The overall objective of the Hashim Welfare Hospital Appeal (HWHA) is to provide high quality, accessible healthcare, with a focus on the poor and needy in rural Pakistan. We want to achieve this through our core project; the Hashim Welfare Hospital. The Hospital is a combined project of two charities, and was founded in the UK by Dr Idrees Awan. HWHA is based in the UK and the Hashim Welfare Association is on the ground in Pakistan. ## **Facing the challenge** ## **Helping change lives** There are about one and a half million people in Pakistan who are blind or visually impaired because of cataracts. The death of mothers due to complications in labour and childbirth is 178 per 100,000 births. The under-fi ve mortality rate is 81 deaths out of a thousand, and there is an almost 25% probability of dying from preventable and treatable health conditions such as diabetes and heart disease. The development of a country is measured by the quality of its healthcare and education. With the help of our staff , volunteers, and supporters we can help more people access quality healthcare, change lives, and build better futures. ## **Hashim Welfare Hospital** The foundation stone for the Hashim Welfare Hospital was laid on 9 April ## **Chairman’s** review The COVID epidemic has lasted much longer than anyone anticipated and it has had a devastating effect on individuals and communities around the world. ## **How we have managed** some maintenance on the landscaping of the front garden and built four outdoor public toilets. During the first lockdown in March 2020, we briefly closed the eye department but managed to keep the maternity department open to deal with emerg encies. Our staff quickly adapted to the Hospital’s COVID guidelines, and through learning lessons in the first lockdown, we were able to better manage patients during the second and third waves of the virus. ## **Start of new services** During Ramadan 2021, we started five new clinics, which had been planned for some time. These include: 1) paediatrics (2) physiotherapy (3) urology (4) diabetic eye screening and fundus imaging, and (5) ultrasound. Along with the new clinics we have increased the use of technology to track patients, digitise the pharmacy, and help with administration. Also in March 2020, We finally started work on the long-planned hospital extension. However, we were forced to abandon this. The lockdowns have also meant a shortage of building material and a sharp increase in market prices. ## **Adapting to new realities** Meeting people personally, and building friendships and support, has been an essential part of our work. I have missed these interactions. The pandemic has led to cancellation of all our events and meetings. However, the situation has meant that I have gone back to a more personal approach through telephone and email. The added help of WhatsApp and video conferencing means we continue to The team managed to complete some ancillary projects, including; foundation work on the main building, temporary storage rooms for building materials, a space for the security guard, a ‘yellow room’ for clinical waste storage, and a human tissue waste pit. We completed **Dr Idrees Awan** Chairman and founder reach out to our donors, supporters and well-wishers directly. I hope we haven’t inundated you with too many messages! Despite these difficulties, during Ramadhan we received more donations than previous years. I encourage everyone to continue using our JustGiving page. Our German partners PAKISTAN – Hilfe zur Selbsthilfe e.V. (PHzSH) are using video conferencing to supervise staff at HWH with hearing aids for children. We look forward to eventually meeting you all in person, God-willing. 4 **Hashim Welfare Hospital** Annual Review 2020/21 We have performed 12,583 cataract operations since 2003 ## **Departments** and clinics HWH is currently registered with the Punjab healthcare commission as a fifteen-bed hospital. This will increase to fifty beds after the extension is completed. ## **Ophthalmology** This was the first department created when the original hospital opened. The department currently employs two consultant eye surgeons, and runs two weekly eye clinics and theatre sessions. The bulk of clinical work is related to cataract and associated conditions. Cataract surgery is mainly performed with foldable intra-ocular lenses with the option of extra-capsular surgery. Laser therapy for post-cataract surgery is regularly performed. Staff also have access to diabetic retinopathy screening equipment and a optometrist for refractive eye problems who helps during busy eye clinics. We are planning to provide prescription lenses and glasses in the future. ## **Maternity** The maternity department includes neonatal and gynaecology services. We currently employ one consultant gynaecologists during regular hours, and a consultant surgeon on-call. Specialised pre-natal, paediatrics, child welfare, and family planning clinics are being considered for the future. ## **Diagnostics** This is one of our busiest departments because many doctors from other clinics and hospitals send their patients for tests. This department is supervised by Professor Nasir Iqbal from Lahore. We are in discussions with Dr Adnan Ali from the UK, to supervise a new infection control programme at HWH. ## **Hearing disabilities camps** Since 2016, our charity partners PAKISTAN-Hilfe zur Selbsthilfe e.V. (PHzSH) have conducted ear camps at HWH. The team have also been training our staff and will be assisting HWH in developing a new ENT department. Due to the pandemic there were no ear camps in 2020, and non so far in 2021. Audiologists form PHzSH were able to supervise some hearing aid adaptations by HWH staff via video link in March 2021. ## **Dental department** We have appointed Dr Muhammad Ibrar, dental surgeon, to lead the clinic. The number of patients and procedures has increased, as well as an increase of visitors from abroad using the service. ## **New clinics started in 2021** HWH has introduced a weekly paediatrics clinic, sponsored by Dr Shahid Anis Khan, a professor and consultant physician from the UK. Dr Mamoon Ahmed has been appointed as consultant paediatrician. We have a new full-time physiotherapist to manage musculoskeletal disabilities. This clinic had been sponsored and supervised by Dr Tariq Zaman, consultant orthopaedic surgeon in the UK. Dr Taqi Shah, consultant radiologist, runs a weekly ultra sound clinic and Brig. Dr Tariq Mirza, professor of radiology, supervises the radiology department. 6 **Hashim Welfare Hospital** Annual Review 2020/21 ## **Patients’** stories Mohammed Altaf is from Sarai Alamgir, not far from Hashim Welfare Hospital. ## **Muhammad is a 69 years retired non-commissioned officer. He was bought to the hospital today by his grandson.** Mohammad had been to a local hospital in Kharian several times, but there had been no diagnosis of cataracts. He spoke to three people from his village who had cataract surgery at HWH and decided to try here. When Mohammed visited HWH he was prescribed medicine for high blood pressure. He says. ‘I returned for a [follow up]. My blood pressure was normal and I was given a date for the operation.’ About his experience, Mohammed said, ‘On arrival There was no long wait and it was simple to register. I paid the Rs 10 fee and I was given a token with a number. The staff on the ward were excellent and looked after me very well. I paid a total of Rs 4,500. I need to return to have a follow up check. Overall I have had a great experience throughout my treatment.’ ## Our **organisation** The hospital is jointly run by Hashim Welfare Hospital Appeal in the UK and Hashim Welfare Association in Pakistan. ## In carrying out my examination I have followed the Directions given by the Charity Commission under section 145(5)(b) of the 2011 Act. ## **Independent examiner's statement** I can confirm that I am qualified to undertake the examination because I am a registered member of Association of Chartered Certified Accountants which is one of the listed bodies. Accounting convention** The financial statements are prepared under the historical cost convention and in accordance with the Financial Reporting Standard for Smaller Entities (effective June 2002) and follow the recommendations in 'Accounting and Reporting by Charities: Statement of Recommended Practice' issued in October 2000. ## **1.2. Incoming resources** Income is recognized in the period in which the charity is entitled to receipt and the amount can be measured with reasonable certainty. ## **1.3. Resources expended** Resources expended are recognized in the year in which they are incurred. The Executive Members have regular meetings, and all the Executive Members are responsible for all decisions taken in relation to the running of the charity and the other activities provided by the charity. ## **Recruitment and Appointment of Executive Members** It has been followed according to the Constitution. ## **Risk Management** The Executive Members have assessed the risks the charity faces and have identified the major risks areas and have drawn contingency plans to deal with those risks. The Executive Members are satisfied that systems are in place, or arrangements are in hand, to manage the risks that have been identified ## **Objectives and Activities** We have referred to the guidance contained in the Charity Commission's general guidance on public. benefit when reviewing our aims and objectives and in planning our future activities. The objectives of the charity are set out in the charity's Constitution. ## **Financial Review** ## **Reserves Policy** The Executive Members have reviewed the reserves of the charity. In preparing these financial statements, the Executive Members are required to: - Select suitable accounting policies and then apply them consistently. - Make judgments and accounting estimates that are reasonable and prudent. - Prepare the financial statements on the going concern basis unless it is inappropriate to presume that the charity will continue in business. - The Executive Members are responsible for keeping adequate accounting records that are sufficient to show and explain the charity's transactions and disclose with reasonable accuracy at any time the financial position of the charity and enable them to ensure that the financial statements comply with the Charities Act 2011, the applicable Charities (Accounts and Reports) Regulations, and the provisions of the Constitution. - They are also responsible for safeguarding the assets of the charity and hence for taking reasonable steps for the prevention and detection of fraud and other irregularities. The trustees' annual report was approved on **16/04/2021** and signed by all of the trustees attended This report submitted on behalf of the Executive Members of Hashim Welfare Hospital Appeal UK, by ## **Dr Mohammad Idrees Awan** Chairman 3 ## **Hashim Welfare Hospital Appeal UK Trustee's Report and Unaudited Financial Statements for The Year Ended 31 December 2020** ## **Independent Examiner's Report to the Executive Members of Hashim Welfare Hospital Appeal UK** I report to the Executive Members on my examination of the financial statements of Hashim Welfare Hospital Appeal UK ('the charity') for the year ended 31st December 2020. ## **Responsibilities and basis of report** As the Executive Members you are responsible for the preparation of the financial statements in accordance with the requirements of the Charities Act 2011 (‘the 2011 Act’) Having satisfied myself that the accounts of the Charity are not required to be audited under Part 16 of the 2006 Act and are eligible for independent examination, I report in respect of my examination of the charity’s accounts as carried out under section 145 of the Charities Act 2011 (‘the 2011 Act’). In carrying out my examination I have followed the Directions given by the Charity Commission under section 145(5)(b) of the 2011 Act. ## **Independent examiner's statement** I can confirm that I am qualified to undertake the examination because I am a registered member of Association of Chartered Certified Accountants which is one of the listed bodies. I have completed my examination. I confirm that no matters have come to my attention in connection with the examination giving me cause to believe: 1. accounting records were not kept in respect of the charity as required by section 386 of the 2006 Act; or 2. the financial statements do not accord with those records; or 3. the financial statements do not comply with the accounting requirements of section 396 of the 2006 Act other than any requirement that the accounts give a ‘true and fair' view which is not a matter considered as part of an independent examination: or 4. the financial statements have not been prepared in accordance with the methods and principles of the Statement of Recommended Practice for accounting and reporting by charities applicable to charities preparing their accounts in accordance with the Financial Reporting Standard applicable in the UK and Republic of Ireland (FRS 102).
