance with the Charities SORP (FRS 102) 'Accounting and Reporting by Charities: Statement of Recommended Practice applicable to charities preparing their accounts in accordance with the Financial Reporting Standard applicable in the UK and Republic of Ireland (FRS 102) (effective 1 January 2019)', Financial Reporting Standard 102 'The Financial Reporting Standard applicable in the UK and Republic of Ireland' and the Charities Act 2011. The financial statements have been prepared under the historical cost convention.

All income is recognised in the Statement of Financial Activities once the charity has entitlement to the funds, it is probable that the income will be received and the amount can be measured reliably.
Expenditure
Liabilities are recognised as expenditure as soon as there is a legal or constructive obligation committing the charity to that expenditure, it is probable that a transfer of economic benefits will be required in settlement and the amount of the obligation can be measured reliably. Expenditure is accounted for on an accruals basis and has been classified under headings that aggregate all cost related to the category. Where costs cannot be directly attributed to particular headings they have been allocated to activities on a basis consistent with the use of resources.
Taxation
The charity is exempt from tax on its charitable activities.
Fund accounting
Unrestricted funds can be used in accordance with the charitable objectives at the discretion of the trustees.
Restricted funds can only be used for particular restricted purposes within the objects of the charity. Restrictions arise when specified by the donor or when funds are raised for particular restricted purposes.
