## Building futures, Changing lives
This year marks the 20th anniversary of the Hashim Welfare Hospital, a milestone unimaginable when our founder Dr Idrees Awan fi rst approached his friends in 1999 to share his idea. He wanted to setup an organisation to help people easily access medical care in Pakistan. That meeting led to the founding of the Hashim Welfare Hospital Appeal (HWHA).
## **Hashim Welfare Hospital**
The primary project to achieve Dr Awan’s vision was to build a small hospital in Pindi Hashim, a rural village between Islamabad and Lahore, where he was born. The fi rst brick was laid on 9 April 1999 and the fi rst patients walked through the doors in June 2003. A partner charity, the Hashim Welfare Association was set up in Pakistan to manage the day to day running of the Hospital, as well as other local initiatives.
## **Facing the challenge**
Accessible medical care is a major challenge in Pakistan, a country where the annual health budget is less than 3% percent of spending and there are only 1.1 physicians per 1000 people, compared to 3 in the UK. There are about 1.5 million people who are blind or visually
impaired due to cataracts. The death of mothers due to complications in labour and childbirth is 154 per 100,000 births. The under-fi ve mortality rate is 58 deaths out of 1,000 and there is an almost 25% probability of dying from preventable and treatable health conditions such as diabetes and heart disease. _(World Health Organisation, 2020/2021)._

## Chairman’s review
It is my pleasure to inform you that the fi rst of June 2023 marks 20 years of clinical services at Hashim Welfare Hospital and twenty-four years since we started the Hashim Welfare Hospital Appeal. The Hospital started with one clinic and fi ve part-time staff , and was focused on cataract day-surgery. We currently run ten fully functioning clinical departments, fi ve non-clinical departments, and employ forty-fi ve members of staff . Our cooperation with Hashim Welfare Association, Pakistan, has continued to be life changing for poor and needy families.
## **Child welfare programme**
We are currently working with the Hashim Welfare Association (HWA) to pilot a child welfare programme at a local government school. The Hashim Welfare Hospital (HWH) will provide a medical offi cer, a dentist, an optometrist, as well as an audiology assistant, to examine pupils. Children identifi ed with medical conditions, as well as malnutrition, will be supported by the Hospital.
## **Clinical update**
9,000 babies. Do date, with the support of our German partners PAKISTAN-Hilfe zur Selbsthilfe e.V. (PHzSH), 306 children received hearing aids. The dental department contiues to do well; since 2011 the team has seen 23,441 patients _(see page 6 for more_ ).
From June 2003 to December 2022, the HWH has treated 970,640 patients (64,734 in 2022), the eye department performed over 14,500 cataract procedures, and since 2011, the maternity team helped deliver more than

which work stopped, once again, during Ramadan. The next stage is to complete the grey structure, which means walls, roof and plastering, and is the stage before installing services such as power and drainage. We have been forced to revise the estimated cost of the project to around 1 million pounds. This is almost double the initial estimate and is due to the knock-on eff ects of the pandemic, extremely high infl ation and continued political instability in Pakistan. The progress of building work depends completely upon the amount of donations we receive _(See page 8)_ .
We are proud to have been able to help our Pakistani brothers and sisters in South Punjab and Baluchistan during the worst fl oods of recent times. Medical and relief teams visited the areas to witness the damage and assess immediate needs. They established base camps in Rajanpur and Usta Mohammad, from where they coordinated relief work. The teams were able help more than 22,000 fl ood victims with medical clinics, blankets, food packs, and more.
## **Hospital extension project**

In March 2022 we resumed work on the Hospital extension project, which was pause in 2020 during the Covid pandemic. The foundation was fi nally completed, after

gatherings and sponsored fundraising events. this helps with lowering costs and enables us to target our eff orts more eff ectively
Just before Ramadan 2023, Dr Shahid Sharif sponsored a fundraiser in Reading and received pledges of £42,000. This is the largest amount we have received since we began these events. We are extremely grateful to Dr Sharif and all our donors for their hard work and dedication to our vision.

## Departments and clinics
The Hashim Welfare Hospital started work by focusing on cataract day-surgery, with one clinic and fi ve part-time staff . It has now grown to a multi-department hospital with over 45 staff . It is registered with the Punjab Healthcare Commission and was awarded a provisional licence as a thirteen-bed hospital. This will increase to fi fty beds after the extension is completed.
## **Paediatrics**
In 2022 the paediatrics department treated 1,134 children. The Hospital began a pilot child welfare programme for school children and malnourished babies, in line with a World Health Organisation initiative. This will include ophthalmology, ENT, audiology, dental, and paediatric assessments and treatments. The team will also measure malnutrition. 40.2% of children in Pakistan have stunted growth and 17.7% children suff er from wasting due to lack of adequate diet _(UNICEF, DFiD, Ministry of National Health, Pakistan and AGA Khan University, 2018)_ .
Dr Mamoon Aftab, paediatric consultant, heads the department and has started a weekly clinic which he hopes to increase in the future. This department naturally grew as a result of the work of the maternity department. Our German partner charity, PAKISTAN-Hilfe zur Selbsthilfe e.V. (PHzSH), are providing and supervising free hearing aids to children under 10 years of age.
## **Eye (Ophthalmology)**
The eye department has treated 154,659 patients since June 2003, (12,299 in 2022) and performed 14,512 eye surgeries (949 procedures in 2022). This was the fi rst department created when the original hospital opened. Currently it employs two consultant eye surgeons, and runs two weekly eye clinics and theatre sessions. The bulk of clinical work is related to cataract and associated conditions. Cataract surgery is mainly performed with foldable intra-ocular lenses with the option of extra-capsular surgery.
Laser therapy for post-cataract surgery is regularly performed. Our staff have access to diabetic retinopathy screening equipment supervised and supported by Dr Ayaz Asghar from the UK. An optometrist is helping treat refractive eye problems during regular eye clinics. We are planning to provide prescription lenses and glasses in the future.

## **Maternity**
The maternity department has treated 88,144 patients since it was started (7,118 in 2022) and 9,069 babies have been delivered (397 in 2022). The department includes neonatal and gynaecology services. We currently employ one consultant, with a vacancy for a second, and are looking for a female medical offi cer for maternity emergencies. Specialised pre-natal, child welfare, and family planning clinics are being considered for the future.
## **Dental department**
The dental department has treated 23,441 patients since it started (4,584 in 2022). Last year, 210 tooth extractions, and 1,028 other dental procedures where performed. We have refurbished the clinic area and have installed two dental chairs, one donated by PAKISTAN-Hilfe zur Selbsthilfe e.V. (PHzSH) and a second by an Pakistani-American supporter. in October 2022, PHzSH held an oral health clinic for children. The staff include two dentists, an oral hygienist and a dental nurse.
## **ENT and audiology**
The ENT and audiology department has treated 1,195 patients and fi tted 306 hearing aids since 2016 (50 in 2022). Dr Uzair Mushahid has been appointed as consultant ENT surgeon and will head the department. On 1 May 2023 he started a weekly clinic and surgery. PHzSH have conducted yearly ear camps at HWH and have been training our staff . They will be assisting us in further developing the service.
## **Diagnostics**
Since starting, the department has performed 372,226 biochemistry, haematology and serology tests (37,044 in 2022), as well as 60,382 ultrasounds and 5,430 x-rays. This is one of our busiest departments because many doctors from other clinics and hospitals use its services. The department is supervised by Professor Nasir Iqbal, and Prof Tariq Jarral Mirza from Lahore. We are in discussions with Dr Adnan Ali from the UK, to supervise a new infection control programme thoughout the Hospital.

and out-patient wards, physiotherapy, ENT/hearing disability, and eye clinics. It will also include an operating theatre, and space for administration and teaching.
## **Support from our partners**
As always, the key to our support comes from our regular donors. We are expecting further donations from our long-term charity partners from the UK, and from further afi eld through Muslims Americans for Compassion (MAC). Hans Joachim Gerber and his team at Pakistan Hilfe zur Selbsthilfe e.V. (PHzSH) continue to support us and have played a crucial part in the project by assessing the current building and needs, selecting architects, reviewing designs, liaising with local construction companies, and fi nalising architectural plans.
## **How much do we need to raise?**
We have been forced to revise the estimated cost of the project to around 1 million pounds. This is almost double the initial estimate and is due to the knock-on eff ects of the Pandemic, extremely high infl ation and continued political instability in Pakistan. The progress of building work depends completely upon the amount of donations we receive. We have already raised more than 25% of the above amount and hope to increase our fundraising capabilities in coming years.

## Zohar Bibi
Zohar Bibi is 70 and from Gurrah Habib, 40 km from Pindi Hashim.
She decided come to the Hospital with her son as several people from her **“ My cataract** village had recommended the services it off ered. When asked how her visit **surgery was** went, Zohra said it went smoothly and staff and doctors were very helpful and understanding. She appreciated the **successful. May** good service off ered by HWH and was surprised how aff ordable it was. Zohar **God bring success** said, ‘My cataract surgery was successful. May God bring success and prosperity to the Hospital.’ **and prosperity to the Hospital.”**

## Ishrat Fatima
Ishrat is 27 and came to the hospital to have her second child.
Ishrat is 27 and from Gulyana, 7 km from HWH. She came to hospital to have her second child. She had her fi rst baby delivered here too. Ishrat’s husband Saqib is a stone mason and works hard to provide for his family. Ishrat’s sister gave birth to her children at HWH and was happy to recommended it to her sister. Ishrat felt the whole process from arriving at the Hospital and registering at reception, to the procedure and recovery, had gone smoothly. Ishrat had a baby boy via c-section.
She praised the staff and hospital facilities and said her stay at the hospital was excellent.

## Attia Waseem
## Attia gave birth to a beautiful baby boy.
Attia is 20 and from Mandi Bahauddin. Her husband, Waseem, works as a labourer at a brick factory. This is her third baby and she was apprehensive as the family can’t aff ord expensive medical treatment. She gave birth to a beautiful baby boy via c-section. Attia was really happy with the care she received and encourages other to use the services that the Hospital provides.
She was especially relieved that her treatment was aff ordable. She will encourage others to get their treatment and medication from HWH.
Attia particular wanted to say a prayer for the chairman of Hashim Welfare Association for what the Hospital had achieved so far; ‘May Allah bless him with good health!’

## Ahmad
## Deen
Ahmad came to the Hospital for cataract surgery.
Ahmad is 95 years old and from Sariya. He came with his grandson, who is ancillary worker at a school. Many people from the Sariya area have used the services of the Hospital, including the pharmacy and eye clinic. Ahmad came to the clinic for cataract daysurgery and it was his fi rst experience at the Hospital. He praised the staff and doctors for their care and thanked donors and supporters for making the Hospital possible.

## **Income and expenditure**
In 2017, 77.9% of the income needed to run the Hospital was from Pakistan. This year, the overall income was 49%, with figures from the last few years showing a similar trend. This is due to the large donations being sent from the UK for the extension project. We hope to get back on track once the new building is up and running.

## **Structure, Governance and Management Governing Document**
The charity is constituted as a charitable association registered with the Charity Commission under charity number 1082330 and is governed by it’s constitution.
## **Organizational Structure**
The trustees / Executive Members are fully responsible for the management of the assets of the association and give their time free of charge and receive no remuneration or other financial benefits.
The trustees / Executive Members have regular meetings, and all the Executive Members are responsible for all decisions taken in relation to the running of the charity and the other activities provided by the charity.
## **Recruitment and Appointment of Trustees / Executive Members**
It has been followed according to the Constitution.
## **Risk Management**
The trustees / Executive Members have assessed the risks the charity faces and have identified the major risks areas and have drawn contingency plans to deal with those risks. The Executive Members are satisfied that systems are in place, or arrangements are in hand, to manage the risks that have been identified
## **Objectives and Activities**
We have referred to the guidance contained in the Charity Commission's general guidance on public. benefit when reviewing our aims and objectives and in planning our future activities. The objectives of the charity are set out in the charity's Constitution.
## **Financial Review**
## **Reserves Policy**
The trustees / Executive Members have reviewed the reserves of the charity. Their policy is to hold enough funds to meet the operating costs of the charity

## **Trustees' Responsibilities Statement**
The trustees / Executive Members are responsible for preparing the trustees' report and the financial statements in accordance with applicable law and United Kingdom Accounting Standards United Kingdom Generally
## **Accepted Accounting Practice**
The law applicable to charities in England and Wales requires the charity trustees to prepare financial statements for each year which give a true and fair view of the state of affairs of the charity and of the incoming resources and application of resources, of the charity for that period.
In preparing these financial statements, the trustees / Executive Members are required to:
o Select suitable accounting policies and then apply them consistently.
o Make judgments and accounting estimates that are reasonable and prudent.
o Prepare the financial statements on the going concern basis unless it is inappropriate to presume that the charity will continue in business.
o The trustees / Executive Members are responsible for keeping adequate accounting records that are sufficient to show and explain the charity's transactions and disclose with reasonable accuracy at any time the financial position of the charity and enable them to ensure that the financial statements comply with the Charities Act 2011, the applicable Charities (Accounts and Reports) Regulations, and the provisions of the Constitution).
o They are also responsible for safeguarding the assets of the charity and hence for taking reasonable steps for the prevention and detection of fraud and other irregularities.

## **Independent Examiner's Report to the Executive Members of Hashim Welfare Hospital Appeal UK**
I report to the Executive Members on my examination of the financial statements of Hashim Welfare Hospital Appeal UK ('the charity') for the year ended **31st December 2022** .
## **Responsibilities and basis of report**
As the Executive Members you are responsible for the preparation of the financial statements in accordance with the requirements of the Charities Act 2011 (‘the 2011 Act’)
Having satisfied myself that the accounts of the Charity are not required to be audited under Part 16 of the 2006 Act and are eligible for independent examination, I report in respect of my examination of the charity’s accounts as carried out under section 145 of the Charities Act 2011 (‘the 2011 Act’). In carrying out my examination I have followed the Directions given by the Charity Commission under section 145(5)(b) of the 2011 Act.
## **Independent examiner's statement**
I have completed my examination. I confirm that no matters have come to my attention in connection with the
accounting records were not kept in respect of the charity as required by section 386 of the 2006 1 Act; or
- 2 the financial statements do not accord with those records; or
the financial statements do not comply with the accounting requirements of section 396 of the 2006 3 Act other than any requirement that the accounts give a ‘true and fair' view which is not a matter considered as part of an independent examination: or
the financial statements have not been prepared in accordance with the methods and principles of the Statement of Recommended Practice for accounting and reporting by charities applicable to 4 charities preparing their accounts in accordance with the Financial Reporting Standard applicable in the UK and Republic of Ireland (FRS 102).
I have no concerns and have come across no other matters in connection with the examination to which attention should be drawn in this report in order to enable a proper understanding of the accounts to be reached.

## **1. Accounting Policies**
## **1.1. Accounting convention**
The financial statements are prepared under the historical cost convention and in accordance with the Financial Reporting Standard for Smaller Entities (effective June 2002) and follow the recommendations in 'Accounting and Reporting by Charities: Statement of Recommended Practice' issued in October 2000.
**1.2. Incoming resources** Income is recognised in the period in which the charity is entitled to receipt and the amount can be measured with reasonable certainty.
## **1.3. Resources expended**
Resources expended are recognised in the year in which they are incurred. Management and administration costs are those incurred in connection with administration of the charity and compliance with constitutional and statutory requirements.
## **1.4. Tangible fixed assets and depreciation**
Depreciation is provided at rates calculated to write off the cost less residual value of each asset over its expected useful life, as follows:
