We do this by providing funding<br>towards staffing for direct interventions to<br>children and adults in Government homes,<br>and through advising and supporting staff.<br>The staff team consists of a nurse who<br>leads the project, a project co-ordinator,<br>social worker, physiotherapist and<br>psychologist.<br>Throughout the year we have directly<br>provided medical and therapeutic supplies<br>and have supported a range of educational<br>activities.<br>This year we have also continued<br>supporting individuals in the community<br>who are at risk of institutionalisation, to<br>keep them living with their families.<br>This year we have been successful in<br>opening the community centre.|

## **Achievements and Performance** 

> SORP reference Throughout the year, the multidisciplinary team have offered a wide range of direct therapeutic input and support, assisting children and adults with behaviours of concern, physical health issues and mental health issues in addition to their disability. The team have also offered support and advice to care staff within the Government institutions and families in the community. 



|Summary of the main<br>achievements of the charity,<br>identifying the difference the<br>charity’s work has made to<br>the circumstances of its<br>beneficiaries and any wider<br>benefits to society as a<br>whole.|Para 1.20|The provision of resources including<br>nutritional supplements, specialist chairs<br>and bathing equipment and a wheelchair,<br>have improved health and mobility.<br>Additional resources have included<br>beanbags, sensory toys and a sensory<br>station, to engage all beneficiaries to<br>engage in meaningful activities.<br>A number of community visits have been<br>facilitated this year, including celebrating<br>Ceizini day, cinema, museum and a<br>shopping trip. For the majority of<br>beneficiaries, it was the first time they had<br>enjoyed these experiences. These<br>activities not only teach day to day living<br>skills but also reduce stigma, as<br>beneficiaries are able to engage with<br>members of the general public, who may<br>not come into contact with disabled people<br>often. To further improve day to day living<br>skill, we have continued to offer the Life<br>Skills project in the adult institution.<br>The April trip was focused on feeding and<br>was undertaken by a Speech and<br>Language therapist.<br>Committee members visited the projects in<br>November.|
