What
Our charity provides sUPPOrted accommodation and floatlng supporr. We Wofk closely with
private landlords, housing departrnents, re8lStered social landlords and the local authority to
secure temporary and permaneThr accomrnodation. Ability to Change 15 a Supported Housin8
S•ryi¢e, whom provtdes 4 ￿r$0￿￿¢•￿tred approach support service & nerwork io Service Users
who have become homeless. Our work Is based on bndividually tailored support plans for every
Service user underpinned by a Irarnewo¥k of equal opportunities. anti4i5criminatior •nd muiu•I
respect. We offer the same 5UPPQrt package to boih those who have offended Jnd those who
are at risk 91 offending. This Is In the lorm ol accommodatlon, ETE (Education, TrainlnR &
Employment) and course payments. We also provide any resource5 needed when accessing
these servlces such as books, stationery. and appropr¢ate clothing for Inlerwews or for particular
job roles, Such as the construction industry where s￿claI15ed clothin8 is required.
Our Support Workers and admin staff are all 8iven in-house training for the roles that they are
undertakin& and we also use a HR servi¢t that provides an E-learnin8 Hub. as well 3$ an
Employee Asslstance Programme lor wellbeing and counsellin& that all staff have access to.
How
In order to establish ATC'S service delivery, all staff especially Support Worker5 are required to
work wlihln a specific skill set. Thi5 specific skill set will enable & empower all Service User5 the
Ability To Change their live5, maintain a positive future and move on to Independent living with
eonlidence.
We provide supported aceornmodation and floatin8 support. We understand the importance ol
stable ac¢omrnodation In helpin8 ex-offender5 and thè homelèss to move or Irom crime. We
have continued to developed stron£ working partnerships Wlth Multiple a8èntits in both publlc
and private sector organi5ation5. Collaboratin8 and developin8 new partners and stture
referrals. We Te¢oBnise Ihè benefits of a collaboratNe. multi-ogenry approach.

Delivtry of •imlEvaluaiion
We are continually evaluating our 5efvlce delivery and aim to ensure we achieve our objective5
and mainta1n our Service to our highest standards. We have a monthly tracker that we use to
detail and track the progress being made by each service user. This includes inforrnation on
training, employment and other needs that have been identified lov the client and is
continuously documented. We transfer this information to a yearly tracker whith then provides
an over311 picture of what's been happening durin8 the year. We also Continue to use a record
system Qunotes that holds Iht personal Information of each client and notes are added
whenever any type of Contact is made or an event has happened. The trncker and record system
are used for the duration of the cllent's time with ouf service.
A¢hlevements and perform¥n¢e
We support on average, 50 people each year, and these include service user5 who have been
with us for rnore than a year, as well as new clients who come in when a space become5
avallable. We work wlth cllents for at least two year5 on average as part of the move-on procem.
We are pleased to soy with yet another consecutive year Ouf sèrvlce users are progressin8
posltively and seekSnglobs and livln8 Independentlv.
