**Objects:** THE RELIEF OF NEED FOR PEOPLE IN NORFOLK WHO ARE IN NECESSITOUS ECONOMIC AND/OR SOCIAL CIRCUMSTANCES, IN PARTICULAR THOSE WHO ARE: OLD, FRAIL, EXPERIENCING POOR HEALTH, HAVE PROBLEMS WITH SUBSTANCE ABUSE OR OTHERWISE STRUGGLE TO LIVE INDEPENDENTLY, IN PARTICULAR BY PROVIDING HOLISTIC SUPPORT AND CARE TO AID INDEPENDENT LIVING AND EMPOWER, PROMOTE AND ASSIST THEIR RECOVERY.

**Activities:** Provision of person centered holistic support/care in Norfolk, who are experiencing distress or having difficulties living independently,including people experiencing mental health problems; learning difficulties;life changing illness; older persons, including support to individuals who have been diagnosed with life changing illnesses such as cancer, to promote wellbeing and assist recovery.
