## **SECTION A: GOVERNANCE AND OBJECTIVES** 

The Trustees present their annual report for the year ended 31 January 2024. The financial statements signed off by an independent examiner are attached at the end of this report. 

## **1. STRUCTURE, GOVERNANCE & MANAGEMENT** 

## _**1.1 Structure**_ 

Doctors Worldwide is a company limited by guarantee, its governing document being its Memorandum and Articles of Association. The directors of the company are its trustees for the purposes of charity law and under its Memorandum and Articles of Association are collectively referred to throughout this report as The Board of Trustees. The Charity's principal objectives are to relieve need among people who are victims of poverty, famine, disease, natural and man-made disasters, wars and civil wars, irrespective of race, religion, ideology or politics by the provision of comprehensive medical relief worldwide. 

## _**1.2 Governance**_ 

Doctors Worldwide has a Board of Trustees who meet regularly and are responsible for strategic direction and policy. The trustees during the year are listed on page 2-3. 

Day-to-day responsibility for the provision of services and implementation of the strategy is delegated to the Chief Executive, Monowara Gani. 

## _**1.2.1. Recruitment of Trustees to the Board of Trustees**_ 

The Board of Trustees tries to ensure that the needs of this group are fully reflected in the diversity of skills and experience of the Trustee body. The Charity has sought to recruit suitably qualified individuals to use their skills and expertise to assist the work. 

## _**1.2.2 Induction and training**_ 

New trustees are invited to undergo an induction process and familiarise themselves with the Charity and its workings. Use is made of various Charity Commission and other good practice publications, in particular "The Essential Trustee" guide and the Governance Code. 

## _**1.3 Management**_ 

Responsibility for delivering strategy and day-to-day management of operations is delegated to a management team led by the Chief Executive. 

Systems of internal controls are designed to provide reasonable, but not absolute, assurance against material misstatement or loss. They include: 

- A business plan and annual budget approved by the trustees; 

- Regular consideration by the trustees of financial results, variance from budgets, non-financial performance indicators and benchmarking reviews; 

- Delegation of authority and segregation of duties;

- Identification and management of risks. 

The trustees also consider business risks and implement risk management strategies. The approach comprises: 

- An annual review of the risks the Charity may face; 

- The establishment of systems and procedures to mitigate those risks identified in the plan; 

- The implementation of procedures designed to minimise any potential impact on the Charity should any of those risks materialize. 

An element in the management of financial risk is the setting of a reserves policy and its regular review by the Board of Trustees. 

## **2. OBJECTIVES & ACTIVITIES** 

## _**2.1 Objectives and Aims**_ 

The Charity's principal objectives as set out in its Memorandum of Association are: 

- a) To relieve need among people who are victims of poverty, famine, disease, natural and manmade disasters, wars and civil wars, irrespective of race, religion, ideology or politics by the provision of comprehensive medical relief worldwide. 

- b) The provision of supplies, food, shelter, education, training, medical personnel, procedures and other charitable assistance to:- 

   - preserve and promote good health and well-being; 

   - overcome disease, injury, illness and malnutrition; 

   - for the benefit of the sick, the poor and peoples and communities affected by conflict, instability, discrimination, disadvantage or disaster throughout the world. 

The main objectives and activities for the year continued to focus on the delivery of healthcare and medical services to the poor and needy. Our strategic objectives are based on: 

- Expanding and improving the quality and reach of existing healthcare service provision in areas and communities we work in. 

- Increasing financial resources through creating long-term strategic partnerships with other charities and growing donor income. 

- Focusing on health system strengthening and developing sustainable solutions. 

- Planning, shaping and delivering services in consultation with service users and local partners. 

- Active participation in the local communities we operate in through engagement with local stakeholders and capacity building. 

- Developing long term and sustainable relationships with partners where appropriate.

Our projects cover the following three areas or themes: 

**1) Health Access -** providing access to basic and essential lifesaving healthcare needs. 

**2) Health Improvement -** strengthening healthcare systems to deliver better healthcare. 

**3) Health Emergencies -** enabling local communities to meet their health needs when responding to a crisis situation. 

## **SECTION B: ACHIEVEMENTS & PERFORMANCE** 

## **1. PUBLIC BENEFIT** 

The trustees have taken into account the statutory duties of public benefit. This report highlights examples of Doctors Worldwide activities that illustrate how our work fulfils public benefit and the benefits it brings to individuals and communities globally, irrespective of their race, religion, or ethnicity. We have developed strategic plans to make certain that we provide maximum public benefit and achieve our strategic objectives, which fall under purposes defined by the Charities Act 2011. In setting the charity’s objectives and planning activities, the trustees have given careful consideration to the Charity Commission’s general guidance on public benefit. 

## **2. KEY ACHIEVEMENTS OF ACTIVITIES IN 2023/24** 

Doctors Worldwide (“DWW”) is committed to the provision of high quality and affordable healthcare provision and medical relief in low-resource settings. Our work covered the DWW three thematic areas and this year, with the addition of two emergency relief response activities, namely the Turkiye/Syria Earthquake 2023 and the war in Gaza 2023/4. In addition, a new project in Malawi is seeking to improve access to Maternal care through our Ambu-bike project. Our ongoing direct implementation activities continue through our projects in Malawi, Pakistan, and Rwanda. 

Highlights of 2023/24 include: 

- **116.781 patients have benefited** through our life-saving healthcare activities this year. 

- **Rwanda Palliative Care & Medical Debt Relief -** 1253 people benefited from our Palliative Care project and Medical Debt Relief project this year with 1027 home-based care visits conducted. 90 patients received a holistic package of palliative care including food and shelter and a further 38 children received additional support including education. 66 microfinance projects were launched to support vulnerable families with palliative patients to earn a living. The provision of the DWW community ambulance has facilitated hundreds of hospital trips for the most vulnerable and clinically high-risk patients. 408 patients received medical insurance to help secure any future healthcare needs without impoverishment. 146 patients unable to afford medical care had their medical debts cleared by DWW, including 120 vulnerable mothers and babies who had their full hospital needs covered including baby milk, nappies, clothes and more.

- **Rwanda Milk / Malnutrition Programme –** this year so the continuation of last years pilot of the milk programme for malnourished babies, children and adults who were also suffering from other health conditions such as TB and HIV. 94 patients received 1litre of milk every day and an addition 129 individuals received a monthly food pack for their families, as often chronic hunger is not just limited to the individual clinically diagnosed with malnutrition, but also their families who may not be receiving any food support, and only eating a few mouthfuls of food or less a day. 

- **Pakistan – DWW Hospital KPK –** This year saw the beginning of the last 18 months of work developing a strategy for the refurbishment and reorganisation of services and management of the health facility. A new clinical hospital director was appointed and a new partnership developed with the INGO Alight. Further discussions have also taken place with other like-minded NGOs and health organisations in Pakistan to form strategic partnerships to enhance and promote primary care work in the country. Our KPK hospital is currently being run as polyclinic type facility with walk in patients arriving with a mix of acute and chronic disease illnesses. The internal structure of the facility has been improved and remedial work undertaken to the surrounding land and boundary walls. 2024 will see increased chronic disease prevalence work, more health promotion and outreach work, the reintroduction of more comprehensive maternal care work as well as improvements to the laboratory and diagnostic equipment. Despite the building work 3421 patient consultations were completed. 

- **Pakistan - DWW Hospital Welfare Canteen & Outreach –** After last years launch, 2023/4 year saw the continuation of the welfare canteen and outreach activities as part of the DWW Hospital in Pakistan. Welfare/social issues such as poverty contributes to poor health causing chronic hunger, malnutrition, micronutrient deficiencies, and infectious diseases. The welfare canteen continued to act as a beacon of support for many impoverished families, providing access to urgent welfare needs such as food, clean water, warm clothes and shelter. This year 85 widows, 345 orphans and 27 disabled children were supported through the canteen and 65,000 hot meals were provided directly from the hospital canteen for vulnerable people in the community. An additional 60,200 meals were provided through outreach food projects including dry food packs to 100 families in Chitraal. 

- **Emergency Response, Turkiye/Syria earthquake –** In response to the devastating earthquakes starting on the 5[th] February 2023, DWW medical and assessment teams from our Turkish branch drove through snow blizzards to start work within 48 hrs of the disaster. The first 2 months saw immediate needs being provided such as food packs, hygiene kits, shelter and heating. Our medical teams ran mobile health clinics providing emergency care to the victims including surgical operations. A large medical tent was then established through which ongoing care and support could be provided. In all 8771 medical consultations were carried out. In the North of Syria our medical centre in the already established Jinderas refugee camp saw a further large influx of families displaced by the earthquake. This required extra medical staff to be employed to cope with the increase demand, allowing 21,741 medical consultations to be undertaken. In addition, a psycho-social programme was undertaken aimed at the children in the camps. A large tent was established and developed with a soft play area for children, art work therapy sessions and trained psychologists employed to run services to help the large number of children suffering from mental health symptoms. 636 psycho-social support sessions were completed this year. 

- **Emergency Response, Gaza war 2023/4 –** Doctors Worldwide Turkiye has worked in Gaza for over 14 years with an established physical rehabilitation clinic in Gaza and 19 employees. From the beginning of

the latest devastating conflict work has continued despite the security dangers to staff, damage to the rehabilitation centre and constant displacements of civilians and our staff. A hot food prpgramme and monthly dry food pack distribution was initiated to provide vital nutrition to 14,000 people. Our medical staff attended families unable to leave their houses due to age or disability, some of our staff were also seconded to work in the hospitals to provide extra support to the inundated staff. A training programme was being developed to quickly upskill a cohort of people to become community health workers to provide some vital health promotion, health screening and continuity of care given the almost complete disruption to the normal health care system. 

- **Malawi Health System Strengthening Programme -** as part of our core health system strengthening approach, we are supporting 26 clinics including hospitals and mobile clinics through training, service development, clinic rehabilitation, provision of nurses/midwives/medical officers and more. We are also undertaking assessment work on building a new primary care clinic and maternity clinic in an area where access to healthcare is more than a day’s walk. This year 2,877 antenatal and postnatal consultations were conducted with 52,383 patients treated and 544 safe births through our ongoing clinical care activities. In response to a request from the Ministry of Health due to a Cholera outbreak, we conducted training sessions for community leaders on Cholera prevention and good infection control practice. This year also saw the continued maturation of our Ambu-bike project to help improve access of poor pregnant women to medical care particularly when in labour. Service agreements are being developed with the clinics at which the bikes will be stationed and a pilot is due to be begun in 2024.

## **4. STAFF** 

Our staffing capacity continues to strengthen as we deliver more impactful project. We continued to strengthen our field partners by providing project managers/officers whilst we develop our core team to provide low cost and high impact projects. This year saw our CEO take maternity leave with her roles taken up by trustees and our Programme manager. 

## **5. VOLUNTEERS** 

We express our gratitude for the support and expertise we receive from our volunteers, in particular those who gave their time and money travelling and working in our overseas projects or supporting our resource/content development remotely. Supporting medical volunteers has always been a part of Doctors Worldwide vision and commitment, with an increasing role for volunteers to help develop training material for our projects abroad. This year, _**4 medical doctors/consultants engaged with DWW with their time, expertise and input**_ through a variety of activities including, teaching and content development.

Review of the Results
The trustees consider that the results set out on pages 5 to 9 are satisfactory. They are of the
opinion that the balance on the funds is sufficient to meet any future commitments.
Fixed Assets
The movement in fixed assets is set oul in Note 8 to the Accounts.
Trustees Responsibilities
Charity law requires the trustees to prepare accounts for each financial year (or
other accounting period) which give a true and fair view of the state of affairs of the
Company and of its surplus or deficit for that year. In preparing those financial
statements, the trustees are required to..
select suitable accounting policies and apply them consistently.,
m8ke judgements and estimates that are reasonably prudent;
prepare accounts on a going concern basis unless it is inappropriate to presume that
the company will continue in business.
The trustees are responsible for keeping proper accounting records which disclose
with reasonable accuracy at any time the financial position of the company and to
enable them to ensure that the accounts comply with the Charity Act 1993. They
are also responsible for safeguarding the assets of the company and hence for taking
reasonable Steps for the prevention and detection of fraud and other irregularities.
In preparing the above report, the trustees have taken advantage of special
exemptions applicable to small companies.

ACCOUNTING POLICIES
The financial statements have been prepared under the historical cost convention and
in accordance with the Financial Reporting Standard for Smaller Entities (effective
January 2015) the Accounting and Reporting by Charities. Statement ofrecommended practice
applicable lo charities preparing their accounts in accordance with the Financial Reporting
standard for Smaller Entities published on 16th July 2014 (SORP 2015) and the charities Act
2011 and companies Act 2006.
Income
All income is included in the statement of financial activities when the charity is entitled to the
income.
All expenditure has been reflected in the Statement of Financial Activities on accrual b3SIS.
Expenditure includes any VAT which can not be fully recovered and is reported as part of the
Expenditure to which it relates.

Fixed assets are included in the accounts at net book value.
Depreciation is provided to write off the Cost less the estlmated residual value

The organisation 15 exempt from income tax by reason of its charitable status.
Income tax suffered on investment income is reclaimed in full.
Reserves
The organisation aim to maintain sufficient reserves to cover three months running costs.
INCOME AND ENDOWMENTS
This represents donation receiveable for the provision of services to members. The company
is a non-profit making.
