need by the provlslon of a Parish
Nurse,.
advanclng the Chrtstian faith in the
wlder community by offering a
service of exGellenGe and value
through the Parish Nurse project,
which will be of ben8fit to people of
any faith or none (generally
advanclng the religlon)., and
advancing Ihe Christian faith by
sensitively encouraging service
users, where appropriate, to explore
the Christian faith for themselves,
offering prayer, inviting attendance
at local churches and ultimately
seeklng to bring them to a p8rsonal
Christian faith.
The relief of those in need, by reason of
youth, age, ill-health, disability, financial
hardship or other disadvantage in Croxley
Green, Herttordshire. In furtherance of this
object, but not further or othe￿Ise, the

The Parish Nurses provide advice and
support to residenls in Croxl8y Green in
r8lation to their health n8eds in partnership
with the local GP'S and other health
professionals. This work is ¢arri8d out
through either individual contacts or through
group activities. They provide health
education, advice, family support and where
appropriate initiate the development of local
support groups. Further infomiation about

During 2023, volunteers have had to
step back from supporting the seM¢e for
personal reasons, however two new
volunteers have joined the service so there
are 4 volunteers in total.
A programme of training has been devlsed
for volunteers.
Volun188rs support the Parish Nursing
Service through one to one visiting,
assistant at support groupsltraining events
and administration. Our volunteers are an
invaluable asset to our servir*.

During 2023. th8 Parish Nurses continued
to provide bespoke care and support to the
people of Croxley Green. This took the
form of individual work, group work, support
to local drop In cafes, SUPPOrt groups,
Toddler Groups etc. Work with local health,
social care, churches and businesses
identified a gap in providing bespoke
support and Gare for people with Dementia
and their carers. As a result of this a bid
was successfully $8cur8d from th8 National
Lottery Community Fund for £10.000. This
In turn is funding one of the Parish Nurses
to work an additional 1 day a week lo
d8velop Demenlla fri8ndly provisions 18
churches café's, pubs 8tc and local
support groups.

The following case sludy demonstrates a
fairly typlcal Intervenllon by a Parlsh Nurs8
,A vulnerable lady referred lo the Parish
Nurse with her consent was seen al her
home. This lady was al risk of self-neglecl
and her ability lo live independently was a
concern. On assessment, Il was clear she
had infections in hey legs and some concerns
with her memory, With her consent, I spoke
with the Duly GP and arranged for a double
appointment the following day which. in
absence ol family members. I was able lo
accornpany her lo. Anlibiolics along with
olh9r medications were prescribed. On my
request a Ooselle Box was requested by the
GP to the pharmacy and three 'next slep
appointments were made by the GP for follow
up appointments. INilh this lady s consent, I
contacted her family, who were grateful for
the Parish Nursg input (see below) which
likely resulted in this lady not having an
emergency admission lo hospital al a later
date and safety concerns being highlighted to
the relevant services. Due to the Parish
Nurse inlervenlions and health planning,
social care will be assessing her needs.

and its positive impact.

REVIEW
This 15 just a note to register our deep
appreciation r)f the service provided by your
Croxley Green Parish Nursing Team recently
encountered In Connection with a family
member living in the All Saints Parish.
Th8 delicate. diplomatic and gentle
discussions held with the recipient, in the
presence ol my wife Irecipienl's relalionl 8nd
myself, was an inspiration as lo how effective
an appropriate and meaningful amount of
lima coupled with a proactive allilude can
produce reassurance and results in a very
short period ol lime.
The follow up and patience also received to
date, whilst understandably limited in the
professional lime allowable is impressive,
considerale, polite and exceptionally helpful.
The relief we family members feel now that
hurdles have been overcome and a plan of
campaign formed is immense and lor which
we are all very very gral8lul.
Wod< such as this is not uncommon for us.
The overstretched NHS and Social Care
Services are un8ble to provide the time to
listen to the bigger picture of what is going
on behind closed doors. s8lf-nègl8Ct,
domestic abuse - Including financial,
substance and emotional abuse - Isolation.

support and health promotion are common
themes which run through our Parish Nurse
work..

£17,039. As in 2021 and 2022, thère were
no fundraising events. Gift Aid with interest
totalled £1,020. After expenditure of
£18,053 cash at bank as at 31 December
2023 was £27,186.
