**Objects:** THE RELIEF OF SICKNESS AND SUFFERING AMONGST PEOPLE LIVING IN NORTH CEREDIGION AND THE IMMEDIATE SURROUNDING AREA WHO ARE LIVING WITH ADVANCED, INCURABLE, AND PROGRESSIVE ILLNESS, AND ALSO THEIR CARERS, BY PROVIDING THEM WITH PRACTICAL, SOCIAL AND EMOTIONAL SUPPORT, BY PROVIDING COMMUNITY-BASED SUPPORTIVE END OF LIFE CARE AND BY RAISING AWARENESS REGARDING ALL ASPECTS OF END OF LIFE CARE IN THE COMMUNITY OF NORTH CEREDIGION AND THE IMMEDIATE SURROUNDING AREA.

**Activities:** The relief of sickness and suffering amongst people living in the North Ceredigion area who are living with advanced incurable and progressive illness and also their carers, by providing them with practical, social and emotional support, by providing community-based supportive end of life care and by raising awareness regarding all aspects of end of life care in the North Ceredigion area.
