OBJECTIVES AND ACTMTIES
Objectives and aims
Publlc Bèneflt
The trustees have referred lo the guidance contained in the Charity Commission's general guidance on
public benefit when reviewng the chartys aims and objecb.ves and in planning future aclivrties.
History
The Foundation was first estsblished in the USA in 2010 by th￿e siblings who lost their mother. aged 53. to
stsge IV HPV-related anal Cancer. Frustrated by the limited thera￿Irtlc and Community support available to
her, they created the organisation to fill the void caused by scant public, scientific and medical focus on this
disease and its cause, the human papillomavirus IHPV). HPV causes anal can￿r and at least five other
cancers, amounting lo 5°k of cancers worldwide. These cancers are preventable. In 2012, the organization
expanded lo the UK with 2013 as ils Iirsl full 0￿rational year.
Mission
The HPV and Anal Cancer Foundation emw)wer5 anal can￿r patients and accelerates prevents.on and
research methods that eliminate anal ¢ancer and the wrus that causes the majority of ¢as&s, HPV.
Objectives
Highlight the relevance of human papillomawrus {HPVI in (x)ntracbng cancer
Advocate for prevention methods that will end anal cancer and HPV-relaled cancers, including
gender-neutral vaccination.,
- Acc8lératé a cure for the S¢/0 of all cancers that are c3usÈd by HPV. wth a focus on anal cancer,. and
Provide SLJPPOrt lor survivors, caregivers, scientists and prowders connected to anal cancer.
The Need
A fundamental objective of the Foundation's work is lo redu￿ Ihe inudence of anal cancer and other
HPV-relaled cancers. HPV is a very common skin virus that neady every sexually active person will gel at
some point in their lives. HPV affects both men and women and is responsible for 5 /. of the world's cancer
burden. HPV causes cancers of the anus. Ce￿iX, head and rwk. penis. vagina. and vulva.
Protection against HPV through universal HPV vaccination is a critical public health goal. With HPV identified
as a cause of six drfferent types of cancer, and 5% of cancer cases worldwide, it is imperative that we
protect the entire ropulation against rt. The most effective Meth￿ to redu￿ cancers related to HPV is to
prevent initial infection of the Virus through immunisation. The FoundatK)r) achieves this important aim
through its efforts to advanGe equal a￿sS to HPV protection for both boys and girfs through gender-neutral
vaccination against the virus. At the inlroduGtion of the vaccine in most countries. only girfs were able to
receive this lrfe-saving intervention while bo￿5 were generally ex¢luded. The Foundation created and led the
coalitions in the US and UK to ensure all children have equal access to protection against these cancers.
The US achieved the milestone of recommending HPV protectson lo all children in 2011. Following the
Foundats'on's five-year advocacy campavjn in the UK. Sch￿￿baSed HPV vaccination for boys began in
September 2019. Our focus is on securing the same protection for boys as well as girts across Europe, and
beyond.
While we work to prevent ftjture cases. it is also fundamental that s¢i&nttfi¢ and mèdi¢al disparitiès are
addressed for the population that wll continue to be diagnosed with HPV4elated cancers like anal cancer.
Treatmenl options have barely progressed Sin￿ the 1970s and minimal research has been undertaken in
new therapies. The medical and saentific communities need better research tools and investment in novel
ideas to develop a cure.

The stigma surrounding anal cancer remains one of the central challenges to improving outcomes for anal
cancer patients. Unfortunately, many medical centres prowde only limited informats'on specific to the
experience of anal cancer. Many patients feel isolated and lack the support and educational resources that
significantly improve quality of life during and after treatment.
Strategies to Achieve Our Goals

10 out of 27 EU countries have gender-neutral nalional HPV va￿nation programmes, and eight others have
made a commitment to inlr¢yJuce HPV vaccination for boys. Uptake of the vaccine in many countries is well
below 900/0 second dose Completion rates ranging from 14% in Luxembourg to 24Yo in France to 31 % in
Germany. No EU country has a vacanabon rate of 90 /0 or more for the second dose. In addition, uptake
rates in boys by comparison to gids are also lower, in Italy the varrination rate of boys is 40°k of that of girfs.
It is clear however that the COVID19 pandemic has disrupted vaccination programmes and led to a
significant fall in uptake rates.

OBJECTIVES AND ACTMTIES
A HPV VacGine TraGker would help monitor progress ti)ward5 Europe's Beating CanGer Plan goals, indicate
slow progress and countries where additional suprK)rt may be required, in addition to helping encourage
Member States to adopt best practice. It will be a key indi(2tor to the world how the EU is contributing

Seif•sampling and HPV Screening in Europe
To eliminate ¢ervi¢al ¢an¢er, there must be a large rise in the use of HPV testing for ¢trrvi¢al ¢an¢er
screening.
Providing women and people a ceTViX the chance to self-sample foT HPV, often known as self-sampling
is an imFK>rtant component of any national screening program. SeW-sampling is especially useful for those
who have Irwble 8ccessing normal s¢reening facilrfies.
In a position paper published by the Network, the key rK>ints of re¢(xnmend3b"on were..
Self-sampling should form a central component of national ￿T￿Cal can￿1 screening programmes.
Self-sampling should be utilised to retum screening rates to pre-pandemic levels.
HPV seK-sampling should be endorsed by the European Commission's guidelines on cervical cancer.
There should also be guidance on eff￿tive delwery.
Research should continue into the best meth(Kls of delivering effective self-sampling with high levels of
uptake as part of cervical cancer $C￿enIng programmes.
Numerous studies have shown that self-sampling may be as accurate as traditional testing and is well
received by consumers. In one case study, it is being provided as an altemalive to a clinician assessment in
the Netherlands to posibve response from parb"ciparrts.

ANCHOR Study
A ground-breaking randomised controlled nab.onal tn.al of 4.5￿+ people has shown that anal cancer can be
preventèd by routine screening and ￿Moval of pietancerous cells. The ANCHOR Study hoped to improve
patient outcomes for people with anal precancer and rancer by finding the best way to stop anal precancer
from progressing to cancer. The Anal Cancer Foundation buiA and led the coalition of organizations that
supported and pushed for the study.
ANCHOR stsnds for Anal Cancer HSIL Outcomes Research and the study investigated treating anal cancer

C3nGer Foundation Soienttfic Advisory Board Member Dr. Joel Palefsky, ANCHOR aimed to reGruit 5,000
people living wth HIV and was randomized into gr(KJps." treatrnent of anal precancer and a control
group. The incidence of anal cancer is highest in people living wlh HIV.
What was ANCHOR researching.
p￿ventIon and e8dy dete¢tion a￿ essenti81 to ending anal ¢an¢er. Yet. desrlle its growing in¢iden¢e and

The hope was that the ANCHOR study would illustrate that treating anal precancer will reduce the risk of
anal cancer. The Anchor study had two main objectives.. to determine the effectiveness of screening for and
Ireats'ng anal precancer to reduce the incidenoe of anal cancer in HIV-posib"ve men and women. and lo

Ninety percent of anal cancer is caused by the human papillomavirus (HPVI, which can cause changes to
the skin around 8nd inside the anus. These changes are called "high*rade squamous intr8epilheli81
lesions" or HSIL. Not all anal HSIL wll develop into cancer. Most HSIL wll go away on their own, bul some
bècomè Cancer over time and can eventually sprèad to other parts of thè body.
What were the outcomes?
For now, we know that NIH halted the study in fall 2021 due to the therapys success in reducing the
incidence of anal Can￿r. Researchers wll compile the data and submit for publication after peer rewew. 11 is
expected that the dats will provide guidance on rewmmendations to make anal Gancer prevention programs
the stsndard of care for people al high risk of anal cancer.
What does this mean for the anal cancer communi
Anal cancer is currently rapidty increasing at 3% a sear. More peO￿e are being diagnosed Tmth late stage
anal cancer than ever before.
We need standard methods to find it eady. The study proved that treating anal preCan￿r in people limng
with HIV, the group at most high-risk for developing anal (ancer, reduces their cAn￿r risk. The next step is
ensuring this becomes the standard of care so that we can end anal cancer by finding r( and treating r(
