The trustees present their report along with a consolidated financial statement of the charity for the period 01/07/2020 to 30/06/2021. The financial statement was prepared according to the Charity Commission guidelines and confirm they comply with the requirements of the Charities Act 2011, the trust deed, and the Charities SORP (FRS 102). SADEAF is a charity registered with the Charity Commission under registration number 1178260. SADEAF is a Charitable Incorporated Organisation (CIO), whose only voting members are its charity trustees. 

## **Mission Statement:** 

To reduce the incidence and impact of stroke and diabetes by developing comprehensive education and training programs focused on prevention, treatment, rehabilitation, and support for all patients impacted by stroke and diabetes in Sudan. SADEAF will particularly target the poorer and the needy sectors of communities who are unable to afford the costs of healthcare. SADEAF aims to provide its services free of charge. Our long-term goal is to help establish stroke units in all major centres well-resourced with highly skilled and trained personnel. 

## **Objectives:** 

The objectives we set reflect our aims to promote health through fighting stroke and diabetes in Sudan. These objectives are, however, set in the context of the broader goals we set for the charity. In setting our objectives and planning our activities SADEAF trustees have followed the Charity Commission’s public benefit guidance. 

## **Our long-term objectives are:** 

- 1- To support Stroke and Diabetes care in developing countries through training of allied healthcare professionals in developing countries. 

- 2- To encourage medical professionals and support staff to receive training that enhances their skills locally in their countries and to facilitate further training of key individuals in Britain. 

- 3- To train a group of professionals to become trainers to continue delivering training in developing countries. This is a long-term objective to be delivered later. 

- 4- To support on-going programs aiming at prevention and early recognition of Diabetes and Stroke. 

- 5- To establish hyperacute thrombolysis service for stroke patients.

- 6-To supply essential rehabilitation and diabetes care equipments. 

7-To promote research.

in England, through organising health education activities. 

## **Review of activities and achievements** 

## **Activities** 

Stroke is quite common in the Sudan with severe impact on patients, families, and society. Care for patient with stroke and diabetes is extremely poor, and proper stroke units do not exist there. SADEAF believes that the best method to achieve its long-term goal to improve the care for patients with stroke and diabetes in Sudan is through establishment of a high-quality stroke centre that provide high-quality services and training. This centre is then functions as a central training unit equipped with highly trained staff who will then trains other new staff to establish new stroke units. 

The activities that are planned for the public benefit in collaboration with our partners are: 

1. Establishment of a partner organization in Sudan to participate in running the stroke units, fundraising, and help to raise the stroke awareness in the community. 

2. Health education as one of our mainstay measures in our fight against stroke and diabetes. 

3. Establishment of new stroke units and diabetes centres or renovation of the already existing units and providing essential equipment.

## **How will the charity work?** 

The board of trustees sets the plans for the charity activities according to the charity long term objectives and goals. The activities of SADEAF are governed by clear and robust internal policies that set to safeguard the charity against any risks. These policies were approved by the board of trustees on 17[th] of December 2017. These policies were derived from the guidance of the Charity Commission for England and Wales. These policies come under four major headings: 

## **1: Due Diligence Policy** 

## **2: Anti-Money Laundering (AML) Policy** 

## **3: Anti-Terrorist Finance Policy** 

## **4: Risk Management Policy.** 

The charity activities are planned to be set according to the Charity Commission guidelines for public health benefits, aiming to significantly reduce the burden of stroke and diabetes on the community in Sudan. The trustees will ensure that any activities carried out should be in accordance with the charity aims and objectives. 

We are planning to execute our objectives through two different geographical areas. 

- 1- Locally in the UK, where the charity is raising funds, setting the guidelines for operation, and construct plans to deliver its vision and goals. Also, SADEAF will promote health, educating the public in UK about risks of stroke and diabetes through health check campaigns that can be used as fundraising activities as well. 

- 2- Overseas where the actual activities should take place in collaboration with our partners in Sudan. 

The trustees have identified that the care for stroke patients and diabetes is extremely poor in Sudan. There is only one stroke unit that has just started to work with extremely limited resources, and it is run by staff who was not professionally trained to care for stroke patients. The trustees have decided to take this opportunity to improve the services in this unit and develop it to standard stroke unit that serves as well as a central unit for training staff for other new units.

## **What did we do: Activities and achievements and performance?** 

This period is overshadowed by the global Coronavirus Pandemic which has affected all the charity activities. The charity during this period has experienced difficult times as two major events has determined the outcome of its activities and led to increase of the suffering during this period. The trustees are aware that the suffering of patients with Stroke and their families has increased due to the poor care for stroke patients in Sudan and due to the difficulties COVID-19 pandemic has created on patients’ physical wellbeing, social, mental, and financial status. This unprecedented situation has restricted the abilities of the health professionals working in stroke and in general to develop their full scope of management. The new measures to fight the coronavirus have led to cancellation of most of the face-to-face elective activities and training, but SADEAF is determined to adjust its strategies and plans to adapt to these new situations to achieve its goals through adoption of new and creative ways to overcome these obstacles and resume training and reopen the stroke unit in Soba University Teaching Hospital in Sudan. 

- 1- The first major factor was the long political turmoil that took place in Sudan in December 2018 followed by long political unrest and instability till June 2019 has continued and the political situation and the instability that has followed the new measures declared by the rolling Military group has even made the condition worse as the social, financial and health systems has nearly reach and state of collapse. This has resulted in cancellation of the second and third stage of training programmes which were planned to take place on January and April 2019, and May 2020 and May 2022, respectively, and this has resulted in delaying opening of the stroke unit in Soba Teaching Hospital and delayed all the activities planned, as all the recruited team of allied professionals has all resigned seeking other profitable jobs, because there were no activities since their recruitment. 

- 2- The second major factor was the outbreak of CVID-19 Pandemic, which has hit the world, and put the world at standstill position from December 2019 till now, and this has hindered any possibility to restart the work in the new stroke unit in Soba University Hospital in Sudan for the second time. 

Due to this extraordinary situation SADEAF charity is forced to rethink and adjust its plans looking for new creative ways to restart training again to prepare the new team to reopen the stroke unit again.

## **The main activities planned for the period 01/07/2020 to 30/06/2021:** 

- 1- To resume training through conducting the second and third workshops planned for the stroke unit team in Sob University Teaching Hospital. 

- 2- Improvement of SADEAF website contents through publishing Arabic language translation of scientific contents from The British Stroke Association. 

- 3- Establishment of a partner organization in Sudan 

**Details of the activities planned for year ending 30[th] of June 2021:** 

## **1- Resumption of the second and third stages of training.** 

The charity has conducted successfully the first stage of the training program for the new team in Soba University Hospital in Sudan in July 2018.  The charity was hopeful to conduct the second and third stages of training in January 2019 and April 2019 respectively, but unfortunately due to the Sudanese revolution that erupted in December 2018 and the long political turmoil and unrest that followed it and continued till after the end of the year 2021. The charity was forced to cancel all its activities in the stroke unit in Sudan. This was followed by the outbreak of the Coronavirus Pandemic, which has further delayed all activities from February 2020 till the end of 2021. 

The stroke training project is supposed to be **mentored by Dr. Indira Natarajan,** the clinical director of neurosciences and the stroke service lead at the Royal Stoke University Hospital, Stoke-on-Trent, UK. Dr Natarajan has got enormous experience in stroke service development. 

Unfortunately, all the charity plans to resume training from January 2019 till now has failed and all training programmes which were rescheduled to take place online in March - April 2021 has been put on hold as the political situation in Sudan and the Coronavirus Pandemic didn’t allow the Stroke Unit to start activity and as a result of this inactivity the recruited team of the allied health professional supposed to start the training has stopped working for the stroke unit and so the Stroke Unit in Soba University Hospital didn’t start again. 

Therefore, the Trustees has contacted Omdurman Teaching Hospital who are willing to establish a new stroke unit. They have provided two wards containing 40 beds and planning to recruit 20 members of staff to operate the stroke Unit. SADEAF agreed to provide training of the staff and provide the essential equipments necessary to start activity. 

The charity is waiting for the recruitment of staff to start training from stage one, but this has been delayed due to the current political unrest affecting the Sudan.

- **2- Improvement of SADEAF website contents** through publishing Arabic language translation of scientific contents from The British Stroke Association. 

After the charity was granted permission to publish on our website www.sadeaf.com some of the scientific contents of the British Stroke Association UK ( Stroke Association | Home) , SADEAF Charity decided to translate this scientific content and publish it for the use of Sudanese patients, families, and the public in general to raise the awareness of the public with stroke prevention, diagnosis, treatment, and prevention. 

This is a lengthy process, that should be performed by a certified translator. The process involves four main stages. First the initial language translation, second prove reading, thirdly checking for spelling and grammar mistakes, and then tabulation of contents before publication. 

For the translation to be eligible it should be culturally appropriated because it is particularly important for an Arabic audience to feel familiar with the contents. In the Arabic culture things like gender, religion, sexuality, politics, and food have significance and sometimes different meaning or interpretations. It is important to know your audience and that you get the words, images, logos, and colours are right. 

Unfortunately, the first attempt to perform this translation has failed, although the translator was a certified translator, the translation lacked the right scientific sense and lacked clarity and wasn’t tabulated in a manner that can be followed. This means this task remains to be completed. The charity is looking again for a qualified translators to finish this important educational task. 

Of note this translation was funded by a generous donation from Dr Mohammed Gadour the charity trustee and a chairman. 

## **3- Establishment of a partner organization in Sudan** 

SADEAF application to **stablish a partner organization in Sudan** which was submitted in June 2018 **is approved and now the partner organization is fully operational** awaiting the start of the new stroke unit in Omdurman Teaching Hospital to start collaboration. 

**C** ollaborating with our local partner in Sudan is thought to be vital because it will allow the charity to utilize the local knowledge to run the charity activities and help in health education, and to help in fund raising as our charity is not allowed to transfer funds abroad and all local activities costs should be sourced locally. The partner organization will represent our charity in Sudan to facilitate fund raising that helps to maintain the stroke unit, and help to supply essential equipment, and liaise with the local authorities regarding administrative issues. Also, the participation of the local partner will help to raise the public awareness for stroke and diabetes.

## **Financial report** 

## **Our finance:** 

During the period from 01/07/2020 to 30/06/2021 the net funds SADEAF has increased by 14,335.00 and the total amount in the account by the end of June 2021 was £35,397.00. This entire amount is still in reserve. 

The charity did not spend any funds during this period, due to the political situation in Sudan and the Coronavirus Pandemic which led to cancellation of all charity activities. 

The principal source of income for SADEAF is the regular monthly donation to the charity bank account by our community in Britain through direct debit or standing orders. 

SADEAF strategy is built on educating and training of new stroke team members in the developing countries mainly Sudan, therefore all the funds in reserves are kept for this purpose. The trustees and generous community members have funded all the activities up to the end of June 2021, and all the reserves of the charity has remained save to facilitate the trip and the educational materials for the stroke expert trainers team from UK (Royal Stoke Hospital in Stoke-on-Trent) to Sudan to conduct the third and final stage of training when normal life resume. 

SADEAF has no expenses as it does not pay tax or national insurance contributions, because SADEAF do not hire any employees and the execution of all the charity tasks and activities is shared between the trustees voluntarily to allow the charity spends its funds only for the purpose of training. 

The trustees are responsible for keeping proper accounting records regarding income and expenditures and they ensure that the financial statement presented to the Charity Commission comply with the Charity Act 1993, and the Charity Regulations 2008. The trustees are also responsible for safeguarding the charity assets and ensure that they take all the necessary measures to prevent and detect frauds and money laundering and other irregularities.

## **SADEAF will adopt the following strict procedures to verify the donors and consider any conditions related to any donations:** 

- 1- SADEAF does NOT accept donations from outside the United Kingdom. Any donations coming from outside the UK will be rejected. 

- 2- Donations will only be accepted from bank accounts holders in banks inside the UK. 

- 3- SADEAF will ask for donations in the form of standing orders OR direct Debits coming directly to the charity bank account. 

- 4- SADEAF will NOT accept any cash donations. 

- 5- SADEAF will only accept substantial once off donation if it complies with the SADEAF Due Diligence policy (SADEAF Internal Policies) and the Charity Commission guidelines and regulations. 

SADEAF will be very transparent about all its financial activities and will ensures that there are regular checks taken with the help of specialist professionals such as certified accountants, lawyers, or tax experts. We will also use Tools 6 and 7 advised by the charity commission (Appendix 1), and SADEAF will promptly report any suspicions regarding any transactions to the appropriate authorities such as the police or HMRC or the Charity Commission.

## **Futures plans:** 

SADEAF trustees have reviewed their current strategies due to the current difficult and unprecedented situation that has delayed establishing any functioning stroke unit or provide any significant training to help to provide good care for patients suffering stroke and diabetes in Sudan. 

Two main difficulties facing the charity and the health system in Sudan in general are the political unrest that started since 2018 till now, this has made the health system unable to agree any plans to establish new units or agree any new recruitments of staff. The second factor is funding the stroke units to provide the running cost. 

The charity has focused its attention to the following trying to overcome these obstacles to help improve the care for stroke and diabetes patient in Sudan. 

## **SADEAF plans in the year 01/07/2021 to 30/06/2022 will focus mainly on:** 

- 1- With help of our newly approved partners organization in Sudan SADEAF will work hard to recruit a new team of allied professional to open the stroke unit in Omdurman Teaching Hospital and make it a training centre for future stroke units in other hospitals. 

- 2- Restart of practical training when the new team is recruited from stage I to stage III. Training initially will be online with possible face to face training in future depending on the time the new team is recruited and ready to start training. Due to the current situation SADEAF will try to utilize the available media like; Zoom, MS Teams, skype, etc, to do the training online specially for stage I and II, and part of stage III of training, and conduct face to face practical training when life go back to normality. 

- 3- Complete the Arabic language translation of scientific contents from The British Stroke Association and publish it on the charity website www.sadeaf.com. 

The trustees will continue to review and update their plans, tools, and policies to ensure that these strategies are up to date and comply with our charity objectives and with the charity commission policies and guidelines.

## **Financially SADEAF will work hard to increase its income through considering other ways of fundraising resources:** 

- 1- Organising fundraising events like charity dinners, 

- 2- Organising community campaigns to raise the awareness of the public regarding stroke and diabetes, where the charity can recruit our fellow medical professionals to do public clinics to check weight, blood pressure, sugar and cholesterol, and smoking cessation, and ask for donation. 

- 3- Distribution of donation pots on local shops. 

- 4- Also, the charity will consider investments that hope to generate more income and will seek tax exemptions for these activities.

## **What did we do: Activities and achievements and performance?** 

The continuous political turmoil and unrest and violence in Sudan that has erupted to state of war and fighting on 15[th] of April 2023 has halted the charity activities. These political unrests are continuously going since the overturn of the previous regime in 2019. The charity during this period has experienced difficult times as the continuous war has led to destruction and vandalization of almost all hospitals all over Sudan. Although some of the people who migrated out of the main cities started to return to their homes and the major health centres started working with very limited resources, therefore patients care has moved to peripheral hospitals that are not well equipped to care for patients with stroke. The trustees are aware that the suffering of patients with Stroke and their families has increased due to the poor care for stroke patients in Sudan and due to these difficulties has adversely affected patients’ physical wellbeing, social, mental, and financial status. This unprecedented situation has restricted the abilities of the health professionals working in stroke and in general to develop their full scope of management. These unprecedented situations have led to cancellation of all activities and training planned by the Charity. 

Although SADEAF was determined to adjust its strategies and plans to adapt to these new situations to achieve its goals through adoption of new and creative ways to overcome these obstacles and resume training and reopen the stroke unit in Soba University Teaching Hospital in Sudan, but the continuous violence has created an unsafe environment that make any activity impossible to deliver. Hopes are now high that the opposing political parties to reach an agreement helped by The United Nation Organisation will lead to stability that allow SADEAF to start its activities and fully implements its mission. 

Due to this extraordinary situation SADEAF charity is forced to rethink and adjust its plans and start from the beginning again, exploring new creative ways to restart training to prepare the new team to reopen the stroke unit again.

## **The main activities planned for the period 01/07/2024 to 30/06/2025:** 

- 1- To restart training when the political situation settles to re-establish the stroke unit in Soba University Teaching Hospital or any other suitable hospital for example Oum Durman Hospital. 

- 2- To restart and refresh collaboration with our partner organization in Sudan to re-equip and fund the stroke units when reestablished. 

- 3- To continue fund raising to achieve the above goals. 

## **Details of the activities planned for year ending 30[th] of June 2024:** 

## **1- Resumption of training.** 

SADEAF will start training the new stroke team in Soba University Hospital in Sudan when normal activities resume and a new team is appointed. Unfortunately, the successfully conducted first stage of the training program in Soba University Hospital in Sudan in July 2018 is now obsolete and SADEAF have to start the training from the beginning again. The charity was forced to cancel all its activities in the stroke unit in Sudan. This was followed by the outbreak of the Coronavirus Pandemic, which has further delayed all activities from February 2020 till the end of 2021, and the continued political turmoil that is still going. 

The stroke training project is supposed to be **mentored by Dr. Indira Natarajan,** the clinical director of neurosciences and the stroke service lead at the Royal Stoke University Hospital, Stoke-on-Trent, UK. Dr Natarajan has got enormous experience in stroke service development. 

Unfortunately, all the charity plans to resume training from January 2019 till now has failed and all training programmes which were rescheduled to take place has been put on hold due to the political situation in Sudan.

The previous agreement with Omdurman Teaching Hospital in Sudan to establish a new stroke unit is on hold due to the previous mentioned political unrest which further escalated to a full state of war still ongoing since April 2023 till now. SADEAF has agreed to provide training of the staff and provide the essential equipments necessary to start activity if that Stroke Unit was established. This agreement should now be revisited when the political and security situations allow.

## **2- Reestablishment of a partner organization in Sudan** 

SADEAF partner organization in Sudan which was fully operational before the war erupted will need to be re-established, as the war has led all its activities to hold. 

**C** ollaborating with our local partner in Sudan is thought to be vital because it will allow the charity to utilize the local resources to run the charity activities and help in health education, and to help in fund raising as SADEAF in Uk is not allowed to transfer funds abroad and all local activities costs should be locally sourced. The partner organization will represent our charity in Sudan to facilitate fund raising that helps to maintain the stroke unit, and help to supply essential equipment, and liaise with the local authorities regarding administrative issues. Also, the participation of the local partner will help to raise the public awareness for stroke and diabetes.

## **Financial report** 

## **Our finance:** 

During the period from 01/07/2024 to 30/06/2025 the net funds SADEAF has increased by £4753.00 and the total amount in the account by the end of June 2025 was £58,906.00. This entire amount is still in reserve. 

The charity did not spend any funds during this period, due to the continuous state of war in Sudan which led to cancellation of all charity activities. 

The principal source of income for SADEAF is the regular monthly donation to the charity bank account by our community in Britain through direct debit or standing orders. 

SADEAF strategy is built on educating and training of new stroke team members in the developing countries mainly Sudan, therefore all the funds in reserves are kept for this purpose. The trustees and generous community members have funded all the activities up to now so far, and all the reserves of the charity has remained save to fund training and educational in Sudan to when normal life resume in collaboration with stroke expert trainers team from UK (Royal Stoke Hospital in Stoke-on-Trent). 

SADEAF has no expenses as it does not pay tax or national insurance contributions. SADEAF does not hire any employees, and the execution of all the charity tasks and activities is shared between the trustees voluntarily to allow the charity spends its funds only for the purpose of training. 

The trustees are responsible for keeping proper accounting records regarding income and expenditures and they ensure that the financial statement presented to the Charity Commission comply with the Charity Act 1993, and the Charity Regulations 2008. The trustees are also responsible for safeguarding the charity assets and ensure that they take all the necessary measures to prevent and detect frauds and money laundering and other irregularities.

## **SADEAF will adopt the following strict procedures to verify the donors and consider any conditions related to any donations:** 

- 1- SADEAF does NOT accept donations from outside the United Kingdom. Any donations coming from outside the UK will be rejected. 

- 2- Donations will only be accepted from bank accounts holders in banks inside the UK. 

- 3- SADEAF will ask for donations in the form of standing orders OR direct Debits coming directly to the charity bank account. 

- 4- SADEAF will NOT accept any cash donations. 

- 5- SADEAF will only accept substantial once off donation if it complies with the SADEAF Due Diligence policy (SADEAF Internal Policies) and the Charity Commission guidelines and regulations. 

SADEAF will be very transparent about all its financial activities and will ensures that there are regular checks taken with the help of specialist professionals such as certified accountants, lawyers, or tax experts. We will also use Tools 6 and 7 advised by the charity commission (Appendix 1), and SADEAF will promptly report any suspicions regarding any transactions to the appropriate authorities such as the police or HMRC or the Charity Commission.

## **Futures plans:** 

SADEAF trustees have reviewed their current strategies due to the current difficult and unprecedented situation that has delayed establishing any functioning stroke unit or provide any significant training to help to provide good care for patients suffering stroke and diabetes in Sudan. 

Two main difficulties facing the charity and the health system in Sudan in general are the political unrest that started since 2018 till now, this has made the health system unable to agree any plans to establish new units or agree any new recruitments of staff. The second factor is funding the stroke units to provide the running cost. 

The charity has focused its attention to the following trying to overcome these obstacles to help improve the care for stroke and diabetes in Sudan. 

## **SADEAF plans in the year 01/07/2024 to 30/06/2025 will focus mainly on:** 

- 1- To re-establish connection with our partners organization in Sudan SADEAF to work hard to establish new stroke units in Teaching Hospitals and make it a training centre for future stroke units in other hospitals. 

- 2- Restart of practical training when the new stroke unit is re-established, and a new team is recruited from stage I to stage III. Training initially will be online with possible face to face training in future depending on the time the new team is recruited and ready to start training. Due to the current situation SADEAF will try to utilize the available media like; Zoom, MS Teams, skype, etc, to do the training online specially for stage I and II, and part of stage III of training, and conduct face to face practical training when life go back to normality. 

The trustees will continue to review and update their plans, tools, and policies to ensure that these strategies are up to date and comply with our charity objectives and with the charity commission policies and guidelines.

## **Financially SADEAF will work hard to increase its income through considering other ways of fundraising resources:** 

- 1- Organising fundraising events like charity dinners, with help of a professional fundraisers 

- 2- Organising community campaigns to raise the awareness of the public regarding stroke and diabetes, where the charity can recruit our fellow medical professionals to do public clinics to check weight, blood pressure, sugar and cholesterol, and smoking cessation, and ask for donation. 

- 3- Distribution of donation pots on local shops. 

- 4- Also, the charity will consider investments that hope to generate more income and will seek tax exemptions for these activities.
