pand&mic risk$.
Working Ythhin govemment COVID-19 gUh4d1n￿ w¥ wèr• able to reinstate many of Ouf ¢linkal services but
with reslricled numbers to albw for •degu¥t¢ $c¢ial distancing. Ouf daycare Hospice serwce waB reduced to 4
days a week, the chronic obstru¢lNe, pulm*)nary diseasè ¥nd heart failure program cne day per week and our
Hospice al Hcffle cThilinued to cover 365 d¥ys of th¥ year.
Collaboration betNeen the Hospice at Home team and the distri¢t nursin9 team enabled us lo ¢ontinu• to
provide nights of care over the per*)ts and this ha8 helpwj • ￿nth¢Onl nUM￿r of paI￿ntS to remain at htsme
towards the end of their INes.

were incorporated into the new ways of working to $Lyport paiients and their carers who faGed a great deal of
uncertainty and disrupts'on.

patients a￿ $af$ and secure. Throughout the period and lo dale there have bo&n zero positNe Covid case$,
which is a t¥stament to the load•rship of the Nurse Manager.
