**Objects:** THE PROMOTION OF SOCIAL INCLUSION AMONG ELDERLY PEOPLE AND PEOPLE WITH DISABILITIES LIVING WITHIN A 10 MILE RADIUS OF THE CITY OF WELLS WHO ARE EXCLUDED FROM SOCIETY BECAUSE THEY ARE HOUSE-BOUND, OR WHO ARE UNABLE TO DRIVE OR EASILY ACCESS PUBLIC TRANSPORT, BY THE PROVISION OF:1) A TRANSPORT SERVICE TO AND FROM RECREATIONAL AND DAY CARE FACILITIES PROVIDED BY THE CHARITY; 2) OUTINGS AND SOCIAL TRIPS INCLUDING TRANSPORT TO AND FROM HOME.

**Activities:** We provide day opportunities for older people including those living with memory loss and other health issues.  We are open 5 days a week and 1 Saturday a month with up to 20 people a day attending.  Our main aim is to help prevent social isolation and give family carers a break.  We also provide an outreach service  to promote physical and mental wellbeing and companionship.
