The piirpose of the chality is the advancement of health for public benefit by improving the availability,
delivery and quality of acute medical care through tlie development of clinical training for healthcare
prolesslona15, clinica5 teams and healthcare services. The charity has a particular (but not exclusivel focus in
Low and Middle Income Countries ILMICI by promoting translational research and collaborative working

and ultimately improve health care sy5tem5. Facilitating research and enab5ing quality improvement 5UPPOrt
these priorities. Better training and better healthcare delivery Systems lead to better patient care, providing
publtc benefit.
The charity kcliieves ihese objectives predominantly through a partnership with NICS-MORU. NICS-MORU
is Not-for-Profil O¥gaiiisation INPOI registered in Sri lanka. aiming to improve acute and critical healthcare
through the development, maintenance and improvement of systems enabling routine Surveillan￿ of care.
The N po, based in Sri Lanka. established in 2015 was pivotal in the development of one of the first national
critical care registries in a LMIC. and remains at the forefroni of developing and implementing systems to
enable evaluation and improvement of acute care service5 in Asia and Africa. NICS-MORU has the expertise
to be able to 5UPPOrt NICST in meeting its charitable objectives. NICST and NICS-MORU share common
objectives of capacity building and health system 5trengtheninB. A Checklist 3: Charitie5 in a regular
partnering or funding relatioiiship with a non-charity that is not its founder or subsidiary document has been

objectlVES of the partnership. NICS-MORU employ5 LMIC based healthcare workers and scienlists, providin8
einployment and opportlinily fur setting based research in the reglon. NICS-MORU employ5 a local tean) of
17 FTE researchers and research support staff, data collectois, web application developers. NICS-MORU
activities are non profit making with all charitable donations to the orEalli5ation going directly towards
achieving the above objectives. Of note, Rashan Hanifla, first trustee ol NICST is a director of NICS-MORU.
His role in botl) organisations is voluntary, and the activilies of both organisations doe5 not provide him with
aiiy fii)3ncial remunerJtion. All Cliaiity trustees have completed a conflict of interest form.
2.0 Achievement5 and performance: how our objertives delivered public benefit.
Tl)e benefits of NICST'S work are,. the professional mentorship and Formal academic training for clinical,
acadei#ic and rÈsearch team members. Ihe development of new and strengthening of existing inf rastructure
for data driven research lobservational and inteNentiotiall and the funding of on-going research to aid care
of patients including those with critical illness and non communicable diseases. The trustees are pleased that
the activities, impact, learning ènd achievements of the charity, as set out below, reflect these goals.
The NICSTINICS-MORU critical and acute care sytweillante platform remoins operational In 17 countries
across Asia and Africa. The surveillance platforni; o central terlet of a 3 year 15 country Wellcome-
Edinburgh-oxford. "CRIT Care I￿53 Africa °award funded from 2023-25. wemains central to the newly
awarded Wellcome Award "CRIT Cafe Asia- Africa~ funded from 2023-2025, of which NICS- MORU is the
project implementation and dala coordinatirhg centre. Key deliverables of this new projert are,. transition
to federated registries as a pathways to future 5UStainability, adaptstion of the registry to enable data
driven platform trials, and probabilistic modelling, capacity buildinE lor quality iinprovement and facilitatlon
of international research to improve outcomes for critically ill patients, including through pandemic
surveillance, multi morbidity and infectious disease management and improved patient experience. NICST
and NICS-MORU wlll continue to provide expertise in implementation, data coordination, curation and
analysis of data for Service evaluation. clinical trials and quality improvement. As such the objectives of the
charity will align with the deliverables for the Wellcome funded proje￿ and will be a leading priority for the
charity for 2023-2025. The surveillance plaiform provide5 a mechanism for continuous evaluation of quality
in healthcare acros5 multiple countrles. Data capturèd through the platform is enabliDg patients and
healthtare providers to engage in high qualtty inlernational observational and interventional research. Data
der¢ved from registry ovtput is enhancing knowledge in response to the glob31 COVID 19 pandemic, and
witl)in the wider global health and quality improvement community for regional healthcare priorities,
inclvding infectious disea5e5. injury and psycholoEicJI outcomes follov4ing critical Illness.
NICST has 5upporled the initiation and expansion of registries in Asia-Africa, where there was little to no
mechanism to provide systematic routine evaluation of the quality of critical cafe and no comprehensive
information orh critical care infrastructure. The information measured through the platform is directly
accessible to the frontline clirsician team with the aim ol improving individual patient care. The aggregate
information captured through the platform has enabled evaluation of proce55es and quality of care for
patienls who are critically and aclrtely unwell. for example. infection rate5, delays In recovery. and mortality
following hospital dischaEBe. Thi% work is published in peer reviewed, clinical academic joumals.
Continuing the succe55ful work delivered in 2020-22, in 2023-25, the plattorm contlnues to be developed
to support registry enabled clinical research (including trials} led by LMIC rese8rcher5. NICSTI NICS-MORU
have grown to be the data coordinating centre for ir)tern3tional cllnical trials,- REMAP- CAP and Me
and for regional LMIC led studies incltkding ￿ARISE and BREATHE. NICST and NICS-MORU further play a

cenlral role in trial coordination, monilor¥n& patient $3fety, and data management prior to analysis. Within
this Tole NICSTI NICS MORU has supported research naive healthore communities In Asia to Impleinent
clinical trials, together seeking solutions to known barriers to clinical triaLs, including ethical, regiilatory and
resource barriers. To dale NICST/ NICS-MORU hhs ei)abled the recruitment of 2300+ patients to clinical
In 2022-25, as part of its objettwe io build research capacity, NICST and NICS MORU will sUPPOrt PhD
candidates from Asia alld Africa lo lead work to evaluate antimicrobial presctibing practices in critical care
and implement antimicrobial stewardship programs, included registry enabled audit and feedback, and
stewardsliip guidelines.
Commenced in February 2023, aligned with its core objectives to build research capacity in LMICS, NICST
has partnered with the James Lind Alliance. to undertake a patierbt, public and clinician led research prioritv
setting exercise. Th(s project inlends to idenlify population specific and scalable research quest1Dns, wliich
reflect the priorities of those delivering. and receiving critical care in LMICS. The projert is underway and
planned for completion %n lune 2024. MO￿ information on this project can be found here.
NICST continues its collaboration with University College London and AIGHD (The Netherland51, to facilitate
opportui)ities for higher education IMSCI in periopor3tNe medicirie and fellO￿hIpS global health and
quality improvement methods for he6lthcare worker5, primarity from LMICS. Thi5 work Is under revlew for
publication. The work continues to 5UPPOrt the QI and education platform wl)ereby experts and clinician5
from LMICS provide resources for health Ca￿ professionals seeking to improve the quality and experience
of care for patient5 in Asia and Africa. These resources continue to benefit the ongoing Wellcome CCAA
network.
NICST has sought to translate learning from projects in LMICS to support improving care for critically ill
patients in the UK. In partnership with the Universily of Hertfordshire, NICST has supported the
development and pilot of a patient and famÉly communication application. The pilot arEd evaluation is
currently in progress and will be reported in the 2023 reporting peri￿.
NICST and NICS MORU continue their partnership with AIGHD (Netherlandsl (established in 20191 to
provide opportunities for new career researchers and clknicians to e￿gage in bi directicnal research
proKrammes. Twelve academic fellows have successfully completed MSC in perioper3tive medicine and
global health, coniributing to the charity's projects in Asia and Africa. More information about this

In 2022, NICST contributed to the Lancet Commission WFSA Utstein metric for Perio
erative Dutcome
m*tric development of an International dataset to benchmark outcomes for surgical activity and Iritically
ill 5uroical patients which are now being evaluated.
Founded in 2019, the collaboration between University of Birmingham. Institute of Applied Health Research
and University ol jaffna, Department of Communily and Family Medicine has continued to flourish. Tlie
partnership has resulted in the suicessful completion of health servicos evaluation of existing AF and NCD
patliways in Sri Lanka, identifying barriers to quality of NCD care. This published work is now informing the
development of patierbt led community engagemenl prog¥3mmes to improve access and refefral for NCD
community care. This work commenced in 2022. is being undertaken by clinician5 and PhD candidates based
in Sri Lanka. Thi5 work is part of an NIHR funded Global Health Research Grou
for AF mana
einent,.
spanning Brazll, Sri Lanka and China. NICST continues to support Sri Lank3 based collaborators Iuniverslty
of Jaffnal witli the evaluation and improvement of existing community to tertiary management of AF. This

project hhs recruited a longitudinal cohort for over 10,000 patients to monitor incidence and prevalence ol
non communicable disease. for which NICST 15 supporting coordination and curation of the data and
5UPPOrting utilisation of the data for analysis. Work published to date can be found here.
3.0 structuye, Governance and Management
3.1 Striicture
Tlie ¢l)arity is constituted as a chaTltable incorpo¥ated organisation ICIOI and 15 governed by a CIO foundation
conslitutiDn constituted in 2017. The CIO'S orgènisational structure CDnsi5ts of a trustee board of five first
Irustees. The CIOS trustees all contribute without remuneratiOFb or pay in aCCDrdance wilh the CIOS
foundation constitution. All decisions related lo the structure and governance of the CIO are made by the
Inembers of the CIO acting on bel)alf of the charity. As set out in our governing document, every charitv
trustee is a natural person, older than 16 years of age. Any new truslee will be appointed for a minimum of
three years by resolutiors and formal meeting of the trustees. Regard for skills. expertise anfl experÉence to
the contribution of the CIO 15 considered when selectin8 new trustees. Prior to appointment of a trustee the
fallowing will be made availablè to ihe truslee.. 3 copy of the governine document, a copy of the CI0,5 most
recent annual report, and a copy of the charitvs accounts for the most recent financial year. Tyustees are
required to disclose all relevant interes15 and registerthem with the CIO and in accordance with the charity's
policy withdraw from decisions where a conflict of interest arises. Notrustees have declared interests (except
s stated above) which confltct with their role in the CIO.
3.2 6overnarpce ond manooement
The CIO does not employ 3ny management or committees to act on its behalf. The trustees aim to meet face
to face annually, with a minimum of a quoyum in attendance. The trustee meeting was held remotely in the
first qLlarter in March 2022 online. All trustees were in atlendance. Email communication to implement the
actions of the meeting were completed during the second and third quarter5 of 2022.
In addition, email correspondence is used for all charity trustee communications. including spending
commitment5, interim progress updates and financial update5. These communications are archiv&il for
accouiitability and audit. A minimum of 2 trustees are in attendance for any decision to be made or whereby
decisions are sought via email, written SUPPOTt of a miiiimum of 2 trustees is required. Quèstions arising at
meetings are decided by an eligible majortty. In the case of equality of vote the chair will cast a second vote.
A chair is appointed for each meeting and has responsibility for the accurate recording, reporting
dissemiiiation and storage of minutes Jnd decisions.
3.3 Monitoring ochievement
The succes5 of the charit(s activilies is evaluated by the successful collaboration of the charity with
universities, ￿searCh institutions and the LMIC health care community. Success is further measured by the
ward of funding for academic research and development Irom UK and foreign scientific and philanthropic
fundiiig agencies. Translational research success is assessed by the timely completion ol planned research
projects and the publications in peer reviewed journals. The outcome of the researcl) 15 also assessed by the
identific3tioii of the changes or improvements to clinical or care practice5 Stemming from the research.

The charity trustees are responsible for preparing a tiustees, annual report and financial stètemenis in
accordanee with applicable law and United Kingdom Attounting Stand3id5 (United Kingdom Generally
Accepted Accounttng Practice).
The law applicable to charilies in England and Wale5 requires the charity trustees to prepare financial
statements for each year wliich give a true and fasr view of the state of affairs of the charity and of the
11)coming resources and application of iesources. of the charity for that period. In preparing the financial
statements, the trustees are required to-

observe the methods and princÈples in the applicable Charities SORP;

departures that must be disclosed and explained in the financial statements.

The trustees are responsible for keeping proper accounting records that disclose with rea50n3ble accuracy

provisigns of the CIO foundation constitution. They Bre also responsible for safeguarding the assets of the
charity and taking reasonable steps for the prevention and detection of fravd and other irregula¥ities.

The trustee5 are responsible for the maintenance and Trritegrity of the Charity and financial information
included on the charity's website in accordance with legislation in the United Kingdom governing the
preparation and dissemination of financial st3tements.
4.0 Contrtbution made by volunteers
Tlie charity and its triistees and beneficiaries are very grateful to the volunteers who contribute to achieving
the aims and objectives during this period. Given the ongoing global travel restriction5 of COVID-19, the
international fellowship commenced in the 2022 period. Ms Alba Ko¥ver and Dr Patirck Chappell, both MSC
Global He8lth students from AIGHD and UCL respectively successfully completed their fellowship5, for 2022,
ur)dert3kin8 mixed methods research re¥iew5 of implemeniation 5cieiice methods in IMICS. This work 15
currently undèr review for publication. In Jan 2023. post grad intern, Ms Emma Donley. joined NICST to
develop research Skills related to the Research priority setting project de5criL7ed above. Volvnteer5 continue
to be supported by a tA15tee to mentor thetn in theirvoluntary contributions. Support during overseas work
(suspended lor 2020-221 was provided by a support crew with face to face and remote communication as
required.
5.0 Financial Review
5.1 Trustee Fin(Jncial stutement
The trustees have prepared the financial report for the period in accordance wTrth legislation in the United
Kingdom governing the preparation and dissemination of financial statements. The trustees confirm that
proper accounting records disc105e with feasonable accuracy the financial position of the charity and that
these records compbl with the Charities Act 2011, the applicable Charitiès (Accounts and Reportsl
Reguldtions, and the provi5È0ns of the cio foundation constitution. They can report that there are no
irregularities within tl)e financi31 reports for this period. As per the Charities Act 2011, they have sought
independent review from Stephenson Smart.
5.2 Flnoncial report.
The charity has no investmeftts or endowments. The charity received £82857.97 during the reporting period.
The charity's principal source5 of funds duririg the period were in services to Monash Univer51ty for the
implementation of REMAP-CAP {£81854.071. The charity allocated funds to support the activities to achieve
objectives 1- 5. During the financial year the charity's expenditure was E35328.44, of which £29900.57 was
in charitablÈ donations, primarilytowards NICS-MORU projeits. The charity currently has no investments but
its trustee5 are aware of the social investment poliry. The charity face5 no financial untertaintie5 forthe next
linanci81 year. Fuiids have been secured to meet the projected commitments and the current deficit of the
cliarity. The Mot)ash Ui)iversily loan account from 202V22 has been ck)sed. The charity has no subsidiary
undertaklngs.
6.0 Risk management
7.0 Future plans for the charity
NICST'S success Fn achieving their chariiable objectives in 2022-23 15 a testimony to the generosity of the
volunteers contributions and the productivity of the collaboration with NICS- MORU and other healthcare
organisations, now funded by international grant awarding bodies. The trustees, will continue to
priorltise promoting the developmenl of clinical training for healthcaie professiona15, clinical teams and

healtlicare se¥vices. aiid advancing translational research. We continue to work with our key partners in
NICS- MORU and the CCAA coinmunity of practice, to develop and deliver clinical-academi£ and reseaich
traiiiing opportunities. We will build on the collaborations with Un5versily College London, Unlversity of
Oxloyd, UKRII MRC and Wellcome, with whom we have common goals to underlake trènslational
research. An area of fresh locus will be the developmeEJt of context relevant quality evaluation and
improvement methods With a 5pecKfic focus on improving primary- secondary and tertiary care pathwayst
aiid antimic¥obial stewardship. This focus. continijed forward from 2022, complements the established
objective of 'tstablishing and developing data platforms to enable better patient care in LMICS. This will
benefit the public by providing frontline healthcare teams with information to evaluate existing care,
iilentify areas where quality of care is not as expected and identify priorities for caye improvement.
Our medivm term goals for 2023- 2025 w411 be to further the establishment and development of a
community of practice to improve the quality of acute care LMICs through south to 50Uth collaboration.
To achieve this goal, NICSTwill complete its Collaboratlon with the James Lind Alliance to publish researcl)
priorities for the critical care community in LMICS. NICST will continue to partner with NICS-MORU and
with Mahldol Oxford Tropical Research UnFt. IMORUI aTI(I Wellcome. In addilion, we seek to strengthen
Internat￿0￿31 researcli partnerships wilh the University of Edinburgh, University of Jaffna, Monash
University. Australia and AIGHD, Netherlands. NICST'S role in thi5 collaboration will be to train healthcare
workers in the ski115 to undertake research includirTg clinical tria15 and servtce improvement for patients
intemationally-

## **Offsetting** 

Grants and donations are only included in the SoFA when the general income recognition criteria are met (5.10 to 5.12 FRS102 SORP). 

**Grants and donations** 

In the case of performance related grants, income must only be recognised to the extent that the charity has provided the specified goods or services as entitlement to the grant only occurs when the performance related conditions are met (5.16 FRS 102 SORP). Legacies are included in the SOFA when receipt is probable, that is, when there has been grant of probate, the executors have established that there are sufficient assets in the estate and any conditions attached to the legacy are either within the control of the charity or have been met. 

## **Legacies** 

**Government grants** 

The charity has received government grants in the reporting period 

Gift Aid receivable is included in income when there is a valid declaration from the donor.  Any Gift Aid amount recovered on a donation is considered to be part of that gift and is treated as an addition to the same fund as the initial donation unless the donor or the terms of the appeal have specified otherwise. 

**Tax reclaims on donations and gifts** 

**Contractual income and** This is only included in the SoFA once the charity has provided the related goods or **performance related** services or met the performance related conditions. **grants** 

Donated goods are measured at fair value (the amount for which the asset could be exchanged) unless impractical to do so. 

**Donated goods** 

The cost of any stock of goods donated for distribution to beneficiaries is deemed to be the fair value of those gifts at the time of their receipt and they are recognised on receipt. In the reporting period in which the stocks are distributed, they are recognised as an expense at the carrying amount of the stocks at distribution. 

Donated goods for resale are measured at fair value on initial recognition, which is the expected proceeds from sale less the expected costs of sale, and recognised in 'Income from other trading activities' with the corresponding stock recognised in the balance sheet.  On its sale the value of stock is charged against 'Income from other trading activities' and the proceeds from  sale are also recognised as 'Income from other trading activities'. 

Goods donated for on-going use by the charity are recognised as tangible fixed assets and included in the SoFA as incoming resources when receivable. 

Gifts in kind for use by the charity are included in the SoFA as income from donations when receivable. 

**Donated services and** Donated services and facilities are included in the SOFA when received at the value of **facilities** the gift to the charity provided the value of the gift can be measured reliably. Donated services and facilities that are consumed immediately are recognised as income with an equivalent amount recognised as an expense under the appropriate heading in the SOFA. 

**Support costs** 

The charity has incurred expenditure on support costs. 

The value of any voluntary help received is not included in the accounts but is described **Volunteer help** in the trustees’ annual report. 

**Income from interest,** This is included in the accounts when receipt is probable and the amount receivable can **royalties and dividends** be measured reliably. 

**Income from membership** Membership subscriptions received in the nature of a gift are recognised in Donations **subscriptions** and Legacies. Membership subscriptions which gives a member the right to buy services or other benefits are recognised as income earned from the provision of goods and services as income from charitable activities.

The amortisation rates and methods used are disclosed in note 9.5<br>They are valued at cost.<br>The charity has heritage assets, that is, non-monetary assets with historic, artistic,<br>scientific, technological, geophysical or environmental qualities that are held  and<br>maintained principally for their contribution to knowledge and culture.  The depreciation<br>rates and methods used as disclosed in note 9.6.1.4.<br>This includes any realised or unrealised gains or losses on the sale of investments and<br>any gain or loss resulting from revaluing investments to market value at the end of the<br>year.<br>**2.3 EXPENDITURE AND LIABILITIES**<br>**Liability recognition**<br>Liabilities are recognised where it is more likely than not that there is a legal or<br>constructive obligation committing the charity to pay out resources and the amount of the<br>obligation can be measured with reasonable certainty.<br>**Investment gains and**<br>**losses**<br>The charity accounts for basic financial instruments on initial recognition as per<br>paragraph 11.7 FRS102 SORP.  Subsequent measurement is as per paragraphs 11.17<br>to 11.19, FRS102 SORP.<br>**Tangible fixed assets for**<br>**use by charity**<br>The depreciation rates and methods used are disclosed in note 9.2.<br>**Governance  and support**<br>**costs**<br>Support costs have been allocated between governance costs and other support.<br>Governance costs comprise all costs involving public accountability of the charity and its<br>compliance with regulation and good practice.<br>Support costs include central functions and have been allocated to activity cost<br>categories on a basis consistent with the use of resources, eg allocating property costs<br>by floor areas, or per capita, staff costs by the time spent and other costs by their usage.<br>They are valued at cost.<br>**Investments**<br>Fixed asset investments in quoted shares, traded bonds and similar investments are<br>valued at initially at cost  and subsequently at fair value (their market value) at the year<br>end. We report in respect of my examinatton of the Trust's accounts carried out
