Thi5 Past year has marked signrfitant progress for the mrtochondrial disease communrty. From
scientrfic breakthroughs to the tireless efforts of families. researchers and supporters across the UK, I
feel immense pride in what we have achieved together and renewed hope for the future.
It has beèn a remarkable period for research. We saw the success of mitochondrial donation IVF. a
cause The Lity Foundation championed for many years, alon8Stde the approval of the first ever
treatment for one form of mrtochondrial drsease. We also celebrated the launch of the £7.5 million
LifeArc Cenire for Rare Mr(ochondrial Diseases. as well as new initiatwes including our IMPACT
Patient Advisory Cornmtttee and the Lity Research Zone. boih designed to Slrenyhen the role of
patients in shaping research and improvin8 access to information.
Looking ahe3d, we remain committed to a¢fvaft¢ing research and ensuring that patieftt voices are
centr31 to the future of mttochondri31 medicine. Recruitment to the Lity Precision Medicine
Diagnostic Project will continue across specialist centres in the UK. helping to return dia8n05es to
patient5 while strengthening NH5 genomic services. Plans for the UK Mitothondrial Disease Research
Instituie will also continue to develop alongside our ongoing support for existing research studies.
Support for families remains at the heart of everything we do. Our Adult Support Weekend has now
become a valued fixture alongside our Famity and Young Adult events, and we continue to provide
both ernotional and prattical sUPPOrt through initiatives such as online coun5ellin& virtU31 coffee
mornings and peer-support netwoiks. Over the coming year we will also expand the distribution of
our Lily patient information packs to help ensure farnilie5 receive clear and con515tent information at
the point of diagnosis.
Our patient-led work will continue to Brow. with IMPA￿ helpin8 to shape research. clinic31 tria15 and
policy discu55ions. while the next phase of the Experience of Diagn051S Study will help us advocate for
bmprovements in how mitochondrial diagnoses are delivered and 5UPPOrted.

2025 has been a strong and important year lor The Lity Foundation- one in which we have focused
not only on delivering for the mr(ochondrial disease cornmunity today. but also on building the
foundations for the future.
From the 8oard's perspertive. one of the most signrficani ach*vements of the year has been the
progress made in strengthening the charity's financral posit￿￿ and oversight. In a challenging climate
for many charitie5, Lily has worked hard to improve transparency and financial scrutiny, and make
careful. disciplined decisions about Pr￿TitieS and tnvestment. That approach has helped us end the
year in a stronger positN)n. with income recovering well on the previous year and the charty better
placed for the future.
Alongside this. the organisatioft has continued to play at) influential role across the mitochondrial
disease landscape. While the decision was taken to pause the research call during the year. this was
the right and responsible slep at that point in time. Importantty. it did not dirninish Lily's wider
contribution to research and patient engagement. Our team has continued to strèngthen
relationships with leading clintcians. researchers and specialist ￿ntre$. and has represented LI￿ on
the international research stage. The charrty has also played a key role in the delNery of the LifeArc
pro8ramme- regular￿ cited by LrfeArc as'best in class. when rt comes io patient engagement.
Perhaps most excrtingly, 2025 has also marked the early development of an ambrtious new vision..
the UK Mitochondrial Oisease Research Institute. This has the potential to be a maior step forward
not only for Lity, but for the wider mitochondrial disease community. The strong support already
shown by senior medical leaders is encouraging and reflects the credibilrty and ielationships the
charity has built over many years.
At the Same time, the Board bs clear that realising thi5 V15ion will require significant thought, planning
and support. Delivering an initiative of ihis scale will call for new partnerships, drfferent capabilities.
strong governance and substantial fundraising. tt represents a bold next ¢hapier for Lily, and one that
we must approach with both ambition and care.

The charity relies on voluntary help to host fvndrai%ing events, patient residential weekends and
information days. Around 30 volunteers regularly assist the charity at events. helping reduce cost5.
The Lity Foundation supports anyone in ihe UK who is affected by mitochondrial disease. We exist to

Research
Since 2008 The Lily Foundation has invested heavi￿ rn dtagnostic research, tncluding the Lity Exome
Sequencing Proiea. helping to diagnose more patients and identify new disease-cau5ing genes. Our
strategy continues io involve close collaboration with NHS diagnostic services. clinicians and research
institutes. enabling the sharing of data and resources to streamline dia8nostic pathways. Geneti¢

Precislon Medi<ine Dia8nostl¢ Project
In 2023124 we launched the four-year Lify PrecIs￿n Medicine Diagnostic Project. During 2024125 the
project moved inio recrurtment and Èarty delNery, with the first diagnoses now being returned to
patients. RecTur(ment sites include UCLH. Oxford, Northern Care Alliance (Manchester), Moorfields,
Birmingham. GOSH. Gu￿S and St Thomas,. and Cambridge. with further srtes expected to open.
The project is expeaed to increase mitochondrial disease diagnosis rates by around 20%. By August
2025, 112 patients had been recruited. resulting in nine confirmed genetic dtagn05es and two novel
genes cuirently under review. To support delNery. The Lily Foundaiion funded a Senior Translational
Clinical Screntist and additional project infrastructure. including bioinformatics and project
Management support.
Trealrnenls Research
During 2024125 we paused our annual Treatments Research Call while reviewing our research
strategy, but we continued to manage a stron8 Portfolio of funded studies. Three projerts from the
2023124 call are now underway, exploring immune pathways in mitochondrial disease, developing an
mRNA-based gene therapy for MNGIE, and testing wearable technology to support balance and
exercise at home. A Siudy validating mrtochondrial disease rating scales is also underway. with
completion expected in 2026.

This includes a programme of community webinars on topics such as emotional wellbein&
physiotherapy and living well wilh mitochondrial disease.

Patient and Family Support
As diagnosis and awareness of mitochondrial disease increases. demand for 5UPPOrt seNices
continues to grow. The li￿ Foundation remains the UK'5 leading tharty provKlin8 dedicated support
to people affected by mitochondrial disease. Our Patient and Farni￿ Support programme provides a
coordinated package of prartical, ernotional and educattonal 5UPPOrt, designed to complement
clinical care and address the wider impact of mitochondrial disease on daity life. The programme

Helpline and peer support
We are often the first point of contact for families following dia8nos5. Through our telephone
helpline, email and social media channels, we offer practical guidance. emotional reassurance and
signposting to other specialist services where appropriate.
Our closed Facebook peer support groups continue to grow, providing patients and families with a
Safe and accessible space to connect with others facing similar challenges. In addrtion. Objr
community-based 5UPPOrt activities provide opportunrties for families io find local connection5 in-
person with others who understand the realities of INing with mrtochondrial disease.
In 2025 we also launched our Zoom Room webinars. a series of online sessions hosted by
prtsfessiona15 des￿rted to help farnil*s navigate the day-to-day realriies of INing with mrtochondrial
disease. 83 8uests tuned in to the first two 5e55ions. with a fvrther 322 views via YouTube where the
recordings are available for those who couldn't join live.

We ¢ontTrnue to work with Rareminds to deliver Spec￿1151 counselling for the mitochondrial
community. Between January and Detember 202S the service deliVe￿d 230 individual ¢ounselling
sessions and 92 couples. sessions, providing vital emotional supFK)rt for patients, parents, partners

Building on Lily-led researth into the diagnostic journey. we developed a Lity-brandod patient
information pack for newty diagnosed families. Created with clinical teams in Newcastle and UCL, the
pack provides clear information and signposting at diagnosis. Distribution to metabolic units across
the UK and Northern Ireland will begin in 2025126.
Resldential Support Weekends
Our residential weekends remain a comerstone of community support and continue to grow in

These events bring together emotional support, edu¢ation. peer connection, expert-led medical
sessions and much-needed respite in a safe. inclusive environment. Families often describe these
events as a 'lifeline'_ helping to reduce i501at40n, increase resilience and improve emotional
wellbeing.
Following attendance at our Family Support Weekend in 2025, 92% of respondents felt their famivs
wellbeing had been positively impacted. 88% felt more connected to the mito community. and 80%
felt more confident in managing the challenges of mbtochondrial disease.
Livs Wish Fund
Lily's Wish Fund provides practical financ431 support to families facing the often-s￿nIflCant additional
pressures that come wrth mitochondrial disease. A drdgnosis can affect every part of life, brin8lng
emotional strain. finaneial uncertainty ènd the need for costly adaptatiofts or specialist equipment.
Through the Wish Fund, Lity helps families access one-off item5 and support that ¢an make day-to-
day life easier. safer and more enjoyable- from specialist equipment and home adaptations to

Advocacy- Empowering the Patient
Ensuring that patient voices shape research. care and policy remains central to our work.
Patient and Public In¥ol¥ement IIMPAcn
IMPACT, our Mitochondrial paI￿nt Advisory Comrnittee, has grown into a network of around 70
Palients. parents and caregivers. Members contribute to researth desEn. patient resources and
policy discussions.
During the year IMPACT supported projects including the development of a patient research toolkit
and a qualitative study exploring reproduciive decision-making among women with mitochondrial
DNA mutations. The group also contributed to a Medical Research Council grant proposal examining
menial health and mrtochondrial disease using MRI and wearabk technology.
IMPACT played a key role in the LrfeArc Centre's inaugural meetin& delivering a PPI workshop that
signrficantly increased professional confidence in working with the mitochondrial community.
Fundift8 has t*en alk)cated for a part-time PPI coordinator to support the programme's continued
growth.
Exper*n¢e ol Diagnosis Study
During the year we delivered our h.rst Lity-led ￿Search study exploring experien¢es of receiving a
mitochondrial diagr￿S￿ in the UK. The suryey received 176 responses and highlighted signifTrnt
variation in diagnostic experiences and limited psychological support at diagnosi5.
FindinEs were presented at the UK'S largesi mitochondrial conference and have prompted strong
calls for servKe improvefflents. Phase 2 interviews have received ethical approval and will inform
advocacy for improved diagnostic pathways and specialist genetic counselling support.

We continue to advocate for equitable access to emerging treatments. Following the birth of eight
healthy bèbies using mitochondrial donation techniques in 2025, The Lily Foundation contritsjted to
national and international media discussions supporting wider implementation.
We welcomed NICE (National Institute for Health and Care Excellence) approval of Idebenone for
LHON ILeber Hereditary Optic Neuropathy) in England and supported patient expert invofvement in
the appraisal process.

potentially the first disease-modifying mitochondrial treatment available on the NHS.

Raising awarenèss of mitochondri31 disease remains a vital part of The Lily Foundation's work.
Through media engagement. conferentes and digttal communications. we aim not ofi￿ to increase
visibility of a rare and often misunderstood condition. but also to ensure that patients and farnilies

understood.

In January 2025, we launched the Research Zone to make complex research developments more
accessible to pat￿ntS and families. through interviews. articles and short videos. The content
generated over 46.0￿ views across platforms between January and July 2025. wrth particular
strong interest in short-form video updates from researchers. This response highlighied the
communivs appetite for clear. accessible research communicat¥on and reinforc￿ the importance of
helping families Stay infomied about progress in the field.
Fundraising
Fundraising remains vital to The li￿ Foundaiion's ability to sUp￿rt patients and families. raise
awareness of mitochondrial disease and invest in research. We are incredibly grateful to the donors,
funders. partners and supporters whose generosity and cornmr¢meni make ths work possible.
The chariV5 performance durin8 the year was strong and alEned with it5 0bjertive5. 5LbPPQrted by
increased income from sporting events and higher grant funding compared with the prior year.
During 2024125, fundrai5in8 activity focused on comrnunity and individual 8Ning. challenge and
spec131 events. trusts and foundations. and corporate partnerships. All fundraising was carried out
directly by The Lity Foundation team and monitored throu8h project plannin& KPIS and safeguarding
procedures. All fundraising staff have regular face to face meeting Wlth senior leadership with KPIS
and objectives in place. We monitor all commvnity fvndraising through regular contact and our
customer relationship management ICRMI system. No fundraising complaints were received during
the year. The charity is not currently registered with the Fundraisin8 Regulator. but has plans to in
the future.
Community and Individual Giving
PublK fundraising continues to be the foundation of Lilvs income. supported by a loyal and hghly
en8a8ed communtty of families. friends and supporters with a strong personal cOnneCt￿n to the
cause. Supporter-led fundraising and 5POrts challenge events remain particular strength5 durin8 the
vear. Improved stewardship. refreshed supporter materials and partnership5 With third-party event
providers expanded participation opportunities. New initiative5 such as ihe Lrly Skydive and an online
auction also helped divers￿ income. while individuals. schools and local groups continued

Although the Lily Ball was noi held during the year. other evenis performed well. Our Comedy Nights
and GolF Days continued to generate income and en8a8ement. while the launch of a Manchester
Comedy Night helped broaden our geograph* reach and introduce Lity to new audience5.
Corporate Partnerships
Corporate fvndraising continued to develop during the year, wtih progress in tr*)th corporate
sponsorship and strengthened relat￿nships. We a150 took a more proactive approach to identifying
and enÈaging with potential Charity of the Year and longer-term partners. successfully securing new
partnerships wrth Eco Green Group and Infinty Management.
Grants, Trusts and Foundatiuns
We continued to pursue grant funding to support both research and wrfler charitable activitie5.
While this remains a developing area of fundraisin& it ￿ an increasingly important part of LI￿'S
lonBer-tem income mix and a key opportunity to diversify funding and build resilience.

from sporting events and htgher grant funding compared with the prKJr year. Expenditure reduced

The principal sources of funding are through individual donations and Ofganised fundraising events.
Alongside donation5 from corporate partners and grant fundin& these income streams ensure we
have sufficient resources to fund research, raise awareness. and support Pat￿nts and families
affected by mitochondrial dbsease.
In the year, in addrtion to recognising £91.921 of expenditure relating to new research initiative5, the
charrty also paid £267,616 towards research projects that had been approved and committed to in
prior years.

Ihe research programme whilst minimising the likelihood of harm to research volunteers and
animals. Applications made in the correct format are peer reviewed by our international expert
