## Provider Agreement Form 

Somerset's Local Provider Agreement 2026, effective from 01 April 2026, is published online at Early Years Entitlement information for providers and sets out the terms and conditions that providers must abide by to offer the entitlements for government funded childcare places. A summary of the areas where the Agreement has been updated can be found using this link also. 

All providers offering funded childcare must abide by the terms and conditions set out in the Provider Agreement. By signing and returning this form, you agree to abide by the terms and conditions of the Provider Agreement and the declarations below. Please read Somerset's Local Provider Agreement 2026 and the declarations below carefully and add name, role, signature and date where indicated. Group providers should add the name of the setting as well. This can be done directly onto the form using the edit tools or by printing, signing and scanning or photographing the form. Please note: a printed signature will not be accepted. The signature must be written or a scanned version. Please return your signed form to educationplaces@somerset.gov.uk by Wednesday 25" March 2026.
